Subarachnoid Block-Induced Deafferentation Pain Successfully Treated with Pentazocine.

Deafferentation pain induced by subarachnoid block (SAB) is rare, but it can appear in the form of recurrent phantom lower limb pain, new acute-onset stump pain in amputees, lower limb pain in patients with tabes dorsalis, and neuropathic pain. We have previously reported that thiopental is an effective treatment for deafferentation pain induced by therapeutic SAB applied to treat neuropathic pain of central origin. Here, we report the case of an amputee who developed new stump pain in his lower limb immediately after subarachnoid tetracaine was administered prior to appendectomy. A 51-year-ol…

Non-operative management of early, acute appendicitis in children: is it safe and effective?

PURPOSE: The purpose of this study was to determine if early, acute appendicitis in children can be safely and effectively managed with antibiotics alone. METHODS: A retrospective review was performed of children (<18 yrs) treated non-operatively (NOM) for early, acute appendicitis since May 2012. These were compared to patients treated with appendectomy between January 2011 and October 2011 (OM). Inclusion criteria included: (a) symptoms <48 h, (b) localized peritonitis, and (c) ultrasound findings consistent with early, acute appendicitis. RESULTS: Twelve patients (66% female, mean age 12.…

Tubal endometriosis diagnosed within one month after menarche: a case report.

A 13-year-old nulligravida girl, 158.5 cm in height and 76.0 kg in body weight, came to our department complaining of continuous right lower abdominal pain. One month earlier, an ovarian cyst in the right ovary, about 3 cm in diameter, was found when she underwent appendectomy at another hospital, but was left untreated. Menarche occurred at the age of 13 years and 1 month, which was after the appendectomy and 24 days before the present operation. Right hematosalpinx with peripheral obstruction and a para-ovarian serous cyst on the same side were diagnosed, and therefore right salpingectomy wi…

Analysis of risk factors for unplanned readmission following acute appendicitis surgery in children.

BACKGROUND: Acute appendicitis (AA) is the most common acute abdominal condition in children. The rate of unplanned readmission within 30 days after appendectomy ranges from 4% to 13%, significantly increasing the healthcare burden and affecting patient outcomes. Identifying independent risk factors for readmission is crucial for optimizing management of high-risk pediatric patients; however, relevant evidence in the Chinese pediatric population remains limited. METHODS: This study employed a single-center retrospective cohort design, enrolling 5,016 pediatric patients who underwent appendect…

Utilization and Performance Benchmarking for Postoperative Imaging in Children With Complicated Appendicitis: Results From a Multicenter Collaborative Cohort Study.

OBJECTIVE: The aim of this study was to characterize hospital-level variation and establish diagnostic performance benchmarks for postoperative imaging in children with complicated appendicitis. SUMMARY BACKGROUND DATA: Wide variation in preoperative imaging in children with suspected appendicitis has been previously described. Variation in the use and accuracy of postoperative imaging to diagnose suspected organ space infection (OSI) following appendectomy has not been characterized. METHODS: Multicenter retrospective analysis of children who underwent appendectomy for complicated appendici…

Appendicitis Perforated by Endometriosis in a 50-Year-Old Woman: A Case Report.

People often experience pain in the lower right side of the abdomen due to appendicitis, which is a frequent issue for both men and women. E … senting with acute right lower quadrant pain, diagnosed with complicated appendicitis. Laparoscopic appendectomy was performed, and histopathology revealed appendiceal endometriosis at the site of perforation. Appendiceal endometriosis remains a rare and often unsuspected cause of appendicitis, posing diagnostic and surgical challenges.

Atypical perforated appendicitis secondary to schistosomiasis: a rare case report.

INTRODUCTION: Schistosomiasis is the second most common parasite disease after malaria and a rare cause of appendicitis. It has been well-documented in the literature that schistosomiasis infection can have several multisystem effects. The unusual condition known as schistosomal appendicitis was initially described by Turner. CASE PRESENTATION: We present a 48-year-old man with perforated appendicitis and schistosomiasis-like radiological findings. An appendectomy was performed, and schistosomiasis was confirmed by a histological study of the excised appendix. Praziquantel was administered in…

Acute appendicitis presenting as an abdominal wall abscess: A case report.

INTRODUCTION: Amyand hernia (AH) is a rare type of hernia characterized by the presence of appendix vermiformis in the inguinal hernial sac. It is rarely reported in women. PRESENTATION OF CASE: We presented a case of a 60- year old woman who was admitted initially with an abdominal wall abscess and found to have perforated appendix in the right inguinal hernia. The patient underwent standard open appendectomy and the post-operative course was uneventful. DISCUSSION: The initial presentation of our case as an abdominal wall abscess is rare in the contemporary literature. CONCLUSIONS: A high…

Mixed Adenocarcinoid Tumor Presenting as Acute Appendicitis.

Mixed adenocarcinoid tumors are not uncommon neoplasms of appendix. The clinical presentation of these tumors is often similar to that of acute appendicitis or may present as asymptomatic. These tumors are found incidentally during histopathological examination of the resected appendix following appendectomy or other abdominal procedures. Mixed adenocarcinoids usually behave as adenocarcinomas with rapid metastasis, so prognosis depends upon how aggressive the tumor behaves. The present study reports a case of a 53-year-old male who presented with abdominal pain and fever for 1 day and underwe…

Embryonal rhabdomyosarcoma of the cervix and appendiceal carcinoid tumor.

