Preparation and characterization of hydroxyapatite nanoparticles carrying insulin and gallic acid for insulin oral delivery.

Although nanoparticles carriers for oral delivery of insulin have been researched for many years, this method still fails to solve issues with toxicity, biocompatibility, and degradability in the organism. We therefore developed an innovative conjugation system to solve this problem. Nano hydroxyapatite (HAP) particles were used as the core, then polyethylene glycol (PEG) was wrapped onto the surface of hydroxyapatite, and, finally, insulin (INS) and gallic acid (GA) were conjugated with PEG. PEG functionalized HAP was increased the hydrophilicity of the nanoparticles, also protected them from

A primer on exocrine pancreatic insufficiency, fat malabsorption, and fatty acid abnormalities.

Exocrine pancreatic insufficiency (EPI) is characterized by a deficiency of exocrine pancreatic enzymes, resulting in deficits in digestion of all macronutrients, with deficiencies in digestion of fats being the most clinically relevant. The leading cause of EPI is chronic pancreatitis. However, many other causes and conditions may be implicated, including cystic fibrosis, pancreatic duct obstruction, gastric and pancreatic surgery, diabetes mellitus and other conditions. Physical and biochemical causes of EPI include decreased production and secretion of lipase, increased lipase destruction,

Topical Metronidazole for Odor Control in Pressure Ulcers.

There are many remedies that have been recommended for the treatment of foul odor associated with pressure ulcers. This article seeks to review the literature surrounding the use of metronidazole as a safe and effective solution to an oftentimes stubborn and frustrating problem. Other tools used to control odor include bleach-based solutions and charcoal dressings. Metronidazole, with its antianaerobic properties, appears to have a useful role in therapy when applied topically to a pressure ulcer. Commercially available products include 0.75% and 1% creams, gels, lotions, and intravenous solut

MR imaging findings in alcoholic and nonalcoholic acute Wernicke's encephalopathy: a review.

Wernicke's encephalopathy (WE) is a severe neurological syndrome caused by thiamine (vitamin B1) deficiency and clinically characterized by the sudden onset of mental status changes, ocular abnormalities, and ataxia. Apart from chronic alcoholism, the most common cause of WE, a lot of other conditions causing malnutrition and decreasing thiamine absorption such as gastrointestinal surgical procedures and hyperemesis gravidarum must be considered as predisposing factors. Due to its low prevalence and clinical heterogeneity, WE is often misdiagnosed, leading to persistent dysfunctions and, in so

[NSAIDs and its gastrointestinal side effects: relation of NSAIDs variety and influence of concomitant medicine].

Non-steroidal anti-inflammatory drugs (NSAIDs) are known to increase the risk of gastrointestinal injury including gastric ulcer. Topical injury under low pH environment and cyclooxygenase-1 (COX-1) inhibition resulting in gastric prostaglandin (PG) suppression are two main mechanisms of gastric damage. Topical injury can be avoided by prodrug which is activated after absorption or alter drug administration route such as suppository. But, gastric injury by PG suppression cannot be avoided even by such contrivances. Use of selective COX-2 inhibitor is another way to reduce gastric injury and ce

[Iatrogenic Cushing syndrome and mutatio tarda caused by dexamethasone containing nose drops].

Nasal steroid drops can produce iatrogenic Cushing's syndrome if the recommended dose is exceeded. We now report a 19-year-old male patient for whom dexamethasone nasal drops were prescribed because of nasal obstruction. He had been using these drops for more than 5 years, with a daily dexamethasone dose of 0.7-1.0 mg. It is likely that significant amounts of dexamethasone were swallowed and absorbed by the patient's gastrointestinal tract. As a consequence, the patient developed Cushing's syndrome with panhypopituitarism, growth retardation, osteoporosis and hypertension. A boyish size of the

Pathophysiologic mechanisms responsible for the reversible (thiamine-responsive) and irreversible (thiamine non-responsive) neurological symptoms of Wernicke's encephalopathy.

