The Communication Profile for the Hearing Impaired (CPHI) (Demorest & Erdman, 1986) is a self-assessment inventory that was developed on a clinical population that included predominantly male, active-duty military personnel. Choledochal cysts have generally been described to communicate with the biliary system. We recently saw three adult patients in whom we could not demonstrate biliary communication between the cyst and the biliary tree, despite a carefully performed endoscopic cholangiopancreatography (ERCP). These cases may represent a variant of type II choledochal cysts. During the 5-year period of review of ERCP records, 30 choledochal cysts were diagnosed, of which 3 (10%) were the noncommunicating cases described in this report.

Reality orientation is commonly used in nursing homes with disoriented persons; experienced nurses, however, often find it meets with withdrawal, vegetation or increasing hostility. 3. Validation strategies are communication techniques which are specifically matched to a particular stage of confusion. 4. When validation techniques are appropriately matched to confusional stage the effectiveness of the intervention can be significantly improved.

A 51-year-old woman was referred to our institute for surgical treatment from an other hospital where she was diagnosed as having a mycotic aneurysm located at the aortic annulus due to infective endocarditis. The aneurysm communicated with the left ventricle, with aortic stenosis and regurgitation, mitral stenosis and tricuspid regurgitation caused by rheumatic fever. On admission to our institute, the infective endocarditis was at the healed stage. The mycotic aneurysm was located at the aortic annulus of the right coronary cusp, and was closed using a prosthetic patch with mattress sutures.

This article describes the outcomes of a study involving family members of communication-impaired long-term care residents in a collaborative nursing/speech language pathology intervention designed to increase the residents' communication ability. Family members provided memorabilia and artifacts or produced audio or video tapes, for use in conjunction with a speech therapy enhancement program (STEP). Findings revealed that, despite a minimal improvement in speech ability, there was a dramatic increase in family members' satisfaction.

A 17-year-old patient was found to have cyanosis. A right-to-left shunt was suspected clinically and confirmed by noninvasive techniques such as perfusion lung scan and contrast echocardiography. Certain structures which indicate probable involvement of a filamentous phase in life cycle of M. leprae have been noted in preserved skin biopsy suspensions from lepromatous leprosy cases. These include (i) filaments with empty or pink round spaces within them (ii) conidia-like structures and (iii) membranes with acid fast bacilli. These structures were rare in the fresh material.

Open heart surgery was performed in a 5 year-old boy with severe hemophilia A and large ventricular septal defect with pulmonary stenosis. High doses of Factor VIIIC superconcentrates delivered as small transfusion volumes allowed the use of extracorporeal circulation with heparin. Transfusions began 5 hours before surgery and were stopped 21 days later. No adverse reaction was observed. Thus major surgery is possible in severe hemophiliacs provided strict rules for replacement therapy are followed.

Communication development was studied in 77 subjects with long-term tracheostomies ranging in age from two months to 7 years. Children were categorized according to tracheostomy status (cannulated or decannulated), presence of speech practice prior to cannulation, and level of language development (prelinguistic or linguistic) at the time of decannulation.
