Sensitivity and predictive value of ultrasound in pediatric cholecystitis.

BACKGROUND: Ultrasonography has a high sensitivity and positive predictive value (PPV) for diagnosing cholecystitis in adults. The objective of this study was to determine the sensitivity and PPV of ultrasonography in the diagnosis of pediatric cholecystitis. METHODS: We performed a single-institution retrospective review of the records of all patients undergoing cholecystectomy with a preoperative ultrasound during 2005-2010. We calculated sensitivity, specificity, and PPV using pathologic findings as the standard for the diagnosis of cholecystitis. RESULTS: In the 223 included patients, th…

[Surgical tactics in patients with cholecystitis complicated by bilirubinemia].

Basing on experience of treatment of more than 11 000 patients there were analyzed its results in 248, who were admitted to the hospital in emergency for an acute cholecystitis and raising of a bilirubin level from 29.54 to 167.16 micromol/l. Miniinvasive tactic was applied, surgical treatment was divided on the stages: laparoscopic cholecystectomy with the common biliary duct (CBD) draining, postoperative transdrainage cholangiography (in 184 patients any calculi or other obstacles to the bile outflow were not revealed), endoscopic papillosphincterotomy--in accordance with the indications est…

Acute Acalculous Cholecystitis due to Viral Hepatitis A.

Inflammation of the gallbladder without evidence of calculi is known as acute acalculous cholecystitis (AAC). AAC is frequently associated with gangrene, perforation, and empyema. Due to these associated complications, AAC can be associated with high morbidity and mortality. Medical or surgical treatments can be chosen according to the general condition of the patient, underlying disease and agent. Particularly in acute acalculous cholecystitis cases, early diagnosis and early medical treatment have a positive effect on the patient and protect them from surgical trauma. ACC is a rare complicat…

A Rare Presentation of Xanthogranulomatous Cholecystitis as Bouveret's Syndrome.

The purpose of this paper is to present sonographic and CT imaging findings of xanthogranulomatous cholecystitis (XGC) presented as Bouveret's syndrome, a very rare cause of gastric obstruction. While the patient's physical examination, upper GI endoscopy, and radiological findings all pointed to Bouveret's syndrome, CT differential diagnosis suggested either XGC or gallbladder carcinoma, and the final diagnosis was done histopathologically. Our paper aims to increase awareness in radiologically diagnosing XGC cases by introducing the possibility of existence of Bouveret's syndrome.

Payer status and treatment paradigm for acute cholecystitis.

HYPOTHESIS: Medicaid recipients who present to the emergency department with acute cholecystitis (AC) would have reduced access to cholecystectomy compared with a similar population of private insurance carriers. DESIGN: The Nationwide Inpatient Sample (NIS) database from 1998 to 2008. PARTICIPANTS: Emergent hospitalizations (843 179) with AC as a primary diagnosis. INTERVENTIONS: Insurance type was analyzed against cholecystectomy in propensity score-matched cohorts. MAIN OUTCOME MEASURES: Surgical intervention and surgical outcomes. RESULTS: Approximately 200 000 patients were in each m…

A Rare Case of Cholecystitis Caused by Raoultella planticola.

A 62-year-old female presented with right upper quadrant pain. Clinical examination and ultrasound scan were consistent with gallstones and acute cholecystitis. She received 3 days of intravenous Co-amoxiclav and was discharged with 5-days of oral antibiotics with arrangements to return for an elective cholecystectomy. This was performed 5 months later which revealed an inflamed gallbladder and a localised abscess secondary to gallbladder perforation. Fluid from the gallbladder was taken which cultured Raoultella planticola, a gram-negative, nonmotile environmental bacteria (Bagley et al. (198…

Non-operative management of acute cholecystitis in the elderly.

BACKGROUND: Although cholecystectomy is the standard therapy for acute cholecystitis (AC), operative morbidity in the elderly may be high owing to medical co-morbidities and decreased physiological reserve. Outcomes of AC in the elderly have not been fully defined with regard to operative and long-term non-operative management. METHODS: Patients aged 65 years or over admitted to a tertiary care centre with a diagnosis of AC between January 2000 and December 2009 were reviewed retrospectively. Patient data, operative and postoperative details were obtained. To determine cholecystectomy rates i…

[A case of perforated xanthogranulomatous cholecystitis presenting as biloma].

