Late Activation of the Vastus Medialis in Determining the Risk of Anterior Cruciate Ligament Injury in Soccer Players.

CONTEXT: Activation time of the quadriceps is important in determining injury risk in professional soccer players. OBJECTIVE: To compare the activation time of the quadriceps and hamstrings muscles during a movement that puts stress on anterior cruciate ligament (ACL) to assess the risk of ACL injury. DESIGN: Case series. SETTING: University laboratory for movement analysis. PATIENTS OR OTHER PARTICIPANTS: Twenty (10 males and 10 females) professional soccer players. INTERVENTION(S): An inertial sensor and 4 electrodes positioned on the quadriceps and hamstrings muscles were used for the

A previous hamstring injury affects kicking mechanics in soccer players.

BACKGROUND: Although the kicking skill is influenced by limb dominance and sex, how a previous hamstring injury affects kicking has not been studied in detail. Thus, the objective of this study was to evaluate the effect of sex and limb dominance on kicking in limbs with and without a previous hamstring injury. METHODS: Forty-five professional players (males: N.=19, previously injured players=4, age=21.16±2.00 years; females: N.=19, previously injured players =10, age= 22.15±4.50 years) performed 5 kicks each with their preferred and non-preferred limb at a target 7m away, which were recorded

The effects of whole body vibration on pulse wave velocity in men with chronic spinal cord injury.

OBJECTIVE: A pilot study to evaluate the therapeutic potential of 40 weeks of passive standing with whole body vibration (PS-WBV) on central and peripheral arterial stiffness among men with chronic spinal cord injury (SCI). METHODS: Consenting participants were pre-screened to ensure safe participation. Fifteen individuals with chronic SCI were enrolled to participate in PS-WBV sessions three times per week for 40 weeks on a modified WAVE platform custom-fitted with an EASYStand 5000. Knee angle was set at 160°, and vibration parameters were 45Hz frequency and 0.7mm displacement. Each 45-minu

Prevalence of saphenous nerve injury after adductor-canal-blockade in patients receiving total knee arthroplasty.

BACKGROUND: Adductor-canal-blockade is a new technique for pain relief after knee surgery. This block could cause nerve injury and the aim of this follow-up study was to determine the prevalence of saphenous nerve injury in patients receiving adductor-canal-blockade for pain treatment after total knee arthroplasty. METHODS: All patients included in two former studies of adductor-canal-blockade following total knee arthroplasty were invited to participate in this follow-up study 3-6 months after surgery. We examined the cutaneous area on the medial aspect of the lower leg (medial crural branch

Postural stability deficits during the transition from double-leg stance to single-leg stance in anterior cruciate ligament reconstructed subjects.

The goal of this study was to evaluate postural stability during the transition from double-leg stance (DLS) to single-leg stance (SLS) in anterior cruciate ligament reconstructed (ACLR) (n=20) and non-injured control subjects (n=20). All ACLR subjects had fully returned to their pre-injury sport participation. Both groups were similar for age, gender, height, weight, body mass index and activity level. Spatiotemporal center of pressure outcomes of both legs of each subject were measured during the transition from DLS to SLS in eyes open and eyes closed conditions. Movement speed was standardi

Surgery in the Standing Position by a Surgeon with Achilles Tendon Rupture.

Unexpected injuries can have a profound effect on a surgeon's performance and thus on patients and surgical departments. Here we describe a technique for performing surgery in the standing position, as done by a surgeon with an Achilles tendon rupture. During his prescribed 45-day non-weight-bearing period for the left ankle after surgery for an Achilles tendon rupture, the surgeon was able to participate in 15 surgeries as an operator or assistant, due to his use of a combination of injured-leg genuflection on a stool and a 'Surgical Body Support' device. Similarly injured surgeons may benefi

Modern military surgery: lessons from Iraq and Afghanistan.

The types of explosive devices used in warfare and the pattern of war wounds have changed in recent years. There has, for instance, been a considerable increase in high amputation of the lower limb and unsalvageable leg injuries combined with pelvic trauma. The conflicts in Iraq and Afghanistan prompted the Department of Military Surgery and Trauma in the United Kingdom to establish working groups to promote the development of best practice and act as a focus for research. In this review, we present lessons learnt in the initial care of military personnel sustaining major orthopaedic trauma in…

[Bilateral luxatio erecta of the shoulder joint--a rare injury. Management and therapy in polytrauma patients].

The case of a 36-year-old male patient is reported who fell from 20 m, sustaining injuries to the abdomen and pelvis and fractures of both arms and the left leg as well as erect dislocation of both shoulders (luxatio erecta humeri). The injury on the right was subcoracoid, that on the left subglenoid-a compound dislocation of the humerus head through the axilla. Based on this case and the pertinent literature, the pathophysiology, diagnosis and treatment of this rare injury are discussed. After immediate closed reduction, soft tissue damage, fractures or neurovascular lesions should be operate

Sticking the landing: leg mechanosensory feedback coordinates the transition from flight to landing.

Landing is an especially demanding stage of flight. Flying animals must rapidly transform multimodal sensory information into the coordinated movement of multiple limbs in order to transition from flight to a stable standing posture, often on surfaces that vary in stability, texture, and orientation. Failure to do so can be costly, leading to injury or death. Here, we examine the role of mechanosensory feedback in guiding landing in the fruit fly, Drosophila melanogaster. We describe a novel behavioral assay in which we deliver localized mechanical contact to individual legs of tethered, flyin

Association Between the COL5A1 rs12722 Genotype and the Prevalence of Anterior Cruciate Ligament Rupture in Professional Football Players.

