We present surgical clipping of a giant middle cerebral artery aneurysm. The patient is a 64-year-old woman who suffered subarachnoid hemorrhage in 2005. She was treated with coiling of the aneurysm at an outside institution. She presented to our clinic with headaches and was found on angiography to have giant recurrence of the aneurysm. To allow adequate exposure for clipping, we performed the surgery through a cranio-orbito-zygomatic (COZ) skull base approach, which is demonstrated. The surgery was performed in an operating room/angiography hybrid suite allowing for high quality intraoperati

OBJECT: Frontoethmoidal meningoencephalocele (fMEC) is relatively common in many Southeast Asian countries, with devastating aesthetic and social consequences for affected children. No cause has been detected to date. Among other factors, the authors of this paper attempt to identify a statistically significant difference in the spread of fMEC births throughout the year compared with other births. This seasonal variation in the incidence of fMEC births may provide clues to the causes of this condition.
METHODS: From a group of 175 children with fMEC who underwent surgery at the authors' human

INTRODUCTION: When a child walks in the clinic with a unilateral frontal flattening, it is usually associated in our minds with unilateral coronal synostosis. While the latter might be the most common cause of anterior plagiocephaly, it is not the only one. A patent coronal suture will force us to consider other etiologies, such as deformational plagiocephaly, or synostosis of another suture. To understand the mechanisms underlying this malformation, the development and growth of the skull base must be considered.
MATERIALS AND METHODS: There have been few reports in the literature of isolate

PURPOSE: The pterion is an anatomic landmark formed via junction of the temporal, sphenoid, parietal, and frontal bones on the lateral aspect of the cranium. Due to the proximity to the middle meningeal artery (MMA), pterion-based surgical approaches have a potential risk of arterial rupture, leading to subsequent epidural hematoma formation. This study characterized the localization of the pterion and its relationship to the MMA in the four main pterion configurations.
METHODS: Ninety-one pterions were exposed, and suture patterns were uncovered to determine pterion classification. Dura mate…

We report serial computed tomography (CT) findings in a rare case of a rapidly calcified epidural hematoma. A 21-year-old female patient was admitted to our hospital after being involved in a motor vehicle accident. An initial cranial CT revealed a right frontal bone fracture. She complained of right frontal headache, but showed no neurological deficit or tendency for bleeding. Therefore, she was treated conservatively without surgical intervention. Follow-up CT revealed an ossified epidural hematoma (EDH) 17 days after the head injury, and the ossification later thickened. However, a decrease…

Salmonella focal intracranial infections are reported rarely. They tend to occur in immunocompromised patients. We present here a case of Salmonella typhimurium epidural empyema, with osteomyelitis of the adjacent frontal bone, in a 37-year-old human immunodeficiency virus positive man who presented with a three-day history of headache, fever, and sweats. He was treated successfully with antibiotics and surgical drainage.

OBJECTIVE AND IMPORTANCE: Camurati-Engelmann disease (CED) is a rare, autosomal-dominant genetic disorder resulting in hyperostosis of the long bones and skull. Patients often develop cranial nerve dysfunction and increased intracranial pressure secondary to stenosis of nerve foramina and hyperostosis. Surgical decompression may provide symptomatic relief in select patients; however, a small number of reports document the recurrence of symptoms due to bony regrowth. We present a patient who had been treated previously with bilateral frontal and parietal craniotomy who experienced recurrence of

Rhino-orbito-cerebral mucormycosis (ROCM) is the most commonly noted form of mucormycosis, which is the most common secondary fungal infection following severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Osteomyelitis is one of the rare sequelae of ROCM, frontal osteomyelitis being the rarest. We present four patients of coronavirus disease 2019 (COVID-19)-associated mucormycosis, who presented with frontal bone osteomyelitis after being treated for ROCM surgically and medically. This is the first case series highlighting this complication in post-COVID-19 mucormycosis pat…

