Engineered electrical conduction tract restores conduction in complete heart block: from in vitro to in vivo proof of concept.

BACKGROUND: Cardiac electrical conduction delays and blocks cause rhythm disturbances such as complete heart block, which can be fatal. Standard of care relies on electronic devices to artificially restore synchrony. We sought to create a new modality for treating these disorders by engineering electrical conduction tracts designed to propagate electrical impulses. OBJECTIVES: This study sought to create a new approach for treating cardiac conduction disorders by using engineered electrical conduction tracts (EECTs). METHODS: Paramagnetic beads were conjugated with an antibody to gamma-sarco

Regulatory dendritic cell infusion prolongs kidney allograft survival in nonhuman primates.

We examined the influence of regulatory dendritic cells (DCreg), generated from cytokine-mobilized donor blood monocytes in vitamin D3 and IL-10, on renal allograft survival in a clinically relevant rhesus macaque model. DCreg expressed low MHC class II and costimulatory molecules, but comparatively high levels of programmed death ligand-1 (B7-H1), and were resistant to pro-inflammatory cytokine-induced maturation. They were infused intravenously (3.5-10 × 10(6) /kg), together with the B7-CD28 costimulation blocking agent CTLA4Ig, 7 days before renal transplantation. CTLA4Ig was given for up t

Optimization of multivalent bispecific antibodies and immunocytokines with improved in vivo properties.

Multifunctional antibody-based biologics, such as bispecific antibodies and immunocytokines, can be difficult to produce with sufficient yield and stability, and often exhibit inferior pharmacokinetics. Dock-and-Lock (DNL) is a modular method that combines recombinant engineering with site-specific conjugation, allowing the construction of various complex, yet defined, biostructures with multivalency and multispecificity. The technology platform exploits the natural interaction between two interactive human protein binding domains that are modified to provide covalent fusion. We explored the p

CTLA4-IgG ameliorates homocysteine-accelerated atherosclerosis by inhibiting T-cell overactivation in apoE(-/-) mice.

AIMS: Cytotoxic T lymphocyte antigen 4 (CTLA4) exerts inhibitory effects on T-cell activation by competition with CD28. In this study, we investigated the effect of CTLA4-IgG on homocysteine (Hcy)-induced T-cell activation and potential signal pathways involved in atherosclerotic formation. METHODS AND RESULTS: The CD28 signal was significantly amplified by Hcy treatment in splenic T cells and hyperhomocysteinaemia (HHcy)-accelerated plaques in apolipoprotein E-deficient (apoE(-/-)) mice. As a major competitor of CD28, CTLA4-IgG (abatacept) pretreatment, 100 μg/week, in apoE(-/-) mice could r

[Immunosuppressive treatments: mechanisms of action and clinical use].

Renal transplantation is the treatment of choice of end stage renal failure. It both improves the quality and the quantity of life compared to other techniques, such as hemodialysis. These results are partly related to the use of immunosuppressive therapy more effective and whose handling has improved over time. Advances in understanding the mechanisms of lymphocyte activation and the phenomena of rejection have in fact better defined the use of these treatments and their associations. Treatments can be broadly classified according to their characteristics (biological or chemical). Among chemi

Belatacept-versus cyclosporine-based immunosuppression in renal transplant recipients with pre-existing diabetes.

BACKGROUND AND OBJECTIVES: Renal transplant recipients with pre-existing diabetes (PD) have reduced graft survival and increased risk of mortality and ischemic heart disease compared with nondiabetic transplant recipients. To assess the effect of belatacept in this high-risk group, we evaluated outcomes of the subpopulation with PD from previously published BENEFIT and BENEFIT-EXT trials. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: A post hoc analysis evaluated pooled data from BENEFIT (living donors or standard criteria donors) and BENEFIT-EXT (extended criteria donors). Patients were ran

Bench to bedside of CTLA-4: a novel immuno-therapeutic agent for inflammatory disorders.

Co-stimulatory molecules are antigen-independent generators of secondary signals which aid in maintaining the homeostasis of the immune system. CTLA-4 is one among extensively studied co-stimulatory molecules which down-regulate immune response. The attributes of immunosuppressive qualities and capacity to induce tolerance have made its recognition as a potential immuno-therapeutic agent for autoimmune mediated inflammatory disorders. In 2007, European Medical Drug Agency (EMEA) has approved administration of CTLA-4Ig (commercial name: Orencia; generic name: Abatacept), as a mean for co-stimul

Induction of regulatory cells and control of cellular but not vascular rejection by costimulation blockade in hamster-to-rat heart xenotransplantation.

