Health and economic impact of oral PrEP provision across subgroups in western Kenya: a modelling analysis.

INTRODUCTION: Oral pre-exposure prophylaxis (PrEP) is a priority intervention for scale-up in countries with high HIV prevalence. Policymakers must decide how to optimise PrEP allocation to maximise health benefits within limited budgets. We assessed the health and economic impact of PrEP scale-up among different subgroups and regions in western Kenya. METHODS: We adapted an agent-based network model, EMOD-HIV, to simulate PrEP uptake in six counties of western Kenya across seven subgroups including serodiscordant couples (SDCs), adolescent girls and young women (AGYW), adolescent boys and yo

Effect of music on clinical outcome after hip fracture operations (MCHOPIN): study protocol of a multicentre randomised controlled trial.

BACKGROUND: Patients undergoing proximal femur fracture surgery are at high risk of postoperative complications, with postoperative delirium occurring in 25%-40% of patients. Delirium has profound effects on patient outcome and recovery, the patient's family, caregivers and medical costs. Perioperative music has a beneficial effect on eliciting modifiable risk factors of delirium. Therefore, the aim of this trial was to evaluate the effect of perioperative recorded music on postoperative delirium in patients with proximal femur fracture undergoing surgery. METHODS AND ANALYSIS: The Music on C

Epidemiology and Predictors of all-cause 30-Day readmission in patients with sickle cell crisis.

The 30-day readmission rate after hospitalization for a sickle cell crisis (SCC) is extremely high. Accurate information on readmission diagnoses, total readmission costs and factors associated with readmission is required to effectively plan resource allocation and to plan interventions to reduce readmission rates. The present study aimed to examine readmission diagnoses and factors associated with all-cause 30-day readmission after hospitalization for SCC. We analyzed 2016 nationwide readmission database (NRD) to identify patterns of 30-day readmission by patient demographic characteristics

Genotype-Informed Versus Empiric Management Of VirEmia (GIVE MOVE): study protocol of an open-label randomised clinical trial in children and adolescents living with HIV in Lesotho and Tanzania.

BACKGROUND: Globally, the majority of people living with HIV have no or only limited access to HIV drug resistance testing to guide the selection of antiretroviral drugs. This is of particular concern for children and adolescents, who experience high rates of treatment failure. The GIVE MOVE trial assesses the clinical impact and cost-effectiveness of routinely providing genotypic resistance testing (GRT) to children and adolescents living with HIV who have an unsuppressed viral load (VL) while taking antiretroviral therapy (ART). METHODS: GIVE MOVE is an open-label randomised clinical trial

SUcceSS, SUrgery for Spinal Stenosis: protocol of a randomised, placebo-controlled trial.

INTRODUCTION: Central lumbar spinal stenosis (LSS) is a common cause of pain, reduced function and quality of life in older adults. Current management of LSS includes surgery to decompress the spinal canal and alleviate symptoms. However, evidence supporting surgical decompression derives from unblinded randomised trials with high cross-over rates or cohort studies showing modest benefits. This protocol describes the design of the SUrgery for Spinal Stenosis (SUcceSS) trial -the first randomised placebo-controlled trial of decompressive surgery for symptomatic LSS. METHODS AND ANALYSIS: SUcce

Lives saved from malaria prevention in Africa--evidence to sustain cost-effective gains.

Lives saved have become a standard metric to express health benefits across interventions and diseases. Recent estimates of malaria-attributable under-five deaths prevented using the Lives Saved tool (LiST), extrapolating effectiveness estimates from community-randomized trials of scale-up of insecticide-treated nets (ITNs) in the 1990s, confirm the substantial impact and good cost-effectiveness that ITNs have achieved in high-endemic sub-Saharan Africa. An even higher cost-effectiveness would likely have been found if the modelling had included the additional indirect mortality impact of ITNs

Organ quality metrics are a poor predictor of costs and resource utilization in deceased donor kidney transplantation.

BACKGROUND: The desire to provide cost-effective care has lead to an investigation of the costs of therapy for end-stage renal disease. Organ quality metrics are one way to attempt to stratify kidney transplants, although the ability of these metrics to predict costs and resource use is undetermined. METHODS: The Scientific Registry of Transplant Recipients database was linked to the University HealthSystem Consortium Database to identify adult deceased donor kidney transplant recipients from 2009 to 2012. Patients were divided into cohorts by kidney criteria (standard vs expanded) or kidney

Clinical and cost-effectiveness of contingency management for cannabis use in early psychosis: the CIRCLE randomised clinical trial.

BACKGROUND: Cannabis is the most commonly used illicit substance amongst people with psychosis. Continued cannabis use following the onset of psychosis is associated with poorer functional and clinical outcomes. However, finding effective ways of intervening has been very challenging. We examined the clinical and cost-effectiveness of adjunctive contingency management (CM), which involves incentives for abstinence from cannabis use, in people with a recent diagnosis of psychosis. METHODS: CIRCLE was a pragmatic multi-centre randomised controlled trial. Participants were recruited via Early In

A cost-effective breast cancer screening strategy for Urban China: Findings from a Shenzhen-based modeling study.