BACKGROUND: Rhabdomyosarcomas, particularly those of gynecologic origin, are very rare in adults. As a result, there is little literature on the optimal staging procedure and treatment modalities for this population. CASE: A 43-year-old woman presented with a long-standing history of menorrhagia and was subsequently diagnosed with embryonal rhabdomyosarcoma of the cervix. She underwent a total abdominal hysterectomy, bilateral salpingo-oophorectomy, lymph node dissection, omentectomy, and appendectomy, which also revealed a synchronous tubular carcinoid tumor of appendiceal origin. Plans for …

Stump appendiceal abscess after appendicectomy: A case report.

INTRODUCTION: Stump appendicitis, an inflammation of the residual appendix after appendicectomy, presents with all the clinical features of acute appendicitis. This variant poses diagnostic challenges, particularly in patients with right iliac fossa pain. CASE PRESENTATION: We present a case of a 58-year-old man who developed an appendiceal stump abscess eight months after laparoscopic appendectomy. In addition to stable vital signs, abdominal examination revealed pain in the right lower quadrant. The CT scan showed a collection with a tubular structure corresponding to a remaining appendicea…

Combined Use of Laparoscopy and an Open Inguinal Approach for Repair of a De Garengeot Hernia.

Femoral hernias carry an increased risk of incarceration. De Garengeot hernia, a rare subset, occurs when the appendix herniates through the femoral canal. Due to its rarity, various surgical approaches have been explored, including isolated groin incisions, combined approaches, and exclusive laparoscopic interventions. This case involved a 58-year-old female diagnosed with a De Garengeot hernia and nonperforated acute appendicitis, managed through a combined laparoscopic and an inguinal approach, and underwent laparoscopic appendectomy and open repair of femoral hernia using a biologic mesh. …

A case of acute appendicitis caused by obstruction due to caecal cancer.

Acute appendicitis supposedly results from appendix obstruction caused by various conditions, including caecal cancer. Here, we report an adenocarcinoma found in the specimen of a 65-year-old man diagnosed with acute appendicitis. The adenocarcinoma was detected in the appendix stump after emergency laparoscopic appendectomy. The patient was diagnosed with caecal cancer based on post-operative lower endoscopy findings, and an additional laparoscopic right hemicolectomy was performed. Acute appendicitis can occur at any age and may require emergency surgery. It is a benign disease that can be c…

An Atypical Presentation of a Pyogenic Liver Abscess Caused by Fusobacterium nucleatum Post-ruptured Appendix.

A rare formation of a pyogenic liver abscess can be found in patients with a ruptured appendix. Here, we discuss a 49-year-old male with a past medical history significant for drug-induced pancreatitis from metformin, type II diabetes, obesity, and hypertension who presented with right upper quadrant pain. He was diagnosed with a 12.6 cm pyogenic liver abscess. Upon further chart review, the patient had a laparoscopic appendectomy done nine months ago, with the findings of a ruptured appendix. The liver abscess cultures grewFusobacterium nucleatum - a common culprit of acute appendicitis repor…

Primary signet ring cell carcinoma of the appendix: a rare case report and our 18-year experience.

Primary adenocarcinoma of the appendix is a rare malignancy that constitutes < 0.5% of all gastrointestinal neoplasms. Moreover, primary signet ring cell carcinoma of the appendix is an exceedingly rare entity. We have encountered 15 cases of primary appendiceal cancer among 3389 patients who underwent appendectomy over the past 18 years. In the present report, we describe a rare case of primary signet ring cell carcinoma of the appendix with ovarian metastases and unresectable peritoneal dissemination occurring in a 67-year-old female patient. She underwent appendectomy and bilateral salpingo…

Appendiceal Mucocele Presenting as a Cecal Bulge on Surveillance Colonoscopy.

Appendiceal mucoceles are rare tumors with diverse presentations and clinical implications. Generally, mucoceles are discovered on imaging or intraoperatively, but, rarely, can be found on colonoscopy. Appendectomy is the recommended next step in management, followed by subsequent treatment according to guidelines dictated by pathology findings. We present the case of a 52-year-old female undergoing routine colonoscopy screening who had incidental findings of a cecal bulge at the appendiceal orifice. The patient was entirely asymptomatic with an unremarkable medical history. Further workup sug…

Balancing a Surgical vs. Non-surgical Approach in Perforated Appendicitis.

Appendicitis is the inflammation of the appendix, one of the common causes of acute abdomen that may indicate emergency abdominal surgical i … tics and percutaneous drain placement. Although a conservative, non-surgical approach with interval appendectomy has become more of the popular approach in developed countries for complicated appendicitis due to increased morbidity in immediate appendectomy due to inflammation-associated adhesions, we wanted to present a case where a conservative approach can …

Incidental Findings of Appendiceal Diverticulitis Presenting as Acute Appendicitis.

Diverticulosis of the appendix (DA) is a rare pathological finding that clinically imitates acute appendicitis and is most commonly found in adult males with chronic abdominal pain. It has a higher rate of perforation compared to acute appendicitis (84% vs 12%, P<0.01), and is consequently associated with a higher rate of mortality. Appendiceal diverticulitis has been found to have a significant association with incidental appendiceal neoplasms, therefore elective prophylactic appendectomy is recommended to prevent the risk of complications and to rule out the possibility of a coexisting neopl…
