Alcohol cue-induced heart rate variability and drinking behavior: The role of impulsivity.

Research has demonstrated that exposure to alcohol-related cues (e.g., sight or smell of a preferred beverage) elicits heightened self-reported craving and psychophysiological responses, including changes in heart rate (HR) and heart rate variability (HRV), particularly among individuals high in trait impulsivity. While cue-induced psychophysiological reactivity is well documented, its predictive value for drinking behavior, particularly in individuals with elevated impulsivity, has received more limited empirical attention. The present study examined whether trait impulsivity predicts cardiov…

Modeling heart rate (HR) dynamics to reconstruct missing HR data in trail running.

BACKGROUND: Heart rate (HR) is widely used to guide exercise intensity because it is non-invasive and easy to measure. However, outdoor recordings often contain artifacts and gaps, which can bias downstream metrics if not reconstructed accurately. Common reconstruction methods (e.g. linear interpolation) perform well in short gaps but ignore HR kinetics and changing external load, which is problematic in trail running with frequent intensity changes. The present study presents a HR reconstruction approach that leverages GNSS data and a model of HR dynamics. METHODS: 12 recreational trail runn…

Effects of acupuncture on stride, speed, and heart rate variability in Thoroughbred racehorses.

Thoroughbred racehorses are at increased risk of injury during race preparations and trials, primarily due to intensive workloads that predispose them to musculoskeletal injuries. Evaluating clinical interventions to improve health and prevent injury is essential for addressing animal welfare concerns. This study investigated the medium-term effects of acupuncture on stride, speed and cardiac autonomic regulation (assessed via heart rate variability [HRV]), in Thoroughbred flat racehorses. This prospective, randomised, parallel-group study included an intervention and a control group. Thirty T…

Heart Rate Threshold Settings for Ventricular Tachycardia in Patients with Wearable Cardioverter-defibrillators.

The wearable cardioverter-defibrillator (WCD) uses programmable heart rate (HR) and detection time to determine the need for shock delivery, but the optimal ventricular tachycardia (VT) HR detection zone is unknown. The LifeVest™ (ZOLL Medical, Chelmsford, MA, USA) WCD has a default VT detection rate of 150 bpm. We found that 15% of appropriate shocks among WCD recipients were delivered for VT 150-170 bpm and that 30% of these patients experienced loss of consciousness. Based on these findings, a programmed VT detection threshold of 150 bpm should be considered.

Hypohydration Increases Heart Rate Despite Preserved Hemodynamic Compensation During Blood Flow Restricted Exercise.

During blood flow restriction (BFR) exercise, blood becomes sequestered in the active limbs due to impeded venous return, contributing to ce … nced: with regular fluid consumption (Hydrated), or after 24-h of fluid restriction (Hypohydrated). Heart rate, beat-to-beat blood pressure, and thoracic impedance were continuously measured, from which stroke volume and cardiac output were estimated. Hypohydration reduced body mass (-2.3% ± 0.7%, p < 0.0001) and estimated blood volume (-4.1% ± 3.1%, p < 0.0 …

Ultra-short term Heart Rate Variability in moderate and severe Obstructive Sleep Apnea.

INTRODUCTION: Obstructive sleep apnea (OSA) is characterized by intermittent hypoxia and recurrent arousals, leading to sympathetic overactivity and adverse cardiometabolic consequences. Heart rate variability (HRV), which reflects beat-to-beat fluctuations in heart rate, serves as a non-invasive marker of cardiac autonomic regulation. Although reduced long-term HRV has been described in OSA, literature on short- and ultra-short-term HRV (uST-HRV) is sparse. METHODOLOGY: In this cross-sectional study, adults with moderate and severe OSA were included. Stable, artifact-free 30-s electrocardiog…

Clinical Implications of Resting Metabolic Rate in Heart Failure With Preserved Ejection Fraction.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is causally related to obesity. Adipose tissue accumulates when energy intake exceeds expenditure. Resting metabolic rate (RMR) is a key determinant of energy expenditure, but it has not been evaluated in HFpEF. METHODS: Patients with HFpEF undergoing invasive hemodynamic exercise testing were categorized into lower- or higher-than-expected measured RMR groups based on the difference between directly measured RMR, calculated using the Weir equation (indirect calorimetry) and predicted RMR, estimated using the Mifflin-St. Jeor

Cystic Periventricular Leukomalacia in Preterm Infants: Clinical Risks, Heart Rate, and Oxygenation Patterns.

OBJECTIVE: Cystic periventricular leukomalacia (cPVL) is a major cause of neurodevelopmental impairment in preterm infants, yet associated clinical and physiologic variables remain incompletely defined. We sought to identify differences in maternal and infant clinical variables, including patterns of heart rate (HR) and oxygen saturation (SpO2) in the first 6 weeks after birth, in infants with cPVL compared with matched controls. STUDY DESIGN: We conducted a single-center, retrospective study of infants admitted to a level IV neonatal intensive care unit (NICU) from 2012 to 2022. Infants with…

Palm Cooling's Heart Rate and Thermal Impact on Exercise in Multiple Sclerosis Patients.