A simple technique for orbital aperture expansion to facilitate placement of ocular prostheses is described. Both superolateral and inferolateral orbital margins are released by means of a single burr hole craniectomy of the frontosphenoid bone behind the orbital process of the frontal bone. Vertical and horizontal marginal lengthenings are performed by a rotatory displacement of one bone segment alongside the other. The expanded osseous aperture is secured with wire and plate-and-screw fixation following a supraorbital rim craniectomy to allow an adequate fit. The result provides for easier a…

The fronto-ethmoidal mucocele is a rare condition capable of expansion and erosion of bone. Its etiology is attributed to obstruction of the fronto-nasal duct and inflammation. To elucidate the role of inflammation, mucocele fragments and fibroblasts cultured from them were examined in vitro to assess prostaglandin E2 synthesis. The cultured fibroblasts when stimulated with mononuclear cell culture supernatant produced significant levels of prostaglandin E2 and collagenase, compared with normal frontal sinus mucosa fibroblasts removed at craniotomy. These significantly elevated mucocele levels

INTRODUCTION: Nasu Hakola disease (NHD) is a progressive dementia that presents accompanied by bone cysts and, at random, epilepsy. It is an autosomal recessive hereditary disease and its genetic defect is located at the 19q13.1 chromosome. The genetic mutation was identified at DAP 12. It appears that DAP 12 is expressed in the microglial activation and the differentiation of macrophages in the central nervous system and, at the same time, in the osteoclasts in charge of bone remodelling. This double character consisting of dementia and bone cysts, which contain triglycerides and thin PAS pos

Cleidocranial dysplasia (CCD) is a rare autosomal dominant disorder of membranous bones with characteristic radio-morphological manifestations. There is disordered ossification of the skull vault and the clavicle, leading to a wide-open anterior fontanelle, frontal bossing, persistent metopic suture, multiple wormian bones, and hypermobile shoulders. Other features are absent nasal bones, the presence of supernumerary teeth, absent ossification of the pubic rami causing pseudo-widening of the pubic symphysis, pseudo-epiphyses of metacarpals and metatarsals, short distal phalanges, and a bell-s

BACKGROUND: Nonsyndromic unicoronal craniosynostosis (NS-UCS) is characterized by asymmetric craniofacial deformity and remains one of the most challenging entities in craniofacial surgery. Fronto-orbital advancement (FOAR) may be performed either unilaterally or bilaterally, yet the optimal approach remains controversial. The present study aims to compare surgical outcomes, complication profiles, and long-term stability between unilateral and bilateral FOAR to clarify selection criteria for surgical technique.
METHODS: A systematic review was conducted using the PubMed, Web of Science, and C

Hyperostosis frontalis interna (HFI) is a human skeletal lesion characterized by nodules of hyperplastic bone and thickening of the frontal bone's inner surface. Despite its prevalence in the general population and its long history of observation-it is one of the most frequently observed pathologies in gross anatomy laboratories-HFI's etiology and pathogenesis remain poorly understood. This is largely due to the lack of a thorough survey of its histology across the various stages of its development. Our study has three major aims: (1) assess HFI histology from incipient to advanced lesions; (2…

Children with syndromic forms of craniosynostosis undergo a plethora of surgical interventions to resolve the clinical features caused by th … mechanical loading of cranial joints using a custom designed set-up. Cranial loading applied to the frontal bone partially restores normal skull morphology, significantly reducing the typical brachycephalic appearance. This is underpinned by the delayed closure of the coronal suture and of the intersphenoidal synchondrosis. This study provides a novel treatmen …

The ubiquitous tongue and groove osteotomy remains the standard for treating the anterior craniosynostoses, despite the fact that significant variations in three-dimensional fronto-orbital dystopia can occur with various sutural fusions. Four variations on bandeau design are described that allow surgeons to go beyond a simple forward advancements toward a precise three-dimensional positioning (forward advancement, vertical repositioning, and medial narrowing), or allowing the avoidance of sinus crossing osteotomies that can potentially reduce infection risk. These osteotomy designs permit more