BACKGROUND: In heart allograft in the rat, a sustained costimulation blockade with CTLA4Ig prevents alloreactive T-cell activation and promotes a long-term graft survival through the action of tolerogeneic dendritic cells. It is unclear whether similar mechanisms might occur after xenotransplantation. To test that hypothesis, we have analyzed the action of CTLA4Ig in a model of CD4(+)T cell-mediated xenograft rejection. METHODS: Hamster hearts were transplanted into LEW.1A rats receiving an accommodation-inducing treatment consisting of a short course administration of LF15-0195 and a daily a

Bismuth 213-labeled anti-CD45 radioimmunoconjugate to condition dogs for nonmyeloablative allogeneic marrow grafts.

To lower treatment-related mortality and toxicity of conventional marrow transplantation, a nonmyeloablative regimen using 200 cGy total-body irradiation (TBI) and mycophenolate mofetil (MMF) combined with cyclosporine (CSP) for postgrafting immunosuppression was developed. To circumvent possible toxic effects of external-beam gamma irradiation, strategies for targeted radiation therapy were investigated. We tested whether the short-lived (half-life, 46 minutes) alpha-emitter bismuth 213 ((213)Bi) conjugated to an anti-CD45 monoclonal antibody (mAb) could replace 200 cGy TBI and selectively ta

Evaluation of synthetic schemes to prepare immunogenic conjugates of Vibrio cholerae O139 capsular polysaccharide with chicken serum albumin.

Vibrio cholerae serotype O139 is a new etiologic agent of epidemic cholera. There is no vaccine available against cholera caused by this serotype. V. cholerae O139 is an encapsulated bacterium, and its polysaccharide capsule is an essential virulent factor and likely protective antigen. This study evaluated several synthetic schemes for preparation of conjugates of V. cholerae O139 capsular polysaccharide (CPS) with chicken serum albumin as the carrier protein (CSA) using 1-ethyl-3(3-dimethylaminopropyl)carbodiimide (EDC) or 1-cyano-4-dimethylaminopyridinium tetrafluoroborate (CDAP) as activat

Real-world outcomes of sacituzumab govitecan in patients with pretreated triple-negative breast cancer and brain metastases: data from CEBCC-102 study.

BACKGROUND: Patients with metastatic triple-negative breast cancer (mTNBC) and brain metastases (BM) have poor prognosis and limited treatment options. In the ASCENT trial, sacituzumab govitecan (SG) showed modest progression-free survival (PFS) benefit but no overall survival (OS) advantage in a subgroup with stable BM. To address the limited evidence, we evaluated SGs effectiveness and safety in a real-world cohort. MATERIALS AND METHODS: This retrospective multicenter study included patients with mTNBC and radiologically confirmed BM treated with ≥2 L SG at 13 centers in the Czech Republic,

Bayesian Indirect Comparison and Cost-Effectiveness of Sacituzumab Govitecan Versus Sacituzumab Tirumotecan in Metastatic Triple-Negative Breast Cancer.

OBJECTIVES: Sacituzumab govitecan (SG) and sacituzumab tirumotecan (sac-TMT) are 2 approved antibody-drug conjugates for metastatic triple-negative breast cancer but lack direct comparison. Given their similar indications and high costs, an indirect cost-effectiveness analysis is essential to inform pricing and reimbursement decisions from the perspective of the Chinese healthcare system. METHODS: A partitioned survival model using Bayesian network meta-analysis and flexible parametric modeling estimated long-term survival, quality-adjusted life-years (QALYs), and costs. [Antibody drug conjugates: Use and optimization in HER2-negative metastatic breast cancers (triple negative, RH+/HER2-)].

The therapeutic management of metastatic breast cancers has benefited in the past few years from important progress linked to the development of new molecules, inhibiting poly adenosine diphosphate ribose polymerase (PARP), cyclin-dependent kinases (CDK4/6) or immune checkpoints. More recently, the development of antibody-drug conjugates (ADC) targeting tumor cells, allows for the delivery of cytotoxic agents, at low doses, while increasing their concentration in the tumoral microenvironment. Currently, two ADC are available for the management of HER2-negative metastatic breast cancers, sacitu

ESMO 2025 and new therapies in breast cancer: does society always benefit?

The ESMO 2025 Congress delivered practice-defining data across breast cancer. Key results included the first overall survival benefit for an adjuvant CDK4/6 inhibitor in high-risk HR+/HER2- early breast cancer and T-DXd established as standard in high-risk HER2+ disease. Novel antibody-drug conjugates redefined first-line therapy for metastatic TNBC. Updated guidelines mandate broader NGS profiling for PALB2, AKT1, and PTEN alterations. A strong emphasis emerged on biomarker-driven de-escalation, strategic sequencing, and proactive endocrine resistance management. However, benefits must be bal
