A 60-year-old woman presented with pulsatile tinnitus in the left ear. MRI and CT imaging suggested a soft-tissue mass in the middle ear. Exploratory tympanotomy and biopsy confirmed Schneiderian (inverted) papilloma. Endoscopic and radiological assessment showed no evidence of sinonasal lesions. The patient proceeded to mastoid surgery with removal of the ossicles for disease clearance. The small number of cases published to date of isolated middle ear inverted papilloma suggest a high recurrence and malignant transformation rate and aggressive management is therefore warranted.

Autoimmune inner ear disease (AIED) has been defined as a condition of bilateral sensorineural hearing loss (SNHL), caused by an 'uncontrolled' immune system response. The inner ear can be the direct target of the immune response, but it can be additionally damaged by a deposition of circulating immune complexes or by systemic immune-mediated diseases. IMPORTANCE: Chronic rhinosinusitis (CRS) can cause an obstruction of the tubal orifice and thereby compromise ventilation of the middle ear. Salivary gland choristoma of the middle ear cavity is a very rare condition. These lesions are a result of a defective embryonic development and their adjacent structures may be associated with abnormalities. Here we report a case of salivary gland choristoma of the middle ear who presented to Be'sat Hospital, Tehran, Iran in 2015 with unilateral conductive hearing loss. There are 41 case reports in English and non-English literature from 1961. Taylor and Martin reported the first case of middle ear salivary choristoma.
