A reaction of DBU promoted ring opening in nucleoside-3'-O- and nucleoside-5'-O-(2-thio-4,4-pentamethylene-1,3,2-oxathiaphospholane) monomers with a pyrophosphate or a methylenediphosphonate anion proceeds with substantial loss of stereoselectivity. Depending on the absolute configuration of the phosphorus atom, so far widely accepted the stereoretentive mechanism of condensation is accompanied by a stereoinvertive one, most likely employing an intramolecular ligand-ligand exchange in an uncharged intermediate.

PURPOSE OF REVIEW: Hypercalcaemia is commonly associated with cancer which is usually advanced and portends a poor prognosis. Hypocalcaemia is more often seen as a complication of therapy aimed at reducing skeletal morbidity rather than an effect of the cancer itself. We present an overview of calcium disorders in malignant disease.
RECENT FINDINGS: A significant proportion of patients who have a cancer and become hypercalcaemic have an alternative cause for their hypercalcaemia.Evidence for the use of loop diuretics is lacking, and such agents should not be routinely used unless significant

Osteonecrosis of the jaw (ONJ) is a serious side effect in patients receiving intravenous nitrogen-containing bisphosphonates (B). It has also been reported to occur due to oral administration of B. Most cases will appear after receiving B for more than 1 year. The authors report a case of a 67-year-old woman with osteoporosis who had received oral alendronate sodium for 2 years and stopped the treatment due to dyspepsia. 18 months later she was diagnosed with breast cancer and bone metastases. She started a treatment based on aromatase inhibitors and zoledronic acid (Z). She developed ONJ soo

OBJECTIVE: To assess the effect of bisphosphonates on post-parathyroidectomy hypocalcemia in patients with osteitis fibrosa cystica.
METHODS: Review of the medical records of six patients using bisphosphonates preoperatively.
RESULTS: Mean age was 35.6 ± 10.5 years; serum calcium = 13.51 + 0.87 mg/dL; iPTH = 1,389 + 609 pg/mL. The mean value of urine deoxypyridinoline (UDPD) of three patients was 131 ± 183 nmol/mmol Cr, and of C-telopeptide (CTX), 2,253 ± 1,587 pg/mL. The mean values of bone densitometry (T score) were as follows: 0.673 ± 0.150 g/cm(2) (-4.42 ± 1.23) in lumbar spine (L2-L4);

Synovitis and subchondral bone turnover are associated with pain in osteoarthritis. Bisphosphonates provide tools for investigating these pathogenic mechanisms and also may have therapeutic potential. Translating preclinical findings into new treatments for human osteoarthritis requires a critical appraisal and refinement of animal models, identification of those pathogenic mechanisms that are amenable to intervention, and pharmacological targeting of those mechanisms in the right people at the right time.

Zoledronic acid is a newly FDA-approved bisphosphonate for the treatment of hypercalcemia of malignancy. Although the safety and efficacy of this drug in treating hypercalcemia associated with hyperparathyroidism have not yet been established in clinically controlled trials, its off-label use is not uncommon. We describe a patient with primary hyperparathyroidism treated with zoledronic acid who developed severe postoperative hypocalcemia. A 64-yr-old woman was admitted with severe hypercalcemia. She was treated with rehydration, calcitonin, methylprednisolone, furosemide as well as 4 mg/day o

Zoledronic acid is a third-generation bisphosphonate that is administered as an annual infusion, and it has some interesting features. After a successful preclinical development program, it has undergone extensive testing in postmenopausal osteoporosis where it has substantial antifracture efficacy at the vertebral, nonvertebral, and hip sites. It has been shown to be effective in reducing vertebral and nonvertebral fractures in patients who had recently suffered a femoral neck fracture, and has demonstrated efficacy in preventing and reversing bone loss in glucocorticoid osteoporosis. CONTEXT: Nonvertebral fractures (NVFs) are the most costly and disabling type of osteoporotic fractures. Bisphosphonate therapy effectively reduces the risk for NVFs; however, fracture protection depends critically on adherence and persistence. Approved bisphosphonate regimens with extended dosing intervals increase patient convenience, help patients remain on therapy, and improve fracture protection in clinical practice.
EVIDENCE ACQUISITION: To assess evidence for NVF reduction with extended-interval bisphosphonates, we searched PubMed for phase 3 clinical trials, meta-analyses, and reviews

BACKGROUND: Postmenopausal osteoporosis is a serious, chronic condition, for which nitrogen-containing bisphosphonates are now one of the treatments of choice.
OBJECTIVE: To review the profile of ibandronate, a monthly oral (150 mg) or quarterly intravenous injection (3 mg) of bisphosphonate.
METHODS: The literature search was limited to publications of ibandronate data.
RESULTS/CONCLUSION: Ibandronate is rapidly absorbed and distributed in the bone; it is not metabolised and is excreted in urine. Clinical trial data have demonstrated the efficacy of ibandronate in reducing fracture risk, i

Bone compaction has been shown to increase initial implant fixation. Furthermore, bone compaction creates a peri-implant zone of autograft that exerts osteoconductive properties. We have previously shown that locally applied bisphosphonate (alendronate) at 4-week observation can preserve the autograft generated by bone compaction. We now investigate whether the increased amount of autograft, seen at 4 weeks, can increase implant osseointegration and biomechanical fixation. Porous-coated titanium implants were bilaterally inserted with bone compaction into the proximal part of tibia of 10 dogs.

Various pathogenic conditions including endocrine, inflammatory, gastrointestinal and hematological disorders, medication, and immobilization cause secondary osteoporosis. Secondary osteoporosis occurs not only in postmenopausal women but also in premenopausal women and men. Therefore, the fractures have a great impact on their social lives. Serial articles regarding secondary osteoporosis will appear in this journal, especially focused on glucocorticoid-induced osteoporosis and osteoporosis associated with rheumatoid arthritis.

BACKGROUND: Pamidronate is used extensively for treatment of osteoporotic and high bone turnover conditions. Because it has a long retention time in the human skeleton, concerns have been raised as to safety profile in women of child-bearing age.
METHODS: Four infant outcomes of pregnancies of three women, two with polyostotic fibrous dysplasia and one with osteogenesis imperfecta, all of whom were treated with iv pamidronate before conception, are reported, with biochemical, radiological, and bone density data.
RESULTS: Each pregnancy was uncomplicated, and the four offspring are healthy, w

Osteonecrosis of the jaws is a recently described adverse side effect of bisphosphonate therapy. Patients with multiple myeloma and metastatic carcinoma to the skeleton who are receiving intravenous, nitrogen-containing bisphosphonates are at greatest risk for osteonecrosis of the jaws; these patients represent 94% of published cases. The mandible is more commonly affected than the maxilla (2:1 ratio), and 60% of cases are preceded by a dental surgical procedure. Oversuppression of bone turnover is probably the primary mechanism for the development of this condition, although there may be cont