Chronic alcoholism results in thiamine deficiency as a result of poor nutrition and impaired gastrointestinal absorption of the vitamin. Pyrithiamine-induced thiamine deficiency in the rat reproduces a neurological syndrome and ultimately neuropathological damage of a nature and distribution that is similar to that encountered in Wernicke's encephalopathy in humans. Pyrithiamine-induced thiamine deficiency results in selective reversible decreases in activity of the thiamine-dependent enzyme alpha-ketoglutarate dehydrogenase and concomitant reversible changes in brain amino acids. It is propos…

The clinical importance of hypochlorhydria (a consequence of chronic Helicobacter infection): its possible etiological role in mineral and amino acid malabsorption, depression, and other syndromes.

In a previous paper evidence was presented to show that Helicobacter-induced chronic gastritis is the probable cause of most chronic hypochlorhydria. In this article evidence is presented for the clinical relevance of reduced stomach acid secretion. Reduced mineral absorption is fairly well documented and has sound theoretical support from basic chemistry. Impaired digestion of protein has been suggested by a few studies. Small intestinal bacterial overgrowth in hypochlorhydria probably leads to putrefactive breakdown of the metobolically useful products of protein digestion, thereby reducing

Gastric and intestinal absorption of oxprenolol in humans.

The gastrointestinal absorption of the beta blocker oxprenolol was investigated in four healthy subjects by an intubation technique. Oxprenolol was introduced into the stomach, dissolved in a homogenized meal containing the marker 14C-polyethylene glycol (PEG) 4000. Unlabeled PEG 4000 was perfused during the whole experiment into the duodenum at the ampulla of Vater. Samples of luminal contents were collected at regular intervals over four hours in the stomach, at the angle of Treitz, and 30 cm below this point. Blood was also collected. Oxprenolol was not absorbed in the stomach. About 80% of…

14C triolein breath test: a routine test in the gastroenterology clinic?

The absorption of 14C triolein in a standard fat meal was measured in 60 controls and 66 patients with gastrointestinal disorders by 14CO2 breath sampling. A reference range based upon cumulative eight hour values of the controls was independent of height, weight, and sex. The range was of log normal distribution and declined with age (p less than 0.05). Acceptable 'within-day' and 'between-day' reproducibility was found. All patients tested with untreated coeliac disease, pancreatic insufficiency and most with symptomatic small intestinal Crohn's disease had subnormal values. Twenty per cent

The role of intravenous hyperalimentation in intestinal disease.

This article has dealt briefly with intravenous nutrition in intestinal disorders. The indications for its use and techniques of nutritional assessment have been stressed. The use of intravenous hyperalimentation in a few of the more common diseases of the small bowel and colon has been discussed. Every patient with disease of the small or large intestine has some degree of dysfunction in the gastrointestinal tract. In many instances, this functional impairment interferes with normal ingestion or absorption of nutrients and predisposes the patient to malnutrition. The presence of malnutrition

Toxicology of oil field wastes. Hazards to livestock associated with the petroleum industry.

In oil-producing states, the proximity of livestock to drilling operations and production sites often results in poisoning of animals from ingestion of crude oil, condensate, salt water, heavy metals, and caustic chemicals. The heavy metals encountered most frequently are lead from pipe joint compound and arsenicals and chromates used as corrosion inhibitors. Numerous toxic and caustic chemicals are used in drilling muds and fluids. Crude oil and salt water spills are common occurrences around production sites. Pipeline breaks may result in exposure of livestock to crude oil or refined petrole

Structure, Stability, and Calcium Transport Mechanism of a Novel Duck Plasma Peptide-Calcium Chelate (LHDVK-Ca).

This study constructed a peptide-calcium chelate (LHDVK-Ca) using the duck plasma-derived pentapeptide LHDVK to investigate its binding mechanism, stability, and intestinal transport behavior. Spectroscopic analysis combined with isothermal titration calorimetry (ITC) revealed that LHDVK binds Ca2+ primarily through the carboxyl group of the aspartic acid residue with a single binding site and moderate affinity. Molecular simulations confirmed that Asp plays a dominant role in coordinating Ca2+. The chelate exhibited enhanced thermal stability and maintained high calcium retention under simula

Chronic magnesium supplementation in a patient with relapsing severe hypomagnesaemia due to a novel TRPM6 variant diagnosed in adulthood.

