PURPOSE: To evaluate retrospectively the incidence, predictive factors, and management of acute pancreatitis which develops following placement of duodenal stents in patients with malignant gastroduodenal obstruction.
MATERIALS AND METHODS: Among 242 patients with symptomatic malignant gastroduodenal obstruction successfully treated with duodenal stent placement, acute pancreatitis occurred in 10 patients (4.1%) at 1-7 days after stent placement. Univariate and multivariate analyses were performed to evaluate factors predictive of acute pancreatitis. Management of acute pancreatitis was also

The development of non-surgical techniques for the relief of malignant low bileduct obstruction has cast doubt on the best way of relieving jaundice, particularly in patients fit for surgery whose life expectancy is more than a few weeks. We did a randomised prospective controlled trial comparing endoscopic stent insertion and surgical biliary bypass in patients with malignant low bileduct obstruction. 204 patients were randomised (surgery 103, stent 101); 3 subsequently proved to have benign disease and were excluded, leaving 101 surgical and 100 stented patients for assessment. Background: Obstructive jaundice is one of the most common symptoms which can be caused by both malignant and benign hepato-biliary-pancreatic diseases. The differences and the differential diagnostic roles of the serum CA19-9, total bilirubin (TBIL) and especially, the ratio of CA19-9 to TBIL in these patients have not been well elucidated. This study compared the differences and the differential diagnostic roles of the increase-folds of the serum CA19-9, TBIL and the ratio of increase-folds of CA19-9 to increase-folds of TBIL in 508 cases of malignant (MOJ) and benign (BOJ) obstructive jaund

Most cases of hepatic hydatid disease exhibit uncomplicated clinical course and management. However, the diagnosis and management of complicated hepatic hydatid disease is a special issue. One of the most common and serious complications of hepatic hydatid disease is the rupture of the cyst into intrahepatic bile ducts. The clinical appearance of intrabiliary rupture can range from asymptomatic to jaundice, cholecystitis, cholangitis, liver abscess, pancreatitis and septicemia. Current treatments for major ruptures can result in high morbidity and mortality rates. Furthermore, ruptures that ca

UNLABELLED: CARCINOMA OF THE GALLBLADDER: Only patients suffering from advanced disease have symptoms, that is pain, jaundice, weight loss. Mean 5-year survival rate tends to be lower than 5%. Tumors can be resected with negative margins in 15 to 25% of the patients. Obstructive jaundice can be managed with endoscopic biliary stent in 84% of the patients. After radiotherapy tumor regression is observed in 80% of the patients. Their mean survival time is 10 months.
CHOLANGIOCARCINOMAS: More than 90% of the patients present with jaundice. At the time of presentation, 50% of the patients already

In 31 patients with histologically proven hepatocellular jaundice hepatobiliary scintigraphy with Tc-99m labelled IDA derivatives was performed. Results were classified under morphological (demonstration of liver parenchyma, ductus choledochus, bile ducts, gallbladder, kidney) and functional (Tmax, half time of bile excretion, appearance of activity in ductus choledochus, gallbladder, and intestines) criteria. The liver parenchyma could be demonstrated in 90.3% of cases, ductus choledochus in 71%, and the gallbladder in 72.4%. The type of tracer excretion into the intestines showed 9.5% activi

Biliary stents are widely used as an alternative treatment for benign biliary obstruction, choledocholithiasis, biliary fistulas or leak from the cystic duct after cholecystectomy. They occupy a special position in the palliative treatment of malignant biliary strictures. Like any other treatment method, this one also has its complications. Most often it is the development of stent obstruction and jaundice or cholangitis. Less common complications include proximal or distal biliary stent migration. The authors describe a case study of one of the possible complications of migrated biliary stent

Ascariasis is one of the most common helminthic diseases in humans, occurring mostly in countries with low standards of public health and hygiene, thereby making ascariasis highly endemic in developing countries. In endemic areas, 30% of adults and 60-70% of children harbour the adult worm. Biliary ascariasis is a rare cause of obstructive jaundice. Conventional management involves endoscopic extraction of worm. We are reporting a rare case of ascaris which induced extrahepatic biliary obstruction in a young male who presented with acute cholangitis. The ascaris was removed by laparoscopic exp

Gangliocytic paragangliomas are rare tumors located in the gastrointestinal tract that are considered to be benign. They are composed of spindle-shaped cells, epithelioid cells, and ganglion-like cells. They usually present with abdominal pain, and/or gastrointestinal bleeding, and occasionally with obstructive jaundice. We report a case of obstruction in a 17-year-old female, which on histology was found to be a gangliocytic paraganglioma, with an extremely unusual presentation. Intraoperatively, the patient was found to have local tumor extension and regional lymph node invasion, and so she

Echinococcal disease of the liver generally presents as single or multiple hepatic cysts. Rarely, these cysts may rupture into the biliary tree and cause cholangitis, biliary tract obstruction, or biliary colic. These complications are accompanied by high morbidity, and in the case of biliary tract obstruction, mortality rates as high as 50% have been reported. We present a woman with a hepatic echinococcal cyst, biliary colic, and transient jaundice, who proved to have echinococcal involvement of the biliary ducts and gall bladder. Prompt diagnosis of hydatid disease of the biliary tree allow

