The floating hip: complications and outcomes.

BACKGROUND: To perform a descriptive study of the course, treatment decisions, complications, and outcome of patients suffering simultaneous … f patients. CONCLUSIONS: Ipsilateral injuries to the femur and the pelvis or acetabulum ("floating hip") are severe injuries usually caused by high-energy trauma. The acetabulum and pelvic ring are more commonly fractured together than either alone. The femur fracture will most commonly be addressed first, as in 65% of our cases in which both compone …

Arthroscopy of the hip in children and adolescents.

Intra-articular abnormalities of the hip, such as labral tears, loose bodies, chondral lesions, ligamentum teres tears and femoral acetabular impingement are increasingly being recognized in the pediatric age group. Evaluation for these abnormalities starts with a good history and physical exam. Radiographic imaging with plain films and magnetic resonance imaging help confirm the clinical impression. Arthroscopy of the hip can be utilized to diagnose and treat these abnormalities. Arthroscopy of the hip is a challenging procedure with a learning curve that requires a thorough knowledge of the …

Resurfacing arthroplasty of the hip in osteopetrosis.

A 34-year-old woman with a benign form of osteopetrosis developed osteoarthritis of the hip. In order to avoid the difficulties associated with inserting the femoral component of a conventional total hip arthroplasty, a hybrid metal-on-metal resurfacing was performed. There were several technical challenges associated with the procedure, including the sizing of the component, press-fit fixation of the acetabular component and femoral head preparation, as well as trying to avoid a fracture. No surgical complication occurred. After more than a year following surgery, the patient showed excellent…

Effusion of the hips in a patient with tetraplegia.

BACKGROUND: Patients with spinal cord injury are at risk for knee effusion, most likely as a result of repetitive microtrauma. Patients with paralysis are susceptible to effusions of the hip similar to those seen in documented cases regarding the knee. The etiology is likely similar and is related to repetitive microtrauma, such as that experienced when aggressive range of motion exercises are applied. DESIGN: Case report. SETTING: Acute rehabilitation department of a spinal cord injury center. FINDINGS: A 19-year-old man with a complete cervical spinal cord injury presented to acute rehabi…

Hip arthroscopy in children and adolescents.

Hip arthroscopy has become an established procedure for certain indications in adults, but experience in children and adolescents has been more limited. The purpose of this study is to report the early-term results of hip arthroscopy in children and adolescents. A consecutive case series of 54 hip arthroscopies in 42 patients 18 years old and younger over a 3-year period at a tertiary-care children's hospital with a minimum of 1 year of follow-up was reviewed. Patients were assessed with the modified Harris hip score (HHS) before and after surgery. Overall results and results by common diagnos…

An overview of hip injuries in running.

Running has steadily gained in worldwide popularity and is the primary exercise modality for many individuals of all ages. Its low cost, versatility, convenience and related health benefits appeal to men and women of broad cultural, ethnic and economic backgrounds. With more children and adults participating in recreational and competitive running, the incidence of injuries has steadily increased. Most running-related injuries affecting the lower extremities are due to preventable training errors, and some may necessitate medical evaluation or a significant reduction in training. Hip injuries …

The acetabular teardrop and ultrasonography of the hip.

The acetabular teardrop is an important tool for the diagnosis of acetabular dysplasia. Since it was first described by Köhler its widespread use has not become established due to the relative subjectivity in classification of the types. The acetabular teardrop angle (ATA) is proposed for this purpose, and to show the teardrop's direct relation to acetabular development by correlating it with hip ultrasonography results. Patients (51 male, 53 female) included in this study received hip ultrasonography and concomitant pelvic radiography. In the 296 hips, alpha and beta angles were measured from…

Total resurfacing for osteonecrosis of the hip.