BACKGROUND: Previous studies have tested the association between the COL5A1 rs12722 polymorphism and the risk of anterior cruciate ligament (ACL) injury. Overall, their results are contradictory because most studies used relatively small samples and data from ACL ruptures during sport activities have been mixed with ruptures suffered in non-sporting contexts. OBJECTIVE: To examine the association between the COL5A1 rs12722 polymorphism and the prevalence of ACL rupture in a homogeneous sample of professional male football players. METHODS: A total of 268 professional male football players pa

Avulsion Fracture of the Tibial Tubercle Associated With Patellar Tendon Avulsion.

Avulsion fractures of the tibial tubercle in adolescent athletes are uncommon injuries, believed to be a result of forceful extension of the knee against a fixed leg. Concomitant injury to the ipsilateral patella tendon is even more rare, with few cases reported in the literature. The mechanisms responsible for this association are not well understood. The significance of this double insult to the knee extensor mechanism is the potential deleterious effect of misdiagnosis and/or mismanagement. In this case report, the pathophysiology, mechanism of injury, classification, diagnosis, and managem

Massive de-gloving thigh injury treated by vacuum therapy, dermal regeneration matrix and lipografting.

Frequently lower limb injuries are caused by road and work accidents. The young age of those affected coupled with the anatomical and functional peculiarities of this part of the body with regards to social life during adolescence make the treatment of the leg wound complex and challenging. We present two cases of young girls, victims of serious road accidents who were treated initially with frequent wound washings, vacuum therapy to stimulate granulation tissue, then dermal regeneration matrix (INTEGRA®) and split-thickness skin grafts. After one year, both patients treated with lipofilling h

[Emergency problems in vascular surgery].

The most typical emergency patient of vascular surgery is suffering from urgent lower limb ischemia, potentially threatening the viability of the limb. Life-threatening conditions requiring immediate treatment include rupture of the abdominal aorta as well as septicemia associated with necrosis of a neuropathic-ischemic leg of the diabetic patient. Stenosis of the carotid artery requires an emergency operation, if in spite of antithrombotic medication the patient exhibits persistent transient symptoms of paresis or attacks of blindness. Vascular injury should be suspected and a vascular surgeo

[Maternal paralysis of obstetrical origin. Two case reports].

We report two cases of maternal obstetrical paralysis by injuries to the sacral plexus (lumbosacral trunk). This nervous lesion is rare and occurs more often in young small primigravidae, carrying a large fetus, during a prolonged labor and a delivery requiring midforceps. The symptoms appear usually a few hours after delivery: paresthesias of the leg and the foot as well as weakness and possible footdrop (the paralysis may be mild or severe). The different mechanisms involved are inspected. The prognosis of this lesion is good, the patients recover usually within a period of three months. The

[Malgaigne pelvic ring injury in childhood].

Pelvic girdle injuries leading to anterior and posterior disruption are rarely seen in children. External fixation is useful to reduce and stabilize the pelvis in order to control massive blood loss due to severe venous bleeding. The anterior tension band element of the external fixator can successfully maintain reduction of the pelvic girdle. Dislocation or fracture dislocation of the sacroiliacal joint cannot be reduced with simple traction techniques. Leg length discrepancy is to be expected when the hemipelvis is displaced. Seven cases of this rare injury in children are presented together

[Variations of venous lesions of lower extremities associated with varicose veins].

374 varicose extremities in 238 patients were examined. In 344 (92%) out of 374 extremities valvular incompetence and dilatation of the basal trunk of the great saphenous vein over its length were found. In 212 (56.7%) extremities dilatation of the first median anastomosis between the great and small saphenous veins was found. The upper medial inflow in the leg gets injuried in 36.1% of cases, and the superficial tibial vein in 28%. Lateral varicosity occurred in 0.5% of cases. Injuries to the lateral branches without dilatation of basal trunk constituted 5.6%. There were no cases of dilatatio

Phantom limb experiences in congenital limb-deficient adults.

This case report describes phantom limbs in four adults with congenital limb deficiency. Case 1, with congenital absence of the left arm below the elbow, received a minor injury of the stump at age 16 and subsequently developed a full-length phantom arm, hand and fingers. Cases 2 and 3, at the ages of 26 and 30, underwent minor surgery of the lower limbs and subsequently developed vivid, full-length phantoms of their legs. Case 4, aged 31, was born with a shortened and deformed right leg, and underwent amputation of the foot and deformed part of the leg at age 3. Beginning at age 6, she develo

Golf and racquet sports injuries.

There are specific injuries that are common in golf and racquet sports. These abnormalities have a predilection for specific structures as well and can be divided into two categories on the basis of etiology as either chronic repetitive injury or acute trauma. With golf injuries, upper extremity abnormalities prevail and include rotator cuff disease, epicondylitis, wrist tenosynovitis, and hamate hook fracture. Thoracolumbar spine pain can also occur. The order of frequency of these ailments is different for professional and recreational athletes. With racquet injuries, as in tennis, lower ext