Using percutaneous-transhepatic cholangioscopy with a guided small calibre, cholangioscope (3.5 mm) in patients with obstructive jaundice it is possible to perform biopsies under visual control or to look for the exact intraductal extension of a tumor with only little stain on the patient. The access may be via a puncture of the right or the left hepatic duct. Since 1987 we have used this method 61 times in 45 patients. Some 32 patients suffered from malignancy of the bile duct. Of 13 patients with benign obstruction we performed a bougienage in 8. The other 5 patients were treated by stone ex

Ultrasound was found to be useful in the evaluation of jaundice. Extrahepatic obstruction can be diagnosed by the presence of dilated intra- or extrahepatic bile ducts. Using a high-resolution real-time mechanical sector scanner, it was possible to show the common hepatic duct as it passes anterior to the right portal vein in 98 out of 100 consecutive adults without biliary tract disease. At this level, the normal duct measures 4 mm or less in internal diameter. All patients with a duct larger than 5 mm thus far have had either biliary tract obstruction or previous cholecystectomy.

Results of a series of 2i liver scans with 131-I rose bengal in 21 patients who developed progressive jaundice in the neonatal period are described. Using the 131-I rose bengal scan alone, an attempt was made to separate the patients into two groups, the first with total bile obstruction (biliary atresia) and the second with only partial biliary tract obstruction (intrinsic liver disease). The diagnostic accuracy of the scan was 82%. An additional diagnostic criterion, the count radio, is suggested. The determination of this ratio requires no additional effort or expense and would improve the

BACKGROUND: Malignant distal biliary obstruction (MDBO) is commonly managed by internal drainage with ERCP-guided self-expandable metal stents (ERCP-SEMS) or surgical hepaticojejunostomy (HJ), yet contemporary SEMS-only, patient-centered comparisons focusing on time to recurrent jaundice are scarce. OBJECTIVE: To compare HJ versus ERCP-SEMS in MDBO, with a primary focus on time to recurrent jaundice. METHODS: We conducted a single-center retrospective cohort of consecutive MDBO patients undergoing palliative biliary decompression (February 2013–August 2021). Groups were defined by the first mo

Rhabdomyosarcoma (RMS) comprises approximately 5% of all pediatric malignancies, and the biliary system is considered one of the rarest RMS locations. Awareness, knowledge, and early recognition of the disease are essential for accurate diagnosis and proper treatment of biliary RMS in a child with obstructive jaundice and suspicious radiological findings. We present two pediatric biliary RMS cases requiring different managements because of their primary evaluations at their referring facilities. A four-and-a-half-year-old boy was referred to our institution for liver transplantation following

Gallbladder duplication is a rare congenital anomaly, often discovered incidentally. Its association with other biliary pathologies, such as choledocholithiasis and Lemmel's syndrome (obstructive jaundice due to periampullary duodenal diverticulum), is uncommon. This case highlights the diagnostic and therapeutic challenges in managing such a complex presentation. A 62-year-old woman, presenting with fever, right upper quadrant pain, and deranged liver function tests, unveiled a diagnostic enigma. Initial ultrasonography suggested the presence of choledocholithiasis, though this finding was la

BACKGROUND AND AIMS: EUS-guided choledochoduodenostomy (EUS-CD) using a lumen-apposing metal stent (LAMS) has recently been reported as an alternative treatment approach for patients with malignant obstructive jaundice and failed ERCP. We analyzed the safety and technical and clinical efficacy of EUS-CD using LAMSs in patients with malignant obstructive jaundice.
METHODS: This was a retrospective study of consecutive patients with inoperable malignant distal bile duct obstruction who underwent EUS-CD using an electrocautery-enhanced (EC)-LAMS over a 3-year period (2015-2018). The main outcome

INTRODUCTION: To determine if neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) were predictive of malignancy in pancreatic cystic neoplasms (PCN) and if these improved the performance of the international consensus guidelines (ICG) in the initial triage of these patients.
METHODS: 318 patients with surgically-treated suspected PCN were retrospectively reviewed. Malignant neoplasms were defined as neoplasms harbouring invasive carcinoma. The optimal cut-off for NLR and PLR were determined by plotting the receiver operating characteristics (ROC) curves of NLR/PLR in p

BACKGROUND AND AIM: Despite the benefits of endoscopic nasobiliary drainage (NBD) in endoscopic retrograde cholangiopancreatography (ERCP), post-ERCP pancreatitis (PEP) and nose/throat discomfort can result. We aimed to determine whether the use of a smaller catheter alleviates these complications.
METHOD: A randomized, controlled trial at a tertiary care center compared 4 Fr and 6 Fr NBD catheters; 165 ERCP patients with naïve papillae were randomly assigned to a catheter-size group.
RESULTS: The prevalence of PEP was significantly lower in the 4 Fr group (3.7%; 3/82) than in the 6 Fr group
