Osteoarthritis phenotypes: advancing precision medicine through clinical, structural, and molecular stratification.

PURPOSE: Osteoarthritis (OA) is now understood as a heterogeneous syndrome driven by diverse biological, biomechanical, metabolic, genetic, and molecular mechanisms. This variability explains differences in disease progression and treatment response, challenging the traditional "one-size-fits-all" approach. This review highlights OA phenotyping as a key step toward precision medicine, focusing on clinical, structural, and molecular classifications that inform individualized care. METHODS: A narrative review was conducted using a non-systematic search of major databases and Osteoarthritis Rese

Secreted luciferases as a minimally invasive 3R-compliant tool for accurate monitoring of tumor burden.

Preclinical mouse models are indispensable in cancer research, providing insights into tumor biology and therapeutic responses. This protocol describes a minimally invasive blood-based tumor monitoring approach using secreted luciferases for longitudinal tracking of tumor burden in transplantable xenografts and genetically engineered mouse models. Unlike intracellular luciferases used in bioluminescence imaging, secreted luciferases are actively released into circulation, enabling precise quantification from microliter-scale blood samples. We describe a transplantable model, where tumor cells

The Co-Transduction Conundrum: Improving Dual rAAV Delivery Through Co-Administration of Capsid Serotypes With Complementary Tropism.

PURPOSE: Recombinant adeno-associated virus (rAAV) vectors are promising tools for the treatment of inherited retinal diseases (IRDs) but have a limited carrying capacity for therapeutic cassettes of up to 4.8 kilobase (kb). To circumvent this limitation, multiple vector approaches have been proposed wherein the transgene cassette is split across two or more rAAV vector genomes. In this study, we examine whether rAAV serotype choice has an effect on co-transduction of retinal cells following subretinal delivery and whether co-transduction represents a rate-limiting step preventing successful l

Understanding Causal Relationships Between Imaging-Derived Phenotypes and Parkinson's Disease: A Mendelian Randomization and Observational Study.

Background/Objectives: Observational studies have suggested a correlation between brain imaging alterations and Parkinson's disease (PD). However, data on causal relationships are still lacking. This study aimed to examine the causal relationship between brain imaging-derived phenotypes (IDPs) and PD. Methods: A bidirectional two-sample Mendelian randomization analysis was conducted to explore the causal association between IDPs and PD. Summary-level data for IDPs (n = 39,691), PD (n = 482,730), and PD symptoms (n = 4093) were obtained from genome-wide association studies of European ancestry.

Carotid Endarterectomy Ameliorates Cognitive Impairment in Clinical and Experimental Unilateral Carotid Artery Stenosis.

BACKGROUND: Carotid endarterectomy (CEA) is widely used to treat carotid artery stenosis (CAS). However, the effects of CEA on unilateral CAS-induced cognitive impairment and the underlying mechanism remain poorly understood. METHODS AND RESULTS: Thirteen patients diagnosed with unilateral severe CAS underwent pre- and post-CEA assessments, including 18fluoro-2-deoxy-d-glucose positron emission tomography/magnetic resonance imaging, cognitive assessments, and routine blood tests before and after CEA. Unilateral carotid common artery occlusion and ligation release (reperfusion) surgeries were

Brain Proteome-Wide Association Study Identifies Candidate Genes that Regulate Protein Abundance Associated with Post-Traumatic Stress Disorder.

Although previous genome-wide association studies (GWASs) on post-traumatic stress disorder (PTSD) have identified multiple risk loci, how these loci confer risk of PTSD remains unclear. Through the FUSION pipeline, we integrated two human brain proteome reference datasets (ROS/MAP and Banner) with the PTSD GWAS dataset, respectively, to conduct a proteome-wide association study (PWAS) analysis. Then two transcriptome reference weights (Rnaseq and Splicing) were applied to a transcriptome-wide association study (TWAS) analysis. Finally, the PWAS and TWAS results were investigated through brain

Detecting immunotherapy-sensitive subtype in gastric cancer using histologic image-based deep learning.

Immune checkpoint inhibitor (ICI) therapy is widely used but effective only in a subset of gastric cancers. Epstein-Barr virus (EBV)-positive and microsatellite instability (MSI) / mismatch repair deficient (dMMR) tumors have been reported to be highly responsive to ICIs. However, detecting these subtypes requires costly techniques, such as immunohistochemistry and molecular testing. In the present study, we constructed a histology-based deep learning model that aimed to screen this immunotherapy-sensitive subgroup efficiently. We processed whole slide images of 408 cases of gastric adenocarci

OCT findings in young asymptomatic subjects carrying familial BEST1 gene mutations.

PURPOSE: Best disease is an autosomal dominant retinal degeneration characterized by the presence of yellow lesions in the macula causing decreased central visual acuity at later stages. Best disease is caused by heterozygous mutations in BEST1, a gene located at chromosome 11q13. In the present study, we describe the clinical and molecular analysis of two multigenerational families with Best disease and correlate the optical coherence tomography (OCT) findings in asymptomatic and symptomatic subjects carrying BEST1 mutations. METHODS: Two Mexican families with 3 affected generations each wer

HRDPath: an explainable multi-model deep learning architecture for predicting homologous recombination deficiency from histopathology images.

