Multiple intracranial tubercular abscesses in a child.

2 Year old girl presented with fever and cough for 4 months. Neurological examination revealed right sided hemiplegia. Contrast enhanced computed tomography of brain showed multiple hypodense ring enhancing lesions. Pus on aspiration of intracranial abscess showed positive polymerase chain reaction for Mycobacterium tuberculosis. A possibility of tuberculosis though not commonly recognised may be considered in any child presenting with prolonged fever and multiple intracranial abscesses.

Fungal neuroinfections: rare disease but unacceptably high mortality.

Within last 25 years we have observed 20 cases of fungal meningitis and/or cerebral abscesses. Commonest etiologic agens was Candida spp. (C. albicans 9 of 20). Molds were responsible for 4 cases of brain abscess. Mortality was 50% what seems to be very high. Extremely high mortality is caused by delayed onset of therapy, severe underlying disease and multiresistant fungal organisms such as Mucorales, Fusarium solani and Aureobasidium.

An Unusual Case of Perirenal Abscess Caused by Campylobacter and Peptostreptococcus Species.

Intra-abdominal abscesses mostly derive from the intra-abdominal viscera. Campylobacter spp. are Gram-negative rods which are known to cause oral infections but rarely have been documented to cause extra-intestinal infections resulting in abscesses. We report an atypical case of Campylobacter rectus (C. rectus) and Peptostreptococcus spp. bacteria isolated from a perinephric abscess presenting as abdominal pain. Abscesses originating from outside the gastrointestinal tract have been reported in other similar case reports infecting the head, brain, and thoracic wall amongst others. The potentia

A rare case of childhood-onset systemic lupus erythematosus associated end-stage renal disease with cerebral abscess and hemorrhage.

Systemic lupus erythematosus (SLE) is a chronic autoimmune inflammatory disease that affects multiple organs. More than half of the patients with SLE have kidney involvement, and up to 10% of patients with lupus nephritis develop end-stage renal disease (ESRD). Central nervous system (CNS) involvement in SLE occurs in 21% to 95% of patients. Severe neurological manifestations such as seizures, cerebrovascular disease, meningitis, and cerebrovascular accidents can develop in childhood-onset SLE, but cerebral infections, such as brain abscess and hemorrhage, are seldom reported in lupus nephriti…

Miliary cerebrospinal lesions caused by Nocardia beijingensis in an immunocompetent patient.

The genus Nocardia usually infect immunocompromised patients. Pulmonary nocardiosis is the most common Nocardia-induced infection while central nervous system (CNS) is the most common extrapulmonary site to develop nocardial disease. Approximately 54 % cases of previously reported nocardial brain abscesses were solitary lesions, while 38 % cases had multiple lesions; but miliary-like ring enhancing lesions have not been reported previously. We present a case of immunocompetent patient with miliary-like cerebrospinal lesions caused by Nocardia beijingensis. A 68-year-old Japanese man presented …

Ventriculitis: A Severe Complication of Central Nervous System Infections.

BACKGROUND: Ventriculitis, a dreaded complication of brain abscess, meningitis, and various neurosurgical procedures, has attracted limited attention in the medical literature. METHODS: This is a retrospective, single-center study. We screened the medical files of all patients who had a brain imaging report that included the word "ventriculitis" during years 2005-2019. Only patients with clinical, microbiological, and imaging features of ventriculitis were included. Data were collected through a standardized questionnaire. RESULTS: Ninety-eight patients fulfilled inclusion criteria: 42 women…

Capnocytophaga sputigena Causing Complicated Sinusitis With Intracranial Involvement in a Pediatric Patient.

This report presents a rare case of acute bacterial rhinosinusitis with orbital and intracranial complications caused by Capnocytophaga sputigena in a pediatric patient. A 15-year-old male presented with orbital cellulitis, acute sinusitis, and meningismus. Brain imaging showed evidence of early intracranial abscess formation and areas of cerebral infarction. He underwent urgent endoscopic sinus surgery (ESS) and drainage of an orbital subperiosteal abscess. This case underscores the critical role of timely diagnostic imaging, multidisciplinary care, appropriate surgical management, and effect

Case Report: Pott's Edematous Tumor: Complicated Frontal Sinusitis - An Unremembered Diagnosis.

Pott's Puffy tumor, also called Pott's edematous tumor (PET), is a subperiosteal abscess of the frontal bone, associated with osteomyelitis of the frontal bone. In this paper, we report the case of a 16-year-old patient who presented with headache associated with progressive forehead swelling and fever. Clinical and imaging exams pointed to the hypothesis of PET associated with brain abscess. Patient was submitted to surgical excision of the abscess and treatment of osteomyelitis, with intraoperative findings corroborating the condition. There was a good clinical-radiological recovery associat…

Fungal endophthalmitis in a case of granulomatosis with polyangitis.

