Efficacy, acceptability and tolerability of antipsychotics in patients with schizophrenia and comorbid substance use. A systematic review and meta-analysis.

Patients with schizophrenia and substance related comorbidity or substance induced psychotic disorder are difficult to treat. Although the prevalence of a comorbid substance use is approximately 40% in schizophrenia, such patients are usually excluded from clinical trials. We therefore performed a random-effects meta-analysis of all randomized controlled antipsychotic drug trials in this patient subgroup. We searched multiple databases up to May, 2018. The primary outcome was the reduction of substance user; secondary outcomes were craving, mean reduction of substance use, overall change in sc…

Exploring time-dependent changes in conditioned place preference for food reward and associated changes in the nucleus accumbens.

The conditioned place preference (CPP) procedure has been used to study the incubation of craving phenomenon with rewarding drugs such as cocaine and methamphetamine. The present study examined whether rats trained in a CPP behavioral design would display an incubation of craving response for chocolate-flavored pellets or milk chocolate chips at the behavioral and neural levels. Rats were conditioned using an unbiased CPP design then underwent abstinence from food reward for 24 hs, 7, 14, or 28 days at which point they were tested for CPP. Brains underwent immunohistochemical staining for c-Fo…

Transcranial Direct Current Stimulation in Substance Use Disorders: A Systematic Review of Scientific Literature.

INTRODUCTION: New treatment options such as noninvasive brain stimulation have been recently explored in the field of substance use disorder … d to the dorsolateral prefrontal cortex, and 8 suggested the efficacy of tDCS in reducing substance craving. CONCLUSIONS: In light of these data, it is premature to conclude that tDCS over the dorsolateral prefrontal cortex is a very efficient technique in reducing craving. Small sample size, different stimulation protocols, and study duration were the m …

Altered resting-state functional connectivity of the insula in young adults with Internet gaming disorder.

The insula has been implicated in salience processing, craving, and interoception, all of which are critical to the clinical manifestations of drug and behavioral addiction. In this functional magnetic resonance imaging (fMRI) study, we examined resting-state functional connectivity (rsFC) of the insula and its association with Internet gaming characteristics in 74 young adults with Internet gaming disorder (IGD) and 41 age- and gender-matched healthy control subjects (HCs). In comparison with HCs, IGD subjects (IGDs) exhibited enhanced rsFC between the anterior insula and a network of regions…

[Neuromodulation as an intervention for addiction: overview and future prospects].

BACKGROUND: In recent years several neuromodulation techniques have been introduced as interventions for addiction. AIM: To review and disc … ne dependent patients showed that modulation of the anterior cingulate cortex can decrease smokers' craving for nicotine. In several studies decreased craving was found in alcohol dependent patients after TMS or tDCS stimulation of the anterior cingulate cortex or the dorsolateral prefrontal cortex. The first DBS pilot studies suggest that the nucleus acc …

Stress decreases the ability to resist smoking and potentiates smoking intensity and reward.

We have developed a novel human laboratory model to examine two primary aspects of stress-precipitated tobacco relapse: (1) Does stress redu … oking. Stress significantly increased hypothalamus-pituitary-adrenal (HPA) axis reactivity, tobacco craving, negative emotion, and physiologic reactivity relative to the neutral condition. In addition, increased cortisol, ACTH, and tobacco craving were associated with reduced ability to resist smoking following stress. These findings have implications for …

An acute psychosocial stress enhances the neural response to smoking cues.

Stress plays an important role in drug addiction. It can trigger relapse in abstinent addicts, and both in the everyday world and in the laboratory, a stressor can induce drug craving. Drug cues, such as the sight of drug, can also trigger subjective craving and relapse, and this effect may be amplified by stress. Underpinning this interaction may be the fact that stress and reward-predicting drug cues act on overlapping brain regions. We exposed 15 smokers undergoing functional magnetic resonance imaging to a psychosocial stressor, the Montreal Imaging Stress Task, followed by drug cues consi…

Identification of a primary metabolite of ibogaine that targets serotonin transporters and elevates serotonin.

Ibogaine is a hallucinogenic indole with putative efficacy for the treatment of cocaine, stimulant and opiate abuse. The purported efficacy of ibogaine following single dose administrations has led to the suggestion that a long-acting metabolite of ibogaine may explain in part how the drug reduces craving for psychostimulants and opiates. We report here that 12-hydroxyibogamine, a primary metabolite of ibogaine, displays high affinity for the 5-HT transporter and elevates extracellular 5-HT. In radioligand binding assays, 12-hydroxyibogamine was 50-fold more potent at displacing radioligand bi…

Pupil diameter during cognitive reappraisal predicts the desire to eat.

Cognitive reappraisal involves reinterpreting negative content to reduce its negative impact. Recent findings suggest that cognitive reappraisal can attenuate the link between emotions and eating-related measures. The level of regulatory engagement during the use of cognitive reappraisal can be assessed by physiological markers such as pupil dilation. The present study examined whether trial-by-trial within-person variations in pupil diameter while implementing reappraisal can predict subsequent desire to eat. Forty-three healthy females completed a computerized task combining a standard reapp

Cannabis and alcohol co-use: The effects of intensity of cannabis use among heavy drinkers.

OBJECTIVE: Cannabis and alcohol co-use is highly prevalent and confers a host of risk factors that outweigh those related to the use of eith … co-use. Individuals reporting more frequent cannabis use also reported increased levels of alcohol craving and more heavy drinking days, as compared to those who reported fewer or no cannabis use days. Drinking days and treatment seeking for AUD significantly predicted increases in cannabis use days. CONCLUSION: In clinical practice, younger age, male g …

A Neurobehavioral Approach to Addiction: Implications for the Opioid Epidemic and the Psychology of Addiction.

Two major questions about addictive behaviors need to be explained by any worthwhile neurobiological theory. First, why do people seek drugs … ng neurobehavioral decision systems (CNDS), and the role of the insula in modulating addictive drug craving. In the special case of opioids, particular attention is paid to whether their analgesic effects overlap with their reinforcing effects and whether the perceived low risk of taking legal medicinal opioids, which are often prescribed by a health prof …

Metabolic and feeding adjustments during pregnancy.

Eating behaviours are determined by the integration of interoceptive and environmental inputs. During pregnancy, numerous physiological adaptations take place in the maternal organism to provide an adequate environment for embryonic growth. Among them, whole-body physiological remodelling directly influences eating patterns, commonly causing notable taste perception alterations, food aversions and cravings. Recurrent food cravings for and compulsive eating of highly palatable food can contribute to the development and maintenance of gestational overweight and obesity with potential adverse hea…