Homologous recombination deficiency (HRD) is a critical biomarker for guiding treatment decisions in high-grade serous tubo-ovarian carcinoma (HGSOC), a cancer with few reliable biomarkers. However, existing genomics-based tests for HRD are variable, expensive, and time-consuming. We develop HRDPath, a patient-level deep learning architecture that predicts HRD status from whole slide images, requiring no additional molecular testing. HRDPath is validated across three cohorts of female patients and achieves an AUC of 0.846, significantly outperforming previously reported results in HGSOC. By in

Could thiamine supplementation delay the progression of retinal dystrophy in Rogers syndrome?

Thiamine-responsive megaloblastic anemia (TRMA) syndrome is a rare defect of thiamine transport caused by biallelic SLC19A2 gene mutations. It is characterized by a triad of diabetes mellitus, megaloblastic anemia responsive to thiamine, and sensorineural hearing loss. The pigmentary retinopathy associated with this syndrome remains poorly investigated. Furthermore, while thiamine supplementation is known to reverse some of the systemic features of this syndrome, its therapeutic role in managing the pigmentary retinopathy remains unknown. We report a longitudinal characterization of TRMA syndr

A novel missense variant in the calcium-sensing receptor gene in a dog with hypercalcemia.

A 5-year-old female spayed Border Collie cross was evaluated for a 3-year history of hypercalcemia. Abnormalities were not detected on physical examination. Parathyroid hormone (PTH) concentration was at the upper end of the reference interval, but diagnostic imaging did not identify any enlarged or ectopic parathyroid tissue. Hypercalcemia failed to resolve with unilateral parathyroidectomy, or administration of bisphosphonates, and calcimimetics. Despite hypercalcemia, urine calcium excretion was inappropriately low, consistent with Familial Hypocalciuric Hypercalcemia described in humans wi

A CRISPR view on genetic screens in Toxoplasma gondii.

Genome editing technologies, such as CRISPR-Cas9, have revolutionised the study of genes in a variety of organisms, including unicellular parasites. Today, the CRISPR-Cas9 technology is vastly applied in high-throughput screens to investigate interactions between the Apicomplexan parasite Toxoplasma gondii and its hosts. In vitro and in vivo T. gondii screens performed in naive and restrictive conditions have led to the discovery of essential and fitness-conferring T. gondii genes, as well as factors important for virulence and dissemination. Recent studies have adapted the CRISPR-Cas9 screeni

NEMF-mediated CAT tailing facilitates translocation-associated quality control.

Ribosome stalling during co-translational translocation at the ER causes translocon clogging and impairs ER protein biogenesis. Mammalian cells resolve translocon clogging via a poorly characterized translocation-associated quality control (TAQC) process. Here, we combine a genome-wide CRISPR screen with live-cell imaging to dissect the molecular linchpin of TAQC. We show that TAQC substrates translated from mRNAs bearing a ribosome-stalling poly(A) sequence are degraded by lysosomes and the proteasome. By contrast, the degradation of defective nascent chains encoded by nonstop (NS) mRNAs invo

A Fatal Case of Monkeypox-Associated Encephalitis in a Non-HIV-Infected Patient.

BACKGROUND: Mpox, formerly known as monkeypox, is an infectious disease caused by the mpox virus. Systemic involvement is rare and mpox-associated meningoencephalitis is an uncommon clinical presentation. Because mpox-associated meningoencephalitis is characterized by nonspecific clinical and incidental imaging findings, the disease is difficult to diagnose. CASE SUMMARY: We present a case of fatal mpox-associated meningoencephalitis with negative polymerase chain reaction in cerebrospinal fluid (CSF) diagnosed at autopsy. A young immunocompetent patient with no significant medical history in

Musculoskeletal morphogenesis supports the convergent evolution of bat laryngeal echolocation.

The order Chiroptera (bats) is the second largest group of mammals. One of the essential adaptations that have allowed bats to dominate the night skies is laryngeal echolocation, where bats emit ultrasonic pulses and listen to the returned echo to produce high-resolution 'images' of their surroundings. There are two possible scenarios for the evolutionary origin of laryngeal echolocation in bats: (1) a single origin in a common ancestor followed by the secondary loss in Pteropodidae, or (2) two convergent origins in Rhinolophoidea and Yangochiroptera. Although data from palaeontological, anato

Medullary Sponge Kidney and Its Relationship with Primary Distal Renal Tubular Acidosis: Case Reports and a Comprehensive Genetics-First Approach.

Medullary sponge kidney (MSK) is a description of radiographic features. However, the pathogenesis of MSK remains unclear. MSK is supposed to be the cause of secondary distal renal tubular acidosis (dRTA), although there are case reports suggesting that MSK is a complication of primary dRTA. In addition to these reports, we report 3 patients with metabolic acidosis and MSK, in whom primary dRTA is confirmed by molecular genetic analyses of SLC4A1 and ATP6V1B1 genes. With a comprehensive genetics-first approach using the 100,000 Genomes Rare Diseases Project dataset, the association between MSK
