Evaluation of Motion Compensation Methods for Noninvasive Cardiac Radioablation of Ventricular Tachycardia.

PURPOSE: Noninvasive cardiac radioablation is increasingly used for treatment of refractory ventricular tachycardia. Attempts to limit normal tissue exposure are important, including managing motion of the target. An interplay between cardiac and respiratory motion exists for cardiac radioablation, which has not been studied in depth. The objectives of this study were to estimate target motion during abdominal compression free breathing (ACFB) and respiratory gated (RG) deliveries and to investigate the quality of either implanted cardioverter defibrillator lead tip or the diaphragm as a gatin

Influence of dietary nitrate supplementation on lung function and exercise gas exchange in COPD patients.

BACKGROUND: During exercise as pulmonary blood flow rises, pulmonary capillary blood volume increases and gas exchange surface area expands through distention and recruitment. We have previously demonstrated that pulmonary capillary recruitment is limited in COPD patients with poorer exercise tolerance. Hypoxia and endothelial dysfunction lead to pulmonary vascular dysregulation possibly in part related to nitric oxide related pathways. PURPOSE: To determine if increasing dietary nitrate might influence lung surface area for gas exchange and subsequently impact exercise performance. METHODS:

Leveraging intrinsic non-sinusoidal patterns to infer search behavior to predict exposure to respiratory stressors.

This study introduces a novel entropy-based methodology to quantitatively characterize nonlinear transient breathing dynamics under respiratory stress. Environmental and pathophysiological stressors can disrupt the respiratory system's gas exchange, leading to compromise and compensatory mechanisms. We present a data-driven approach that systematically evaluates classical respiratory features alongside novel entropic features as key indicators under respiratory stress. We demonstrate that conventional metrics like breathing rate (BR), time of inspiration (TI), and expiration (TE) fail to captu

Expiratory Central Airway Collapse: A Comprehensive Narrative Review.

Expiratory central airway collapse (ECAC) encompasses 2 subtypes: excessive dynamic airway collapse and tracheobronchomalacia. ECAC manifests as symptoms of dyspnea, chronic cough, difficulty in clearing secretions, and recurrent respiratory infections. Diagnosis involves dynamic flexible bronchoscopy and computed tomography scans. Medical management addresses respiratory symptoms and concurrent conditions like chronic obstructive pulmonary disease, asthma, gastroesophageal reflux disease, and paroxysmal vocal fold motion. Selected patients may benefit from stent trials to assess candidacy for

Body positioning-related laryngeal narrowing pattern and expiratory mechanical constraints in advanced chronic obstructive pulmonary disease: A case report.

We previously reported that laryngeal widening led to improved exercise tolerance in COPD. However, it is not clear whether laryngeal narrowing occurs as a compensatory response to tracheal movement or is affected by posture. Here, we report the case of an advanced COPD patient whose more prolonged expiration in a head-forward leaning position compared with that in a neck-extended position occurred with an excessive duration of severe laryngeal narrowing without tracheal obstruction, which led to exercise intolerance with expiratory mechanical constraints. This case provided useful insights in

Computational modeling of nasal nitric oxide flux from the paranasal sinuses: Validation against human experiment.

BACKGROUND: Nitric oxide (NO) is important in respiratory physiology and airway defense. Although the paranasal sinuses are the major source of nasal NO, transport dynamics between the sinuses and nasal cavities are poorly understood. METHODS: Exhaled nasal NO tracings were measured in two non-asthmatic subjects (one with allergic rhinitis, one without) using NO analyzer connected via face mask. We subsequently performed computational fluid dynamics NO emission simulations based on individual CT scans and compared to the experimental data. RESULTS: Simulated exhaled NO tracings match well wi

Enhanced airway sensory nerve reactivity in non-eosinophilic asthma.

OBJECTIVE: Impaired diabetic wound healing is one of the serious complications associated with diabetes. In patients with diabetes, this impairment is characterized by several physiological abnormalities such as metabolic changes, reduced collagen production, and diminished angiogenesis. We designed and developed a multimodal optical imaging system that can longitudinally monitor formation of new blood vessels, metabolic changes, and collagen deposition in a non-invasive, label-free manner. RESEARCH DESIGN AND METHODS: The closure of a skin wound in (db/db) mice, which presents delayed wound

Recognizing lung cancer and stages using a self-developed electronic nose system.

