Mesh erosion into caecum following laparoscopic repair of inguinal hernia (TAPP): a case report and literature review.

Repair of inguinal hernia is the most commonly performed surgical procedure. Both open and laparoscopic methods are accepted modalities of surgical treatment. Transabdominal preperitoneal (TAPP) and total extraperitoneal (TEP) are the two types of laparoscopic repair of the inguineal hernia. The main advantages of laparoscopic repair, as compared to open repair, are a shorter hospital stay and a quicker recovery to normal activities. However, laparoscopic repairs are associated with a higher incidence of visceral and vascular injuries. One particular complication is the migration and erosion o

Extraction of the inner coil of a pacemaker lead slid into the pulmonary artery.

A patient required lead extraction for chronic draining sinus, due to abandoned leads. Preoperatively, the chest film showed a filament in the right pulmonary artery: it was the inner coil of an old atrial lead that, while remaining anchored to the auricle, slid outside the outer coil. By right subclavian approach, the old ventricular lead and the outer coil of the atrial lead were removed. Then, by right jugular approach, the freely floating end of the inner atrial coil was grasped by a pig-tail catheter, drawn back into the superior vena cava, exteriorized by a Lasso catheter, and finally ex

[Early migration of the device for transcatheter closure of atrial septal defect. Case report].

The transcatheter closure of the atrial septal defect (ASD) has been proposed as an alternative to surgical therapy for the ostium secundum ASD. It is considered an effective and safe procedure, although rare but potentially fatal complications have been reported. The authors report a case of early migration of a device for transcatheter closure of an ASD to the right pulmonary artery.

Fatal cardiac tamponade as a result of a peripherally inserted central venous catheter: a case report and review of the literature.

We present a case of fatal cardiac tamponade that occurred in association with a peripherally inserted central catheter (PICC) inserted from the right antecubital fossa. Migration of the catheter from the right atrium within 24 h of insertion lead to the administration of a potassium-enriched sodium chloride solution into the pericardial space with the development of ST-segment elevation and progression to pulseless electrical activity and, subsequently, ventricular fibrillation. Although signs of tamponade were seen on echocardiography, we propose that myocardial hyperkalaemia from the diffus

Self-expandable stent loaded with 125I seeds: feasibility and safety in a rabbit model.

OBJECTIVE: To evaluate technical feasibility and acute and subacute radiotolerance of a self-expandable stent loaded with (125)I seeds in the rabbit esophagus. METHODS: A self-expandable stent designed for esophageal application was made of 0.16mm nitinol wire and loaded with (125)I seeds (CIAE-6711). Twenty-seven stents with three different radioactive dosages (n=9 in each dosage group) were implanted in the esophagus of healthy rabbits, while nine stents alone were used as controls. The stents were perorally deployed into the esophagus under fluoroscopic guidance. Radiological follow-up inc

Septicemia in a young dog following treatment of patent ductus arteriosus via coil occlusion.

CASE DESCRIPTION-A 12-week-old female English Springer Spaniel was evaluated for lethargy, vomiting, and pyrexia 1 week after treatment of patent ductus arteriosus (PDA) via coil occlusion. CLINICAL FINDINGS-Test results were consistent with septicemia, and the assumption was made that the PDA occlusion coils were infected. Radiography revealed partial migration of the coil mass into the pulmonary artery and signs of congestive heart failure. TREATMENT AND OUTCOME-After successful treatment of the septicemia and heart failure, surgical removal of the coils and resection of the PDA were underta

Endobronchial migration of prosthetic patch after congenital diaphragmatic hernia repair.

Surgical repair of congenital diaphragmatic hernia (CDH) can be performed by means of either direct suturing of the diaphragm or positioning of a prosthetic patch. However, half of all prosthetic patches show evidence of reherniation. We describe the case of an 8-year-old girl who presented with prosthesis dislocation and fistulization in the right lower bronchus as a complication of a CDH repair that she underwent when she was 1 year old. Abdominal ultrasound and magnetic resonance imaging suggested a hernia relapse, whereas chest computed tomographic scan failed to identify the diaphragmatic

Loss of the guide wire: a case report.

