Massive calcium pyrophosphate crystal deposition is a rare disorder that may affect the temporomandibular joint and temporal bone. The diagnosis is difficult, and misdiagnosis as malignancy, particularly chondrosarcoma, is frequent. This can lead to unnecessarily aggressive management. The clinical, radiographic, and pathologic features of this entity are reviewed, and treatment options are discussed.

Cyclosporine (CsA) is an immunosuppressor widely used in all postoperative transplantation protocols. Nephrotoxicity and hypertension are the major secondary effects of cyclosporine. The CsA acts on two essential regulatory mechanisms of the blood pressure: the extracellular volume and the systemic vascular resistances. We have used published rat micropuncture data to construct a matrix of ion concentrations along the rat nephron. With an iterative computer model of known ion interactions, we calculated relative supersaturation ratios in all nephron segments. The collecting ducts and urine showed expected supersaturation with stone-forming salts. The effects of trifluoperazine on hepatic cell metabolism were investigated using isolated perfused rat liver. The following effects of trifluoperazine were found: (1) trifluoperazine inhibited oxygen uptake, the site of action being the mitochondria. Half-maximal inhibition occurred at concentrations around 50 microM; with 100 microM trifluoperazine the effect was already maximal. When Ca2+ was withdrawn from the perfusion medium and the intracellular Ca2+ pools were exhausted, the inhibitory action on respiration was no longer observable. Variant angina, first described by Prinzmetal in 1959, is a syndrome of recurrent ischemic-type chest pain that occurs at rest and is associated with ST segment elevation. The syndrome has been convincingly demonstrated to be caused by temporary occlusion at a site of local spasm. We report here that the characteristics of phenothiazine inhibition of exocytosis in neutrophils more closely resemble their interaction with the annexins in vitro. We reported on three hypocalcemic patients with various serum creatine kinase (CK) levels and Ca metabolic disorders. Two patients with moderate hypocalcemia had increased CK levels (hyper-CK-emia), which normalized during treatment for the hypocalcemia; a negative correlation between the Ca and CK levels was observed in both patients. Primary hyperparathyroidism (HPT) is associated with cardiovascular disease, but there are no systematic studies on the electrocardiographic findings in HPT. Preoperative electrocardiograms from 139 patients with primary HPT and 97 control patients were therefore investigated.