Exhaled breath contains thousands of gaseous volatile organic compounds (VOCs) that could be used as non-invasive biomarkers of lung cancer. Breath-based lung cancer screening has attracted wide attention on account of its convenience, low cost and easy popularization. In this paper, the research of lung cancer detection and staging is conducted by the self-developed electronic nose (e-nose) system. In order to investigate the performance of the device in distinguishing lung cancer patients from healthy controls, two feature extraction methods and two different classification models were adopt

Blood gas partition coefficient and pulmonary extraction ratio for propofol in goats and pigs.

The interpretation of continuously measured propofol concentration in respiratory gas demands knowledge about the blood gas partition coefficient and pulmonary extraction ratio for propofol. In the present investigation we compared both variables for propofol between goats and pigs during a propofol anaesthesia. In ten goats and ten pigs, expired alveolar gas and arterial and mixed venous blood samples were simultaneously drawn during total intravenous anaesthesia with propofol. The blood gas partition coefficient and pulmonary extraction ratio were calculated for both species. Non-parametric

Bronchial mucus transport.

Effective clearance of inhaled particles requires mucus production and continuous mucus transport from the lower airways to the oropharynx. Mucus production takes place mainly in the peripheral airways. Mucus transport is achieved by the action of the ciliated cells that cover the inner surface of the airways (mucociliary transport) and by expiratory airflow. The capacity for mucociliary transport is highest in the peripheral airways, whereas the capacity for airflow transport is highest in the central airways. In patients with airways disease, mucociliary transport may be impaired and airflow

Breath volatiles reflect diabetic gastroparesis and its recovery by activating Nav1.8-expressing neurons in mice.

Patients with diabetic gastroparesis (DG) experience chronic, challenging symptoms and currently lack effective treatment options. This study examined whether activating Nav1.8-expressing neurons could improve gastric motility and whether volatile organic compounds (VOCs) in exhaled breath could serve as non-invasive indicators of disease status and treatment response. In streptozotocin-induced male diabetic mice, chemogenetic activation of Nav1.8-expressing neurons improved gastric motility in DG mice. At the same time, breath VOC profiles differed between DG and non-diabetic controls, with d

A novel hybrid sensor array with enhanced sensitivity and selectivity for biomarker detection of multiple respiratory diseases.

Electronic nose is an emergent technique for noninvasive disease detection via breath analysis, which is, however limited by the sensitivity and selectivity of the sensor array to complex biomarkers. In this study, we developed a hybrid sensor array to identify various biomarkers of different respiratory diseases, providing a rapid and convenient diagnostic method. Five on-chip microsensors were fabricated and combined with 11 commercial sensors to achieve enhanced sensitivity and complementary selectivity for multiple biomarkers. The array performances were significantly improved by globally

Association of multiple air pollutants with oxygen saturation during sleep in COPD patients: Effect modification by smoking status and airway inflammatory phenotypes.

Air pollution contributes substantially to the development of chronic obstructive pulmonary disease (COPD). To date, the effect of air pollution on oxygen saturation (SpO2) during sleep and potential susceptibility factors remain unknown. In this longitudinal panel study, real-time SpO2 was monitored in 132 COPD patients, with 270 nights (1615 h) of sleep SpO2 recorded. Exhaled nitric oxide (NO), hydrogen sulfide (H2S) and carbon monoxide (CO) were measured to assess airway inflammatory characteristics. Exposure levels of air pollutants were estimated by infiltration factor method. Generalized

Exploring the performance of a functionalized CNT-based sensor array for breathomics through clustering and classification algorithms: from gas sensing of selective biomarkers to discrimination of chronic obstructive pulmonary disease.

An array of carbon nanotube (CNT)-based sensors was produced for sensing selective biomarkers and evaluating breathomics applications with the aid of clustering and classification algorithms. We assessed the sensor array performance in identifying target volatiles and we explored the combination of various classification algorithms to analyse the results obtained from a limited dataset of exhaled breath samples. The sensor array was exposed to ammonia (NH3), nitrogen dioxide (NO2), hydrogen sulphide (H2S), and benzene (C6H6). Among them, ammonia (NH3) and nitrogen dioxide (NO2) are known bioma