A case of a lost guide wire extending from the vena cava to the back of the neck after central venous catheterization is presented. A trainee inserted a central venous catheter via the left subclavian vein in a 40-year-old male patient after surgery, but did not notice that a guide wire was completely inserted in the vein. After 6 months, the lost guide wire was seen extending from the saphenous vein through the vena cava, right atrium, right ventricle, pulmonary artery and lung tissue to the back of neck. Although percutaneous catheterization of central veins is a routine technique, it is a p

[Endoscopic transgastric drainage of a pancreatic pseudocyst--a cause of late perforation of the intestine].

The authors present a case-review of a polymorbid patient, who underwent repetitive endoscopic transgastric drainage of the pancreatic pseudocyst in chronic pancreatitis. 3 months after the stents were released, the terminal ileus perforated. Ileocaecal resection was then completed. Further postoperative course was uncomplicated and the patient's current clinical condition is good.

Osteolysis of the greater trochanter: a result of bone anchors used for abductor reattachment at total hip arthroplasty.

Although bone anchors can help to repair soft tissues to bone, they are not without potential problems. A group of 214 consecutive total hip arthroplasties performed through a direct lateral approach using bone anchors for abductor mechanism repair was evaluated at 8 to 12 (mean 10.2) years. Complications included anchor migration from the bone in 18 (8.4%) patients, a unique pattern of symptomatic progressive osteolysis involving the lateral aspect of the greater trochanter in 8 (3.7%) patients, and pathological fracture of the greater trochanter in 1 (0.5%) patient. Three patients (1.4%) wit

Vocal fold palsy due to plombage for tuberculosis.

A 67-year-old patient, who had previously undergone Lucite ball plombage for pulmonary tuberculosis, presented with a hoarse voice, intermittent stridor and breathlessness. Direct laryngoscopy confirmed a left vocal fold palsy. A left supraclavicular mass became apparent and a computerized tomograph (CT) scan showed that a Lucite ball had migrated into her supraclavicular fossa. Subsequently she developed left arm pain and weakness. The balls were removed surgically, following which her arm symptoms improved but her voice remained unchanged. Migration of implanted material should be considered

Medicolegal aspects of an unusual uterine perforation with multiload-Cu 375R.

Perforation of the uterus is one of the most serious complications associated with insertion of intrauterine contraceptive devices (IUD). According to recent studies, this lesion occurs in 0.87 per 1000 cases, but statistics generally fluctuate between 0.05 and 13 per 1000 insertions. "Primary" perforations occur at the time of IUD insertion, and "secondary," or delayed, perforations are usually assumed to be caused by reactive uterine contractions. We report an unusual case of uterine perforation, which occurred 9 days after the insertion of a Multiload-Cu 375 IUD. The criteria for differenti

Posterior lamellar gold-weight extrusion.

We report a case of a man who had undergone gold eyelid loading for lagophthalmos 22 years earlier. During examination for a complaint of foreign body sensation in the operated eye, the gold weight was found under the palpebral conjunctiva. Although extrusion of a gold weight through the skin anteriorly is well recognized, posterior extrusion has not been previously reported. Although such an extrusion involves tarsal plate erosion, the patient's complaints in this case were minor, probably due to the chronic nature of the process. This finding should serve to draw the attention of surgeons us

[Dislocation of the closing element of the disk mechanical graft at aortic valve replacement].

Presented herein is the first case of dislocation of the closing element of the artificial low-profile aortic valve to the abdominal aorta, which happened during replacement. Toward completion of extracorporeal circulation a 37-year-old man showed the signs of aortic insufficiency because of dislocation of the closing element. After resumption of extracorporeal circulation repeated replacement was accomplished. To detect the site of graft cusp embolism, transesophageal echocardiography and duplex scanning of the abdominal aorta were employed. On the 18th day following the first intervention th

Late aortic dislocation of a stent following stent angioplasty for ostial renal artery stenosis.

A patient with left RAS was treated by stent angioplasty followed by a multivessel percutaneous coronary intervention. Six months later, an aortic dislocation of the stent was diagnosed. The fully expanded stent was caught with a balloon catheter and fixed in the left external iliac artery. Stent migration after initially successful stent angioplasty for RAS is possible. Fully expanded, dislocated balloon-expandable stents can be secured by implanting them into the iliac artery.