A man in his late 50s presented with muscle cramps due to severe hypomagnesaemia. Extensive evaluation established the diagnosis of adult-onset familial hypomagnesaemia with secondary hypocalcaemia (HOMG1, OMIM #602014). As expected, due to impaired gastrointestinal absorption, oral magnesium supplementation proved insufficient to maintain stable serum magnesium concentrations, leading to recurrent emergency department presentations and the need for alternative supplementation routes. Intravenous magnesium two times a week was effective for over 3 years, until device infections and venous occl…

Unlocking the Potential of Sotagliflozin in Diabetes Mellitus targeting SGLT 1 & SGLT 2: A Comprehensive Review.

Sotagliflozin, a novel dual inhibitor of sodium-glucose cotransporters 1 and 2 (SGLT1/2), represents a promising therapeutic advancement for managing diabetes mellitus. By inhibiting SGLT1 in the small intestine and SGLT2 in the kidneys, sotagliflozin uniquely improves glycemic control through reduced postprandial glucose absorption and enhanced urinary glucose excretion. This dual mechanism has shown significant benefits for both type 1 and type 2 diabetes, including reduced insulin requirements, better glycemic control, weight loss, and improved cardiovascular and renal outcomes. Clinical tr

Pernicious Anemia Unveiled: Unusual Hemolytic Complications and Clinical Implications.

Pernicious anemia (PA) is an autoimmune condition resulting in impaired vitamin B12 absorption that commonly presents with gastritis and neurological symptoms. In rare cases, associated vitamin B12 deficiency can contribute to significant red blood cell lysis, and patients can present with PA-induced pseudo-thrombotic microangiopathy (TMA) hemolytic anemia. This case describes a 59-year-old male presenting with a two-week history of gastrointestinal pain with bleeding who had anemia and hemodynamic instability on initial evaluation. After the endoscopy/colonoscopy did not reveal any active sou

Fatal pentastomiasis in captive African dwarf crocodile hatchlings (Osteolaemus tetraspis).

Six eggs from a captive African dwarf crocodile (Osteolaemus tetraspis) nest were artificially incubated. Two abnormal hatchlings with incomplete absorption of their yolk sacs died within 48 hr of hatching. No parasites were detected grossly or histologically in either hatchling. The remaining four apparently healthy hatchlings died without clinical signs at 3 wk of age, 1 wk after being fed live wild-caught fish and commercially raised goldfish. Necropsy revealed multiple white small wormlike organisms within the lungs, liver, and gastrointestinal tract of all four hatchlings. Histopathologic

Iron deficiency and anaemia in bariatric surgical patients: causes, diagnosis and proper management.

Obesity-induced chronic inflammation leads to activation of the immune system that causes alterations of iron homeostasis including hypoferraemia, iron-restricted erythropoiesis, and finally mild-to-moderate anaemia. Thus, preoperative anaemia and iron deficiency are common among obese patients scheduled for bariatric surgery (BS). Assessment of patients should include a complete haematological and biochemical laboratory work-up, including measurement of iron stores, vitamin B12 and folate. In addition, gastrointestinal evaluation is recommended for most patients with iron-deficiency anaemia.

On-Chip modeling of drug-gut interactions in Oral drug delivery.

The gastrointestinal tract is a dynamic ecosystem where biophysical forces, enzymatic gradients, and microbial metabolism converge to govern the fate of orally administered therapeutics. These multifactorial interactions-spanning shear stress, mucus transport, and microbial metabolism-collectively shape absorption, transformation, and therapeutic response. Such complexity drives the wide interindividual variability in oral pharmacokinetics and pharmacodynamics, challenging predictive modeling and formulation design. Capturing these intertwined processes requires experimental systems that bridg
