Cerebral control of saccades and neuropsychological test results after head injury.

Abnormalities in the control of saccades have been described in patients with cerebral pathology.(1, 2) We assessed control of visually guided, reflexive and volitional saccades in 16 patients suffering severe traumatic brain injury and 12 controls and related the results to deficits on neuropsychological tests of speed of information processing and goal directed behaviour. All saccadic latencies were prolonged. Suppression of inappropriate saccades was impaired on volitional saccade tests which proved to be more sensitive in identifying impairment of goal directed behaviour than the neuropsyc

Biomechanical properties of the hypoxic and dying brain quantified by magnetic resonance elastography.

Respiratory arrest is a major life-threatening condition leading to cessation of vital functions and hypoxic-anoxic injury of the brain. The progressive structural tissue changes characterizing the dying brain biophysically are unknown. Here we use noninvasive magnetic resonance elastography to show that biomechanical tissue properties are highly sensitive to alterations in the brain in the critical period before death. Our findings demonstrate that brain stiffness increases after respiratory arrest even when cardiac function is still preserved. Within 5 min of cardiac arrest, cerebral stiffne

Neurochemical biomarkers to study CNS effects of COVID-19: A narrative review and synthesis.

Neurological symptoms are frequently reported in patients suffering from COVID-19. Common CNS-related symptoms include anosmia, caused by viral interaction with either neurons or supporting cells in nasal olfactory tissues. Diffuse encephalopathy is the most common sign of CNS dysfunction, which likely results from the CNS consequences of the systemic inflammatory syndrome associated with severe COVID-19. Additionally, microvascular injuries and thromboembolic events likely contribute to the neurologic impact of acute COVID-19. These observations are supported by evidence of CNS immune activat

Head injury in man and experimental animals: clinical aspects.

Clinical studies have demonstrated that, with regard to death, the two worst types of head injury are subdural haematoma (SDH) and diffuse axonal injury (DAI). These two have different mechanisms of causation; SDH occurs much more commonly in non-vehicular injuries, especially falls, while DAI is caused, almost exclusively by vehicular mechanisms. The production of these two types of injury in non-impact acceleration models helps to explain these causal differences, but also shows that both injuries share a common mechanical cause, differing only in degree. SDH is due to vascular injury that i

Expert consensus on the clinical practice of neonatal brain magnetic resonance imaging.

In recent years, magnetic resonance imaging (MRI) has been widely used in evaluating neonatal brain development, diagnosing neonatal brain injury, and predicting neurodevelopmental prognosis. Based on current research evidence and clinical experience in China and overseas, the Neonatologist Society of Chinese Medical Doctor Association has developed a consensus on the indications and standardized clinical process of neonatal brain MRI. The consensus has the following main points. (1) Brain MRI should be performed for neonates suspected of hypoxic-ischemic encephalopathy, intracranial infection

Biphasic edema after hypoxic-ischemic brain injury in neonatal rats reflects early neuronal and late glial damage.

Magnetic resonance imaging with diffusion- and T2-weighted imaging and 31P magnetic resonance spectroscopy was used to investigate the relationship between development of brain edema and alterations of the brain energy metabolism after hypoxia-ischemia (HI) brain injury in 7-d-old rats. The results were correlated with histologic examinations at various times during recovery up to 5 d. Moderate HI, induced by right common carotid artery ligation and subsequent exposure to 8% O2 for 90 min, produced a cytotoxic edema of 52+/-9% brain volume and depressed the ratio of phosphocreatine to inorgani

The relationship between blast-related mild traumatic brain injury and executive function is moderated by white matter integrity.

Blast-related mild traumatic brain injury (BR mTBI) is a critical research area in recent combat veterans due to increased prevalence of survived blasts. Post-BR mTBI outcomes are highly heterogeneous and defining neurological differences may help in discrimination and prediction of cognitive outcomes. This study investigates whether white matter integrity, measured with diffusion tensor imaging (DTI), could influence how remote BR mTBI history is associated with executive control. The sample included 151 Veterans from the Minneapolis Veterans Affairs Medical Center who were administered a cli

Moderate and severe traumatic brain injury: abbreviated injury scale scoring and coding of traumatic axonal injury from early MRI.

BACKGROUND: In this large prospective study of patients with moderate and severe traumatic brain injury (TBI) examined with early magnetic resonance imaging (MRI), we aimed to explore the differences in the occurrence and degree of traumatic axonal injury (TAI) reported on early MRI compared with (1) the original Abbreviated Injury Scale (AIS)-TAI coding and (2) a pragmatic AIS-TAI coding approach. The latter approach also allowed the assignment of TAI codes to patients with short periods of coma, sedated patients and patients with concurrent mass lesions, which are exclusion criteria in the o

Pontine axonal injury after brain trauma and nontraumatic hypoxic-ischemic brain damage.

Experimental studies have shown that diffuse axonal injury is usually induced by positive or negative acceleration mechanisms. In order to determine the reliability of axonal injury (AI) as a marker of this type of traumatic insult, we compared cases of trauma-induced focal cortical hemorrhage without dural involvement (n = 67) with cases of trauma-induced subdural bleeding without cortical hemorrhage (n = 26). Both groups exhibited a wide range of post-traumatic survival times. The injuries in the first group were caused mainly by direct impact to the head, those in the second by acceleration

Reversible focal cerebral cortical lesions in a patient with heat stroke.