BACKGROUND: Early detection through breast cancer screening significantly enhances survival rates and reduces mortality. However, financial constraints in low- and middle-income countries often limit the implementation of large-scale screening programs. This study evaluates the cost-effectiveness of a combined Clinical Breast Examination (CBE), Breast Ultrasound (BUS), and supplementary Mammography (MAM), screening strategy for women aged 35-65 in Shenzhen, China. It further identifies optimal screening protocols by analyzing variations in screening frequency, starting/ending ages, and long-te

Clinical Outcomes and Healthcare Costs of CART Versus Paracentesis for Malignant Ascites: A Nationwide Retrospective Cohort Study in Japan.

BACKGROUND: Paracentesis temporarily relieves malignant ascites but causes hypoalbuminemia. Cell-free and concentrated ascites reinfusion therapy (CART) reinfuses autologous proteins to prevent hypoalbuminemia and has been increasingly used in Japan. However, CART has not been widely adopted outside of Japan, and its benefit remains unclear. We evaluated the clinical outcomes and healthcare costs of CART compared with paracentesis in metastatic cancer. METHODS: This retrospective cohort study included hospitalized patients with metastatic solid cancer receiving CART or paracentesis across Jap

Strategies in primary healthcare to implement early identification of risky alcohol consumption: why do they work or not? A qualitative evaluation of the ODHIN study.

BACKGROUND: Screening and brief interventions (SBI) in primary healthcare are cost-effective in risky drinkers, yet they are not offered to all eligible patients. This qualitative study aimed to provide more insight into the factors and mechanisms of why, how, for whom and under what circumstances implementation strategies work or do not work in increasing SBI. METHODS: Semi-structured interviews were conducted between February and July 2014 with 40 GPs and 28 nurses in Catalonia, the Netherlands, Poland, and Sweden. Participants were purposefully selected from the European Optimising Deliver

Setting priorities for non-regulatory animal health in Ireland: results from an expert Policy Delphi study and a farmer priority identification survey.

Agriculture is a very important contributor to the Irish economy. In Ireland, national animal health services have been a government, rather than an industry, responsibility. In 2009, Animal Health Ireland (AHI) was established to provide a partnership approach to national leadership of non-regulatory animal health issues (those not subject to national and/or EU regulation). The objectives of this study were to elicit opinion from experts and farmers about non-regulatory animal health issues facing Irish livestock industries, including prioritisation of animal health issues and identification

A stochastic programming approach for dynamic allocation of bed capacity and assignment of patients to collaborating hospitals during pandemic outbreaks.

Sustaining regular and infectious care during an infectious outbreak requires adequate management support for patient capacity allocation. During the COVID-19 pandemic, hospitals faced severe challenges, including uncertainty surrounding the number of infectious patients needing hospitalization and too little regional cooperation. This led to inefficient usage of healthcare capacity. To better prepare for future pandemics, we have developed a decision support system for central regional decision-making on opening and closing hospital rooms for infectious patients and assigning new infectious p

Determinants, implementation strategies, and mechanisms of the 4T Program.

The Teamwork, Targets, and Technology for Tight Glycemia (4T) Program has demonstrated effectiveness in early continuous glucose monitor use with remote patient monitoring among three diverse cohorts of youth with new onset type 1 diabetes (T1D, n = 451). Based on promising single center experience, we examine the potential for program scaling to the T1D Exchange Quality Improvement (T1DX-QI) Collaborative using an implementation science approach. We used the Simplified Implementation Logic Model to identify contextual determinants of implementation success. Then, we described the connections

Mathematical insights into mumps transmission control with optimal strategies.

In this study, we develop an optimal control framework for managing mumps infections through a dynamic model that integrates public health interventions such as awareness programs, isolation protocols, and a two-dose immunization regimen. We begin by establishing the model's fundamental analytical properties, including the existence and stability of disease equilibria, the positivity and boundedness of solutions, and a threshold condition for disease transmission. Local stability analysis is conducted via the Routh-Hurwitz criteria, ensuring robust insights into the disease dynamics. The optim

A mHealth intervention to reduce perceived stress in patients with ischemic heart disease: study protocol of the randomized, controlled confirmatory intervention "mStress-IHD" trial.

BACKGROUND: Stress is highly prevalent in patients with ischemic heart disease (IHD) and is associated with lower health-related quality of life and impaired cardiovascular outcome. The importance of stress management is now recognized in recent guidelines for the management of cardiovascular disease. However, effective stress management interventions are not implemented in clinical routine yet. The development of easily disseminated eHealth interventions, particularly mHealth, may offer a cost-effective and scalable solution to this problem. The aim of the proposed trial is to assess the effi

Design, rationale, and baseline characteristics of a pilot randomized clinical trial of nicotine treatment for pulmonary sarcoidosis.

INTRODUCTION: Sarcoidosis is a systemic granulomatous disease of unknown cause afflicting young to middle-aged adults. The majority of patients with active pulmonary sarcoidosis complain of overwhelming fatigue, which often persists despite administration of immune-modulating drugs typically used to treat sarcoidosis. Nicotine offers an alternative to conventional treatments, which are associated with a spectrum of serious untoward effects, including diabetes mellitus, osteoporosis, bone marrow suppression, severe infections, cirrhosis. The described pilot randomized trial aims to provide prel
