Self-generated lower body negative pressure exercise.

BACKGROUND: Exercise during spaceflight helps prevent musculoskeletal and cardiovascular deconditioning to Earth gravity. This report evaluates the aerobic and anaerobic exercise stimulus provided by self-generated lower body negative pressure. METHODS: A lower body negative pressure cylinder expands and collapses longitudinally, but not radially. As the legs push footward to expand the cylinder, the air pressure in the cylinder decreases, increasing the force required to continue expanding the cylinder. In addition, valves control air flow into and out of the cylinder, and thus workload. In

Acute carbamate and organochlorine toxicity causing convulsions in an agricultural pilot: a case report.

Agricultural pilots are involved in a particularly hazardous occupation with a high occupational mortality rate. They face many risks, not least of which is exposure to the agricultural chemicals that they spray. Many accidents and illnesses have been reported previously in the literature, attributable to the acute and chronic effects of chemical exposure, most notably the organophosphate family of insecticides. There are few reports of post-crash exposure to carbamate and organochlorine insecticides. This report documents the case of a professional agricultural pilot who, following an emergen

Acceleration of lupus erythematosus-like processes by low frequency noise in the hybrid NZB/W mouse model.

INTRODUCTION: We investigated the effect of low frequency noise on the natural history of autoimmune diseases using hybrid New Zealand Black (NZB) and White (NZW) male mice that are prone to spontaneously develop systemic lupus erythematosus after 6 mo of age. METHODS: We exposed 3-mo-old mice to periods of low frequency noise (> or = 90 dB SPL, < or = 500 Hz) for a total of 216 h; the animals were studied up to 12 mo of age and data were compared with results obtained with age-matched controls. RESULTS: We found that noise exposure caused an earlier expression of proteinuria, a change that

Cerebral artery blood velocity in normal subjects during acute decreases in barometric pressure.

To investigate the effect of acute changes in barometric pressure on regional cerebral perfusion we studied the middle cerebral artery (MCA) blood velocity in five healthy male volunteers by means of a low-pressure chamber. The MCA blood velocity, arterial blood and respiratory gases were measured at the barometric pressures of 1, 0.8, 0.65, and 0.5 atmospheres. The observed blood velocity (Vo) showed no systematic changes. Decreases in barometric pressure induced hypoxia and hypocapnia. When normalizing the MCA blood velocity (Vn) to a standard P(CO2) (5.3 kPa), thereby correcting for the hyp

Hyperhomocysteinaemia and upper extremity deep venous thrombosis: a case report.

A case is presented of a 24 yr old military aircrew applicant who developed a right axillary subclavian deep venous thrombosis following physical exertion. Investigations revealed damage to the right axillary subclavian venous system and limitation to flow. Coagulation studies also showed an elevated plasma homocysteine level. Hyperhomocysteinemia has recently been recognized as a risk factor for venous thromboembolic disease. Damage caused by the thrombosis, the hyperhomocysteinemia and environmental factors encountered in flight, may predispose him to recurrent episodes of thrombosis. This c

The use of an independent visual background to reduce simulator side-effects.

BACKGROUND: Simulator sickness (SS) is a major problem which potentially limits interface applications that feature simulated motion. While display imperfections play a role, a large part of SS is motion sickness (MS). Sensory rearrangement theory holds that MS is related to conflicting motion cues; in the case of simulators, mainly a conflict between inertial cues (usually indicating no self-motion) and visual stimuli from the display (indicating self-motion). It is suggested that MS does not arise from conflicting motion cues per se, but rather from conflicting rest frames selected from thos

The retention of adaptation to motion sickness eliciting stimulation.

PURPOSE: This study compared the retention of adaptation to motion sickness eliciting stimulation after 1 mo and 1 yr. METHOD: A total of 34 subjects who developed nausea to an optokinetic rotating drum in a preliminary session repeatedly viewed the same rotation with an interval of 48 h between sessions until they had no feelings of nausea. For each session, subjects' electrogastrograms (EGGs) at 4-9 cycles per minute (cpm) were also recorded as a physiological index of the severity of motion sickness. Subjects were re-exposed to the optokinetic drum either after 1 mo or 1 yr. RESULTS: The

Efficacy of air and liquid cooling during light and heavy exercise while wearing NBC clothing.

BACKGROUND: Studies, to date, have not revealed the extent to which the heat strain of continuous heavy exercise while wearing NBC protective clothing can be reduced by providing liquid- or air-cooling and whether one system is more effective than the other in aiding heat transfer from the body and the clothing. It was of interest to know to what extent present-day cooling technologies can reduce the heat strain of light and heavy exercise in NBC clothing and to compare these reductions to other strategies that have employed changes in clothing design. HYPOTHESIS: It was hypothesized that the

ERP P300 and brain magnetic resonance imaging in patients with vibroacoustic disease.

INTRODUCTION: Subjects occupationally exposed to large pressure amplitude (> or = 90 dB SPL) and low frequency (< or = 500 Hz) (LPALF) noise for long term periods (>10 yr) can develop vibroacoustic disease (VAD). One of the earliest complaints of VAD patients deals with memory and attention disturbances. On the other hand, in over 50% of these patients, magnetic resonance imaging (MRI) of the CNS revealed cortical atrophy and dilation of the perivascular spaces, and small hyperintense lesions in T2 of the cerebral white matter, basal ganglia, and brainstem. The purpose of this study is to inve

Electrical and contractile parameters of muscle in man: effects of 7-day "dry" water immersion.

