Two cases with QT prolongation associated with the administration of standard drug doses are reported. Drug effects are determined by pharmacological and biological parameters. Drugs may be metabolized or transported by two systems: the cytochrome enzymes or the less well-known but nevertheless relevant P-glycoprotein system. P-glycoprotein is a pump that limits intracellular reabsorption and favors the elimination of exogenous substances. These two systems interact with each other to influence drug effects. A better knowledge of their mechanisms of action and their genetic determinants should

In this study we compared characteristics and treatment results in patients of 60 to 79 (group I, n = 682) and of 80 to 100 years of age (group II, n = 593) treated in a geriatric hospital. Neurologic diseases (mainly stroke) predominated in group I (38%), and patients with fractures in group II (53%). Diabetes mellitus and hyperlipidemia were less frequent in patients of group II, but overt cardial insufficiency was more frequent than in group I. While the effectiveness of geriatric treatment was identical in both groups, the efficiency-defined as progress per day-was in some parts slightly h

OBJECTIVE: To determine whether psychosocial factors, chronic diseases, and common geriatric problems are associated with poor physical function 3 years after primary total hip replacement (THR).
METHODS: We studied a sample of Medicare recipients in Ohio, Pennsylvania, and Colorado (n = 922) who underwent primary THR in 1995 (mean +/- SD age 73.1 +/- 5.6 years, 32% men). Participants completed a questionnaire regarding lifestyle factors, medical history, and quality of life approximately 3 years after the surgery. Physical function was measured using the function subscale of the Western Onta

BACKGROUND: Meckel diverticulum (MD) is traditionally considered a pediatric disease that is associated with intestinal hemorrhage or perforation. Symptomatic MD is rarely a consideration in the geriatric population.
OBJECTIVE: To notify clinicians of the clinical variety and diagnostic uncertainty of MD in the elderly, we report 7 cases of complicated MD that presented as common disorders of the gastrointestinal (GI) tract in patients older than 65 years.
METHODS: A retrospective record review at 2 university-affiliated hospitals revealed 7 patients older than 65 years with MD and abdominal

BACKGROUND: Lung cancer is a major health problem throughout the world. It is the leading cause of cancer-related death in men and women in the USA, with a 5-year survival rate of only 14%. It has been hypothesized that variables such as physical and social functioning, cancer-related symptomatology, comorbid conditions, cell type, and treatment are valid predictors of the psychological response to a diagnosis of lung cancer.
METHODS: As part of a larger longitudinal study, 211 patients, 65 years of age or older, with an incident diagnosis of lung cancer, were recruited from 23 sites within a

The number of patients with geriatric diseases will rapidly increase in our aging society. Geriatric diseases tend to progress chronically and disturb the daily activity of the elderly patients. Care for the elderly patients requires a great deal of manpower. The prevention and treatment of geriatric disease are urgent issues that must be addressed. A comprehensive longitudinal study of aging and geriatric disease was started at the National Institute for Longevity Sciences (NILS) in 1997. The participants of the NILS longitudinal study of aging (NILS-LSA) were 2,267 men and women from a local

An investigative study was performed to determine the diagnosis of onychomycosis in a South Florida geriatric population. In this study, 450 cases of suspected onychomycosis involving men and women 65 years of age and older from a private practice office and two nursing home settings were used. Samples were taken from the hallux toenail and sent to a mycology laboratory for fluorescent potassium hydroxide (KOH) preparation and microscopic examination of a fungal culture. Of the 450 cases studied, 46.4% of the patients had a single fungal organism cultured, 30.4% had a mixed fungal infection cu

We report on seven geropsychiatric patients, aged 62 to 86, who had received either in- or outpatient psychiatric treatment for different disorders before being diagnosed with Huntington's disease (HD) by molecular biological methods. At the time of diagnosis, these patients presented only with mild involuntary movements in addition to other, nonspecific psychiatric symptoms such as depressive, paranoid, or dementia symptoms. In six of the seven cases, the HD symptoms had previously been misdiagnosed as tardive dyskinesia because these patients had been treated with neuroleptics in the past. F

It is well known that many elderly patients are referred to nursing homes because of "functional decline" without being thoroughly investigated. We studied 9 elderly patients, all referred to hospital due to functional decline and diagnosed as follows: spinal stenosis--2 cases, depression--3, thyrotoxicosis--1, Parkinson--1, polypharmacy and congestive heart failure--1 patient each. Proper diagnosis and appropriate treatment prevent unnecessary hospitalization in nursing homes. Our study is meant to draw attention to this crucial aspect of geriatric medicine.

BACKGROUND: Rashes and blood dyscrasias are disconcerting adverse effects associated with carbamazepine therapy. Rashes are quite common, as are mild blood dyscrasias, such as mild leukopenias. Fortunately, severe rashes and blood dyscrasias are rare. There are few reports on the relationship between carbamazepine-induced rashes and blood dyscrasias, including a prospective study in which rash appeared concomitantly with leukopenia and/or thrombocytopenia in 10 patients, two case reports in which simultaneous rash and agranulocytosis occurred, and two case reports in which rashes served as har

This prospective epidemiological survey of spinal cord injury (SCI) in Taiwan was carried out among patients attended by physicians from various medical centers and general hospitals all over Taiwan from July 1992 to June 1996. In all, 1,586 new cases of SCI were registered, representing about 70% of all possible SCI cases in Taiwan. The observed average annual incidence of SCI in Taiwan was 18.8 per million people, whereas it was 47.5 for the geriatric section. The mean age was 46.1 years with a plateau distribution after 20 years and older. Geriatric victims (297 cases, 18.7%, group II) form

This study evaluated the influence of prosthodontic status on caries a and periodontal disease in a hospitalized population of elderly people. Among the data gathered was information on caries, gingival condition, oral hygiene, plaque accumulation, and factors related to the prevalence, type and quality of the prostheses present. There was no significant association between the type of rehabilitation and the economic, social, or educational variables, but it appeared that removable partial denture wearers had less concern for oral health. Root caries and periodontal diseases were more frequent