With the resurgence of metal-on-metal bearings, there is renewed interest in total hip resurfacing. A cementless acetabular component used for resurfacing with a wall thickness of fewer than 5 mm is comparable in size to acetabular components used for total hip replacement. The fixation of a porous-ingrowth acetabular component used for resurfacing has been shown to be reliable and durable. There are few clinical reports of total hip resurfacing that stratify results by diagnosis. However, available evidence indicates that the pain relief, function, and activity after total hip resurfacing for…

Detecting developmental dysplasia of the hip.

Recent guidelines from the American Academy of Pediatrics mandate the universal screening of all newborns for developmental hip dysplasia, in an effort to promote prompt recognition and treatment of this condition. The effectiveness of universal screening hinges on the appropriate education, training, and experience of health care providers. This article reviews the normal embryologic, natal, and postnatal development of the hip. Historical risk factors that may have an impact on this developmental sequence are outlined. A step-by-step guide to the examination of the hip is offered, along with…

Hip arthroscopy in the presence of dysplasia.

PURPOSE: Intra-articular pathology of the hip is often associated with dysplasia. The presence of dysplasia is often believed to be a harbinger of poor results for arthroscopy. The purpose of this study was to report the results of operative hip arthroscopy in conjunction with dysplastic disease of the hip. TYPE OF STUDY: Prospective clinical case series. METHODS: All patients undergoing hip arthroscopy are prospectively assessed with a modified Harris hip score performed preoperatively and postoperatively at 3, 12, 24, and 60 months. A minimum of 1-year follow-up has currently accumulated f…

[Prevention of hip fracture in the elderly].

Hip fracture directly resulted in bedridden state in the elderly. Both development of osteoporosis and contusion of greater trochanter by accidental fall are important causes for the fracture. Large-scale interventions with randomized controlled methods revealed that alendronate, risedronate, some estrogens and hip protector were effective for prevention of hip fractures in the elderly although newly administration of estrogens are not recommended due to their increasing effects for cardiovascular complications. This report gives an outline fo prevention of hip fracture in the elderly.

Hip arthroscopy in the pediatric population.

Hip arthroscopy has obvious advantages over arthrotomy in the pediatric population. Hip arthroscopy, used as a diagnostic or therapeutic tool, is significantly less invasive than arthrotomy and allows for quicker recovery and return to activities. In addition, arthroscopy avoids dislocation of the femoral head and the corresponding risk of osteonecrosis. Current indications for hip arthroscopy in pediatric patients include septic arthritis, labral disorders, slipped capital femoral epiphysis, and Legg-Calvé-Perthes disease. The role of hip arthroscopy in the pediatric population will continue …

[Ultrasound screening of the infant hip].

The sonographic screening of the hip has led to a marked decrease in treatment of children with developmental dysplasia of the hip (ddh) as inpatients. The prognosis for this disorder has greatly improved in the past few years due to early diagnosis and correspondingly early beginning of therapy, in many cases by using simple therapeutic methods. This undisputed success has been achieved with a high treatment rate in comparison with international practice (6.4%). The expectancy of reducing the number of checks, and thus costs, by deciding on the 4th to 6th week of life as the time for screenin…

[Acquired pathology of the hip in children].

Hip pathology is a frequent motive for consultation in the field of paediatrics. The clinical examination and the radio-imagery both play a central role in the diagnostic process. If acute benign synovitis is the principal cause in the child, it remains a diagnosis of exclusion and must not in any case lead to a missed diagnosis of septic arthritis. Other diagnoses should be evoked according to the age of the child: epiphysiolysis, true surgical emergency, primary osteochondritis of the hip, early inflammatory polyarthritidies.

The assessment of athletic hip injury.

The differential diagnosis of athletic hip pain is extensive. The physical demands of sport place the athlete at increased risk to injure the hip. The basis of successful treatment of the athlete with hip pain lies in the ability of the physician to assess and diagnose the etiology accurately. A thorough understanding of the evaluation of the athlete with hip pain allows for an earlier diagnosis, earlier treatment, and ultimately an earlier return to sport.