A 70-year-old immuno-compromised man, due to multiple comorbidities, particularly granulomatosis with polyangitis (GPA) and its related treatment, presented with generalized weakness, odynophagia and loss of taste sensation. After a complete evaluation, a diagnosis of right frontal lobe brain abscess was made. The patient then developed headache and sudden painful loss of vision in the right eye. Clinical examination revealed anterior chamber cells and flare, vitreous haze and cells, and hemorrhagic chorioretinitis with severe vasculitis in the right eye. Culture from the drained pus of the fr…

[Cerebral abscesses. Therapeutic attitude].

The surgical treatment of cerebral abscesses has changed over recent years and consists, in most cases, of aspiration based on the data supplied by computerized tomography. The purpose of aspiration is threefold: to avoid a major surgical operation, to empty the abscess and thereby reduce its size, and to identify the responsible germ(s). A retrospective study of 35 cases shows that abscess of the brain remains a serious disease that jeopardizes the patient's vital and functional prognosis. Despite improvements in sample collection and isolation techniques, no microorganism is found in 30% of

The Characteristic "Tunnel Sign" on MRI as a Diagnostic Marker of Tunneling Abscesses in Listeria Rhombencephalitis in a Patient With Multiple Myeloma: A Case Report.

Neurolisteriosis is a rare but potentially serious CNS infection. Diagnosis can be established through bacteriological studies or by identifying characteristic tunneling abscesses in the CNS. Delayed initiation of appropriate antibiotic therapy may result in permanent structural brain damage, disability, or death. We report the case of a 65-year-old woman with multiple myeloma undergoing chemotherapy who presented with an acute febrile illness accompanied by drowsiness, slurred speech, difficulty swallowing, neck stiffness, and complex ophthalmoplegia. MRI of the brain demonstrated characteris

Culture isolation of Acanthamoeba species and leptomyxid amebas from patients with amebic meningoencephalitis, including two patients with AIDS.

Acanthamoeba species and leptomyxid organisms are free-living amebas that cause meningoencephalitis, primarily in immunocompromised patients. We report the isolation and culture of Acanthamoeba species and leptomyxid amebas from four patients with fatal amebic meningoencephalitis. Acanthamoeba species were cultured from brain abscess specimens from three immunocompromised patients (including two patients with AIDS). In the case of the fourth patient, who had no identifiable immunodeficiency, leptomyxid amebas were cultured from a specimen from a subcutaneous nodule and were identified in amebi…

Primary cerebellar nocardiosis. Report of two cases.

The authors describe two cases of primary cerebellar nocardiosis. Confinement of Nocardia abscesses to a localized, accessible portion of the central nervous system should favor surgical cure of this aggressive and often fatal disease. In our two cases multilocularity, tenuous encapsulation, and proximity to the brain stem prevented complete primary excision. Nevertheless, the infections were effectively treated by intensive postoperative antibiotic therapy and, in one case, a second operation to excise the residual abscess. The need for the latter was suggested by the results of sequential co

[Two cases of cerebral aspergillosis with intracerebral hemorrhage].

Two patients who had cerebral aspergillosis with massive intracerebral hemorrhage were presented. Case I was a 59-year-old woman who had slight mental retardation. There was swelling in the left forehead, from which mucosal cysts of frontal sinus had been removed 2 years before her admission. She had a diagnosis of subdural abscess and radical operation was performed. Aspergillus was found in the abscess histologically. Three months after the operation, CT scan revealed multiple abscess in bilateral frontal lobe. When she lost consciousness suddenly 4 months after the operation, CT scan showed

Multiparametric imaging in the evaluation of intracerebral abscesses.

Cerebral abscesses are uncommon space occupying lesions; they are associated with high morbidity and mortality, though are potentially treatable. Patients often present with non-specific symptoms and may have few clinical signs. Routine clinical imaging may not give a definite diagnosis, as the findings can be indistinguishable from those of other intracranial mass lesions. We review the role of advanced MR techniques to characterise brain abscesses and discuss the role of imaging in monitoring their response to the treatment.

Prenatal and Neonatal Infections of the Central Nervous System.

Bacterial, fungal, and viral infections of the central nervous system of the newborn are uncommon but severe and can result in devastating brain injury. These pathogens have their own pattern of brain lesions. Cranial ultrasound and MRI can, therefore, already suggest the causative pathogen. Brain lesions can evolve and repeat neuro-imaging is important to either diagnose abscess formation, cystic evolution, or hydrocephalus. Outcome varies considerably among the different pathogens, and long-term follow-up is recommended.
