Retinal hemorrhages occur with increased frequency in neonates following prolonged or difficult labor, especially if associated with primipara, surgical or mechanical intervention, low Apgar scores, perinatal hypoxia, low birth weight or coagulopathy. When involvement occurs peripheral to the arcades resolution occurs generally without sequelae. Hemorrhages involving the macula are much more serious since resolution in these cases may result in degenerative changes or an exudative or glial scar, resulting in amblyopia and possible strabismus. Thirty-five patients having paretic vertical strabismus were evaluated to determine which clinical parameters could best be used to differentiate congenital or early onset vertical strabismus from late onset vertical strabismus. Those patients with late onset deviations had a higher incidence of diplopia, a higher incidence of cyclodeviation, a higher incidence of compensatory head posture, and manifested larger vertical deviations in the primary position. Simple congenital ptosis is the most common form, characterised by fatty dystrophy and fibrosis of the levator muscle, which leads to a restricted elevation in upgaze and a lid lag in downgaze. The early development of vision in an eye with a dislocated lens is often poor, leading to disappointing postoperative results due to amblyopia. A retrospective review of the best corrected visual acuity of 49 eyes of 27 children with ectopia lentis, who were rendered aphakic either spontaneously or by lensectomy, showed a gradual improvement. Nineteen eyes achieved an early postoperative acuity of 6/12 or better. Of the remaining 30 eyes, 21 were followed up for 1 or more years. During this period, an improvement of two or more Snellen visual acuity lines was noted in all but one of the eyes.

Epikeratophakia for keratoconus was done in 11 mentally retarded patients, using fresh, free-hand made lamellar grafts. The mean follow-up period was 31.5 months. The grafts remained clear and well adapted in 7 cases (63.6%). Necrosis of the lower part of the graft occurred in 3 eyes (27.3%), and was probably caused by persisting epithelial defects. Neonatal cataract surgery is becoming more common because irreversible amblyopia occurs if an eye receives inadequate inputs during an early sensitive period of visual development. To model conditions for treating congenital monocular cataracts in humans, we have reared rhesus monkeys which underwent unilateral lensectomy as neonates. A 4-year-old boy with trisomy 21 and a congenital cataract underwent cataract extraction and implantation of an iris-supported intraocular lens (IOL). Four years later the implant dislocated anteriorly and had to be removed. Now, 11 years postoperatively, the eye is blind, painful and shrunken.