HYPOTHESIS: The purpose of this experiment was to test the hypothesis that the changes in single muscle twitch tension (Pt) in a cosmonaut following exposure to long-duration microgravity may have been due to the sarcoplasmic reticulum, while the decrease in maximal voluntary contraction (MVC) relative to the electrically evoked maximal tetanic tension (Po) may reflect a reduction in control neural drive resulting from the voluntary muscle inactivity. METHODS: The effects of 7-d "dry" water immersion were investigated in six subjects. Changes of contraction properties were studied in the tric

Sister chromatid exchange analysis in workers exposed to noise and vibration.

BACKGROUND: There has been a growing interest in the combined effects of noise and vibration. In a population of aeronautical workers diagnosed with vibroacoustic disease (VAD), a large incidence of malignancy was detected. These workers were exposed to large pressure amplitude (LPA) (> or = 90 dB SPL) noise, with energy content concentrated within the low frequency (LF) bands (< or = 500 Hz) and whole-body vibration (WBV). To our knowledge, there are no studies conducted in humans or animals that address the issue of the potential genotoxic effects of vibration combined with noise. In the pre

Lidocaine pharmacokinetics during hyperbaric hyperoxia in humans.

METHODS: The disposition of drugs may be influenced by hyperbaric conditions, in particular by changes of liver perfusion. The effect of hyperbaric hyperoxia on the pharmacokinetics of lidocaine, a drug eliminated in the liver with a perfusion-limited clearance, was investigated in human volunteers in a crossover trial. METHODS: A single dose lidocaine i.v. bolus (0.69 or 0.75 mg x kg(-1)) was administered to two volunteers under normobaric conditions (NB: 1 bar or 0.1 MPa, air) and under hyperbaric/hyperoxic conditions (HBO: 2.5 bar or 0.25 MPa, alternating 100% O2-breathing for 20 min and a

Coccidioidomycosis--the airborne assault continues: an unusual presentation with a review of the history, epidemiology, and military relevance.

Despite remarkable advances in detection and therapy, coccidioidomycosis remains a persistent threat to military troops deployed in endemic areas. Pregnant women, immunocompromised hosts, and dark-skinned persons, particularly those of Filipino, African, Hispanic or Asian ancestry, are at greatest risk for disseminated coccidioidomycosis. The ethnically diverse military forces have susceptible active duty and reserve members stationed at or temporarily trained on bases located in endemic areas for Coccidioides immitis. Although the vast majority of infections with this organism are subclinical

Effects of exercise training mode on the cardiovascular responses to lower body negative pressure in males.

BACKGROUND: Endurance-trained males may be more prone to orthostatic hypotension than untrained subjects and this is reflected in differences in their cardiovascular responses to lower body negative pressure (LBNP). It is uncertain if the type of endurance training used affects these responses. HYPOTHESIS: Endurance-trained runners will differ from endurance-trained swimmers in their cardiovascular responses to LBNP. METHODS: Male intercollegiate cross country runners (XC, n = 9), sprinters (SP, n = 7) and swimmers (SW, n = 12) underwent exposures to -10, -20 and -40 mm Hg LBNP. Forearm bloo

Carotid artery dissection presenting as a painless Horner's syndrome in a pilot: fit to fly?

We describe a case of a middle-aged Caucasian pilot who presented to us with a painless left Horner's syndrome due to a focal dissection of the infra-petrous portion of the ipsilateral internal carotid artery. He did not suffer symptoms of cerebral ischemia at the time of onset, or during the following 2 yr. The Horner's syndrome persisted unchanged throughout follow-up. Serial magnetic resonance angiograms showed some regression of the focal stenosis at 12 mo, with no further change over the next 10 mo. The literature suggests that the risk of stroke after onset of dissection is usually in th

Effect of hyperbaric oxygen on intercellular adhesion molecule-1 (ICAM-1) expression in murine lung.

BACKGROUND: The lung is one of the most susceptible organs to the toxic effect of high pressure oxygen. However, the contribution of the cellular adhesion molecules and inflammatory leukocytes in the pulmonary oxygen toxicity, especially in an acute exudative phase, is not well understood. In the present study we have investigated the toxic effect of hyperbaric oxygen (HBO) expressed by the analysis of the intercellular adhesion molecule-1 (ICAM-1) on the pulmonary vascular endothelial cells and leukocyte function. METHODS: Mice were exposed to HBO at 3 ATA of PO2 for 3, 4, or 6 h. Expression

Effect of G suit type on cognitive performance.

BACKGROUND: Sustained acceleration protection ensembles are being developed to help pilots of high performance aircraft endure high G exposures for longer periods of time. It has been assumed that better G endurance confers better pilot task performance. This premise was studied on the Armstrong Laboratory Dynamic Environment Simulator centrifuge. METHOD: Human subjects repeatedly endured prolonged high-G simulated aerial combat on a centrifuge to the point of loss of vision or physical exhaustion. Some profiles included over 20 exposures to +9 Gz. While enduring the G exposures, subjects tra