We herein report the rare case of a 56-year-old man who suffered from heat stroke. Although he was in a coma with convulsions on arrival and developed multiorgan failure, he recovered after two weeks of successful treatments. Hyperintense signals on the right temporoparietooccipital cortex, which disappeared within one week, were demonstrated on diffusion-weighted magnetic resonance images. A diagnosis of transient cortical injury caused by heat stroke was suggested. Although the cerebellum is most susceptible to lesion formation, the mechanisms underlying heat stroke are multifactorial and ma

[Electromyographic studies of plantar-mental reflex].

The authors investigated the electromyographic response to mechanical stimulation of the palmar skin (palmo-mental reflex) and plantar skin (planto-mental reflex) in the mentalis muscle. The palmo-mental reflex was present in EMG in all 22 healthy subjects an in 51 cases of 51 cases of limited cerebral lesions. The planto-mental reflex was absent in healthy subjects, but it was found in 14 out of 31 cases of focal brain injury and in 2 out of 20 cases with diffuse lesions. The planto-mental reflex is a pathological reflex appearing mainly in severe focal cerebral lesions. No description of thi

Age Moderates the Effects of Traumatic Brain Injury on Beta-Amyloid Plaque Load in APP/PS1 Mice.

Traumatic brain injury (TBI) has been identified as a risk factor for Alzheimer's disease (AD). However, how such neural damage contributes to AD pathology remains unclear; specifically, the relationship between the timing of a TBI relative to aging and the onset of AD pathology is not known. In this study, we have examined the effect of TBI on subsequent beta-amyloid (Aβ) deposition in APP/PS1 (APPSWE/PSEN1dE9) transgenic mice either before (3 months of age) or after the onset (6 months of age) of plaque pathology. Lateral fluid percussion injury (LFPI), a model of diffuse brain injury, was i

Visual-motor deficits relate to altered gray and white matter in young adults born preterm with very low birth weight.

Individuals born preterm and at very low birth weight (birth weight ≤ 1500 g) are at an increased risk of perinatal brain injury and neurodevelopmental deficits over the long term. This study examined whether this clinical group has more problems with visual-motor integration, motor coordination, and visual perception compared to term-born controls, and related these findings to cortical surface area and thickness and white matter fractional anisotropy. Forty-seven preterm-born very low birth weight individuals and 56 term-born controls were examined at 18-22 years of age with a combined cogni

Inflammatory, White Matter, and Neurodegenerative Mechanisms in Fluid Ability Decrements in Chronic Mild-to-Moderate Traumatic Brain Injury.

Traumatic brain injury (TBI) affects millions globally each year, with mild TBI comprising about 75% of cases. While most mild TBI symptoms are resolved within 3 months, some patients experience persistent issues. This study aimed to identify underlying mechanisms contributing to decrements in fluid cognitive abilities in chronic (>6 months) mild-to-moderate TBI. Specifically, the study focused on the relationships between cognitive performance, white matter integrity, TBI-related symptoms, and blood biomarkers, which are thought to be indicative of biological processes including neuronal inju

Multimodal magnetic resonance imaging after experimental moderate and severe traumatic brain injury: A longitudinal correlative assessment of structural and cerebral blood flow changes.

Traumatic brain injury (TBI) is a worldwide problem that results in death or disability for millions of people every year. Progressive neurological complications and long-term impairment can significantly disrupt quality of life. We demonstrated the feasibility of multiple magnetic resonance imaging (MRI) modalities to investigate and predict aberrant changes and progressive atrophy of gray and white matter tissue at several acute and chronic time points after moderate and severe parasagittal fluid percussion TBI. T2-weighted imaging, diffusion tensor imaging (DTI), and perfusion weighted imag

Neonatal rotavirus encephalopathy with white matter injury: A case report.

Rotavirus is a well-recognised cause of gastroenteritis in infants but has increasingly been linked to neurological complications in neonates. We report a 35-week preterm male neonate, the second twin of an in vitro fertilisation pregnancy, who was readmitted on day 9 of life with lethargy and hypotonia in the absence of fever, diarrhoea, or perinatal asphyxia. Brain MRI on diffusion-weighted imaging (DWI) demonstrated bilateral periventricular and deep white matter diffusion restriction resembling periventricular white matter injury seen in preterm infants with hypoxic-ischaemic encephalopath

The role of abnormalities in the corpus callosum in social cognition deficits after Traumatic Brain Injury.

The corpus callosum (CC) is vulnerable to severe traumatic brain injury (TBI). Social cognition requires integration of non-verbal and verbal information in order to understand social behaviour and may be compromised if the CC is damaged. 17 adults with severe, chronic TBI and 17 control participants underwent structural MRI and Diffusion Tensor Imaging. A region of interest analysis examined fractional anisotropy (FA) and mean diffusivity (MD) across regions of the CC. Performance on The Awareness of Social Inference Test (TASIT): part 1 (emotion recognition) and parts 2 and 3 (social inferen

Abnormal dorsal attention network activation in memory impairment after traumatic brain injury.

Memory impairment is a common, disabling effect of traumatic brain injury. In healthy individuals, successful memory encoding is associated with activation of the dorsal attention network as well as suppression of the default mode network. Here, in traumatic brain injury patients we examined whether: (i) impairments in memory encoding are associated with abnormal brain activation in these networks; (ii) whether changes in this brain activity predict subsequent memory retrieval; and (iii) whether abnormal white matter integrity underpinning functional networks is associated with impaired subseq
