Spread of the radial SNAP: a pitfall in the diagnosis of carpal tunnel syndrome using standard orthodromic sensory conduction study.

OBJECTIVE: To investigate the occurrence of the spread of the radial sensory nerve action potential (SNAP) among patients with carpal tunnel syndrome (CTS) during standard median orthodromic sensory conduction study (SCS) using index finger stimulation. METHODS: We prospectively examined 74 hands in 56 CTS patients. We stimulated the index finger using ring electrodes. SNAPs were recorded at wrist over median and radial nerves. RESULTS: A spread of radial SNAP was clearly identified over the median nerve despite its small amplitude, in 72/74 hands during stimulation of the base of the index

A Comparison between Two Intervals of Daily Total End Range Time for Treatment of Proximal Interphalangeal Joint Flexion Contracture Using an Elastic Tension Digital Neoprene Orthosis.

Focusing on fingers with proximal interphalangeal joint flexion contractures, this study seeks to determine whether significant differences exist between the joint passive range of motion PROM improvement when receiving higher doses of daily total end range time (TERT) compared to those that receive lower doses. The study randomized a parallel group of fifty-seven fingers in fifty patients with concealed allocation and assessor blinding. Divided into two groups receiving different doses of daily total end range time with an elastic tension digital neoprene orthosis, they also participated in a

An Adaptive Actuation Mechanism for Anthropomorphic Robot Hands.

This paper presents an adaptive actuation mechanism that can be employed for the development of anthropomorphic, dexterous robot hands. The tendon-driven actuation mechanism achieves both flexion/extension and adduction/abduction on the finger's metacarpophalangeal joint using two actuators. Moment arm pulleys are employed to drive the tendon laterally and achieve a simultaneous execution of abduction and flexion motion. Particular emphasis has been given to the modeling and analysis of the actuation mechanism. More specifically, the analysis determines specific values for the design parameter

[Surgery of the wrist and hand in rheumatoid arthritis].

Wrist and hand surgery in rheumatoid arthritis has continued to evolve since first being tried fifty years ago. Improvements have flowed from a better understanding of pathophysiological mechanisms, from technical progress and from more effective medical treatment. The use of methotrexate and the subsequent development of biological therapies have transformed the course of the disease, increasing the chances of positive and durable surgical results. Priority must be given to re-alignment of the wrist in order to protect the fingers. It is equally important to perform synovectomies of the extri

[A Case of Hypertrophic Pulmonary Osteoarthropathy Associated with Pulmonary Pleomorphic Carcinoma].

Hypertrophic pulmonary osteoarthropathy(HPO)is a tumor-associated syndrome that features the triad of clubbed fingers, periosteal bone growth in long bones, and arthritis, and is often associated with an adenocarcinoma or squamous cell carcinoma. This report presents details of a case of HPO associated with pleomorphic carcinoma, which was relieved by treatment. A 47-year-old woman was presented with a complaint of generalized arthralgia. A physical examination showed swollen joints in the body and clubbed fingers. Chest CT revealed a mass shadow in the left upper lobe and ultrasound- guided b

Role of the juncturae tendinum in preventing radial subluxation of the extensor communis tendons after ulnar sagittal band rupture: a cadaveric study.

Background. Radial subluxation of the extensor communis tendons at the metacarpophalangeal (MCP) joints is a rarely reportedvinjury. These injuries have proved difficult to reproduce in cadaveric studies and have a low biomechanical likelihood of occurrence due to the ulnar direction of pull of the extensor communis tendons. It has been suggested that the juncturae tendinum may have a stabilising role, preventing radial subluxation after ulnar sagittal band rupture; however this has not been established. Methods. 40 cadaveric digits were dissected to reveal the extensor mechanism around the MC

Impact of metacarpal shortening on finger strength following non-surgical treatment of spiral and oblique metacarpal shaft fractures.

BACKGROUND: Treatment options for spiral/oblique metacarpal shaft fractures (MSFs) include both operative and non-operative approaches. Non-operative treatment with early mobilization has been shown to reduce treatment costs and sick leave, while maintaining grip strength despite metacarpal shortening. However, the impact of metacarpal shortening on strength at the metacarpophalangeal (MCP) or proximal interphalangeal (PIP) joints remains unclear. This study aimed to evaluate whether a shortening of more than 2 mm in spiral/oblique MSFs affects the strength of a single finger. METHODS: A tota

Force Transfer through the Scaphotrapeziotrapezoid (STT) Joint.

Background  The scaphotrapeziotrapezoidal (STT) joint transfers forces to the proximal carpal row from the thumb and fingers. Clinically, STT joint osteoarthritis is frequently observed on plain radiographs though its role in the mechanics of the wrist joint remains unclear. Questions/Purposes  Our purpose was to use a model of normal wrist types, to predict STT motion upon load. Patients and Methods  Five normal computed tomography scans of a wrist type 1 and five wrist type 2 were used to model the wrist. A 200-N force was split and applied to the trapezoid and capitate to replicate forces d

Treatment of chronic mallet finger with swan neck deformity using a modified Spiral Oblique Retinacular Ligament (SORL) reconstruction procedure: A case series and technical note.

INTRODUCTION AND IMPORTANCE: Chronic terminal extensor tendon injury produces mallet deformity and secondary swan neck deformity. It can be found in neglect cases and in a failed cases after conservative treatment or primary surgical repair. Surgery is considered in cases with extensor lag of more than 30° and functional deficit. Reconstruction of the spiral oblique retinacular ligament (SORL) has been reported in literatures to correct swan neck deformity by a dynamic mechanical basis. CASE PRESENTATION: Three cases of chronic mallet finger associated with swan neck deformity were treated by

Effect of slit-regulated 3D embroidered sensor structures on contact area-variation-based strain sensing performance in smart gloves.

Accurate sensing of finger joint angles is a key objective in wearable motion sensing systems for rehabilitation applications and requires high sensing reliability and accuracy. However, conventional textile-based strain sensors often exhibit low reliability, characterized by baseline drift and the double-peak phenomenon during repetitive deformation. These limitations are mainly attributed to the length-variation-based sensing mechanism, in which repeated deformation induces changes in the sensor's length and shape, leading to signal instability. To address these limitations, this study adopt…

Arthroplasty of the Proximal Interphalangeal Joint With the TACTYS Prosthesis: Clinical and Radiographic Results With a Mean Follow-up of 5 Years.

BACKGROUND: Degenerative arthritis of the proximal interphalangeal (PIP) joint of the long fingers is a common disorder affecting mainly a female middle-aged population. Conservative treatment is often effective, but in some cases, pain can persist which can lead to invalidating function. Besides denervation and arthrodesis, arthroplasty is a valuable alternative treatment. The goal of this retrospective study was to determine the clinical and radiological outcomes of the TACTYS prosthesis with a mean follow-up of more than 5 years. METHODS: Between October 2005 and August 2019 10 joints in 9

Finger stability in precision grips.

Stable precision grips using the fingertips are a cornerstone of human hand dexterity. However, our fingers become unstable sometimes and snap into a hyperextended posture. This is because multilink mechanisms like our fingers can buckle under tip forces. Suppressing this instability is crucial for hand dexterity, but how the neuromuscular system does so is unknown. Here we show that people rely on the stiffness from muscle contraction for finger stability. We measured buckling time constants of 50 ms or less during maximal force application with the index finger—quicker than feedback latencie