Heat strain in multiple sclerosis patients during exercise limits their benefits from this treatment. One prior study examined this topic but did not measure thermal outcomes and constitutes a knowledge gap. The current pilot study assessed palm cooling for its impact on heart rates and thermal measures. Using a randomized within-subjects design, nine multiple sclerosis patients performed two 25-minute cycle ergometry workouts against 65 and 45% of their Astrand-Rhyming-based workloads. Heart rates, thermal flux, palm and tympanic temperatures were examined using two-way analyses of variance w…

Stressor-evoked heart rate, perceived physiological arousal, and anxiety symptoms in young adults.

BACKGROUND: Acute psychological stress is associated with increases in heart rate (HR) and state anxiety. Stressor-evoked perceived HR changes relate more strongly to state anxiety than actual HR reactivity. However, research has yet to examine these associations in the context of general anxiety symptoms. METHODS: Participants (N = 160) completed a resting baseline period followed by a standardized acute psychological stress task; HR was recorded throughout. After the stress task, participants rated their perceived stressor-evoked physiological arousal and anxiety intensity. Participants als…

Heart rate variability in children and adolescents with incidentally found early repolarization pattern.

BACKGROUND: Early repolarization pattern (ERP) on electrocardiogram (ECG) was long considered benign, but recent data suggest a potential association with fatal arrhythmia and sudden cardiac death. Its relevance in pediatric populations remains unclear. This study investigated the risk of premature death and arrhythmia in children with incidentally found early repolarization using ECG and heart rate variability parameters. METHODS: This cross-sectional study included healthy children aged 6-18 years with incidentally detected ERP (study group) and age- and sex-matched controls without ERP. Al…

Effects of Induced Physical Fatigue on Heart Rate Variability in Healthy Young Adults.

Detecting physical fatigue can help prevent overexertion. While typically defined at the muscle level, systemic fatigue remains less clear. Heart rate variability (HRV) reflects autonomic adaptability to physical stressors and may provide insight into fatigue-related responses. This study investigated the impact of physical fatigue on HRV and its correlation with endurance performance. Twenty participants (9 F, 11 M; 23.4 ± 5.0 y) walked on the treadmill at 1.25 m/s with progressively increased incline. HRV metrics were derived from baseline standing (STAND), pre-fatigued (PRE) and post-fatigu…

The role of heart rate variability in acute mountain sickness: A meta-analysis.

BACKGROUND: Acute mountain sickness (AMS) is associated with symptoms arising from an individual's failure to acclimatize to high-altitude environments characterized by hypobaric hypoxia. Heart rate variability (HRV) has been proposed as a potential predictor of AMS, but results from individual studies have been inconsistent. Therefore, this study investigated HRV before and after ascent to provide a more comprehensive review of the relationship between HRV and AMS. METHODS: We conducted a systematic search of MEDLINE, PubMed, and Google Scholar from inception to August 2023. Studies measurin…

Association Between Pressure-Adjusted Heart Rate and In-Hospital Mortality in Cardiogenic Shock.

BACKGROUND: Among patients with cardiogenic shock (CS), higher right atrial pressure (RAP) and lower mean arterial pressure (MAP) are associated with higher in-hospital mortality. Pressure-adjusted heart rate (PAHR), defined as heart rate × RAP/MAP, integrates these parameters. The prognostic significance of PAHR has not been assessed in patients with CS. OBJECTIVES: The authors aimed to assess if PAHR values are associated with risk of in-hospital mortality in patients with CS. METHODS: CCCTN (Critical Care Cardiology Trials Network) is a multinational registry of cardiac intensive care uni…

Heart rate variability in the clinical assessment of patients with chronic liver disease.

Autonomic dysfunction (AD) is frequently observed in cirrhotic patients and is associated with poor clinical outcomes and prognoses. Heart rate variability (HRV), a noninvasive tool for assessing autonomic nervous system balance, has been extensively studied in a variety of conditions, including chronic liver disease (CLD); however, no recent reviews have focused on its role in CLD. This article examines the mechanisms of AD in CLD and the foundation for HRV assessment, highlighting its diagnostic, prognostic, and therapeutic applications in CLD, including liver transplantation (LT). Changes i…

Longitudinal fetal heart rate analysis identifies neonates with metabolic acidemia requiring therapeutic hypothermia.

BACKGROUND: Despite widespread use of intrapartum fetal monitoring, rates of fetal brain injury remain unchanged. Neonatal encephalopathy due to hypoxia-ischemia is a leading cause of neonatal morbidity and mortality, often occurring in the absence of acute intrapartum events. Current fetal heart rate interpretation guidelines rely on static, cross-sectional assessments and offer limited guidance for managing "indeterminate" patterns. Machine learning provides an objective, longitudinal analysis of the entire intrapartum fetal heart rate tracing and may enhance the identification of fetuses at…