Xanthogranulomatous cholecystitis is an unusual inflammatory disease of the gallbladder characterized by severe proliferative fibrosis and the accumulation of lipid-laden macrophages in areas of destructive inflammation. Its macroscopic appearance may occasionally be confused with gallbladder carcinoma. We present a case of perforated xanthogranulomatous cholecystitis presenting as biloma. An 80-year-old woman was referred to our hospital with a 1-week history of abdominal pain and febrile sensation. Abdominal CT showed a biloma in the subhepatic area. The follow-up CT showed that the biloma i…

Hepatitis a with pleural effusion, ascites and acalculous cholecystitis.

BACKGROUND: Hepatitis A virus (HAV) infection constitutes an important health problem in developing countries. It is usually a benign self-limiting disease, but may present with atypical clinical findings. CASE PRESENTATION: A twelve-year-old male with ascites, pleural effusion, and acalculous cholecystitis during the course of HAV infection is reported. He was managed conservatively and clinical improvement was observed with resolution of HAV infection. CONCLUSION: To our knowledge, this is the first case in which all these three rare complications were observed in a single patient in the e…

[The role of laparoscopy in the surgical treatment of scleroatrophic cholecystitis].

Atrophic cholecystitis is a chronic inflammatory complication of biliary lithiasis which occurs after many years of evolution and causes many problems in the surgical therapy. Authors analyze a 39 patients cohort and present the difficulties encountered during the treatment, especially in laparoscopic approach. These particular difficulties lead to a increase in conversion rate, 11.1%, much higher that in average laparoscopic cholecystectomy. The conclusion of this study is that this disease still remains a challenge for surgeons, especially in laparoscopic approach, and the conversion an eleg…

Acalculous cholecystitis in a patient with hepatocellular carcinoma on sorafenib.

A 67-year-old woman with compensated cirrhosis type B associated with hepatocellular carcinoma was started on sorafenib for multiple pulmona … he gallbladder was seen sporadically. The fact that this patient had no risk factors for acalculous cholecystitis suggested that the cholecystitis resulted from ischemia, implying a strong causal relationship with sorafenib.

[Acute acalculous cholecystitis--difficulties of diagnosis and treatment].

Acute acalculous cholecystitis is defined by ultrasonographic, intraoperative and pathologic findings of acute cholecystitis, without evidence of gallstones. It is associated to recent operations, trauma, burns, multisystem organ failure and parenteral nutrition. It can also occur as the first episode, in patients with pathological conditions which generate local ischemia: diabetes mellitus, malignant disease, abdominal vasculitis, congestive heart failure. The authors present a series of 20 patients, operated in the Surgical Department of the Clinical Hospital "Dr. I. Cantacuzino", between 20…

[Laparoscopic cholecystectomy in totalis situs inversus for cholecystitis].

Various congenital anomalies, organ transpositions impose special demands on building up the diagnosis; the mirror image of the abdominal organs tests the skills of the surgeon. We report a case of a 68 year old female patient, with a known situs inversus totalis, who underwent a laparoscopic cholecystectomy. We discuss our diagnostic steps (such as US, ERCP and EST), the variation of maneuvers used during the operation. No iatrogenic event occurred. We also stress that the surgeon should be alert of possible local anatomic variations and, if needed, conversion should be a solution of choice.

Acute cholecystitis caused by Haemophilus influenzae in a child.

A 6-year old girl was admitted to our hospital with high fever and right upper quadrant abdominal pain. At 5 years of age she had undergone allogeneic bone marrow transplantation because of intractable congenital pure red cell aplasia, after which she had asymptomatic cholelithiasis. Imaging studies and laboratory findings on admission suggested acute cholecystitis with a gallstone. The patient recovered completely after laparoscopic cholecystectomy was performed and was discharged on day 7 after the operation. Haemophilus influenzae grew on the aspirated purulent bile. It should be noted that…
