Gastric retention of enteric-coated magnesium chloride tablets.

OBJECTIVE: To describe a patient with gastric retention of enteric-coated magnesium chloride tablets. Potential drug and disease etiologies accounting for failure to empty this dosage form are discussed. DESIGN: Single case report. CASE SUMMARY: A seriously ill patient with metastatic small-cell lung cancer accumulated 21 enteric-coated magnesium chloride tablets in his stomach during a four-day administration period. The patient had gastroscopic evidence of mild pylorospasm and suspected gastric motor dysfunction. The latter may have been the result of several factors including concurrent u…

High-dose magnesium sulfate attenuates pulmonary oxygen toxicity.

BACKGROUND AND METHODS: Rats rapidly develop respiratory distress when exposed to 100% oxygen and die within a few days. Autopsy of the lung shows severe histologic damage characteristic of the adult respiratory distress syndrome. The purpose of this study was to evaluate the effects of magnesium sulfate loading in a rat model of acute oxygen toxicity. Thirty-four rats were divided into three groups. Group 1 (n = 18) served as a control (no magnesium therapy), while group 2 (n = 8) and group 3 (n = 8) received varying amounts of magnesium sulfate. All animals were exposed to 100% oxygen for 96…

Eosinophilic hepatitis after ingestion of choline magnesium trisalicylate.

Choline magnesium trisalicylate is a non-acetylated salicylate used widely as a nonsteroidal anti-inflammatory drug. Although mild transient hepatotoxicity associated with aspirin and other salicylates has been well documented, most commonly with high-dose treatment for rheumatologic disorders 112), we report a case of severe hypersensitivity hepatitis with striking tissue and peripheral eosinophilia after ingestion of choline magnesium trisalicylate.

Cardioskeletal mitochondrial myopathy associated with chronic magnesium deficiency.

A 3-year-old boy presenting with convulsions and carpopedal spasm had hypomagnesemia and hypermagnesuria due to congenital magnesium-losing nephropathy. Despite chronic oral and intermittent intravenous magnesium supplementation, he remained chronically hypomagnesemic. At age 4, he developed a progressive proximal myopathy and dilated hypertrophic cardiomyopathy that ultimately contributed to his death at age 14 years. Skeletal and cardiac muscle specimens showed a mitochondrial myopathy with increased numbers of enlarged, structurally abnormal mitochondria. Muscle magnesium content was marked…

Intracellular free magnesium concentration: relevance to cardiovascular medicine.

There is a growing awareness of the role hypomagnesemia plays in cardiovascular medicine. Recent experimental studies have also provided a new understanding og how Mg2+ ions influence various ion channels and transport mechanisms. Yet, the pathophysiological mechanisms that may be responsible for various phenomena associated with hypomagnesemia, have not been clarified. This is partly due to that there, until recently, has been a lack of convenient and reliable methods for measuring cytosolic free Mg2+ concentration. It is the hope that newly developed techniques for measuring the cytosolic fr

Plasma corticosterone in stressed magnesium-deficient weanling rats.

A study was undertaken to learn whether the young Mg-deficient mammal can respond to major stress with increased levels of plasma corticosterone. Plasma corticosterone was determined in 48 weanling rats with dietary Mg deficiency and in 48 Mg-sufficient controls fed a Mg-supplemented diet, studying 12 animals at a time on experimental day 14. Each animal was studied once, either in an unstressed state or after the stress of audiogenic or strychnine seizures. Plasma corticosterone was determined using a radioimmunoassay; Mg was analyzed by atomic absorption spectrophotometry. On experimental da

Low milk fat syndrome and magnesium oxide supplementation.

The low milk fat syndrome most commonly occurs in dairy cows fed diets with a high ratio of carbohydrate to roughage. It is characterized by markedly depressed yields of milk fat. Rumen buffers, such as sodium bicarbonate (NaHCO3) and magnesium oxide (MgO), are known to correct the milk fat depression observed on restricted roughage rations, although by apparently different mechanisms. The mode of action of NaHCO3 can be explained by control of the proportion of ruminal propionate via control of rumen pH. Cows fed diets supplement with NaHCO3 showed increased feed intake and improved digestibi…

Myasthenia gravis presenting as weakness after magnesium administration.

We studied a patient with no prior history of neuromuscular disease who became virtually quadriplegic after parenteral magnesium administration for preeclampsia. The serum magnesium concentration was 3.0 mEq/L, which is usually well tolerated. The magnesium was stopped and she recovered over a few days. While she was weak, 2-Hz repetitive stimulation revealed a decrement without significant facilitation at rapid rates or after exercise, suggesting postsynaptic neuromuscular blockade. After her strength returned, repetitive stimulation was normal, but single fiber EMG revealed increased jitter …

Magnesium-induced hypocalcemia in salicylate-treated pregnant rats.

Pregnant and non pregnant rats were fed diets with different Mg content and treated with 5 oral doses of 300 mg/kg salicylic acid for 5 consecutive days or from day 16 to 20 of gestation. One day after the final dosage, serum Mg and Ca concentrations were measured by atomic absorption spectrophotometry. In maternal rats, salicylate caused a strong decrease of serum Ca with increasing serum Mg concentrations (r = -0.8706). In fetal and nonpregnant adult female rats, serum Ca was not significantly decreased by salicylate with increasing serum Mg concentrations. However, the hypocalcemic effect o

Importance of magnesium chloride repletion after myocardial infarction

Data pertinent to the role of magnesium deficits in coronary artery disease and myocardial infarction are reviewed. Results of clinical, laboratory and epidemiologic studies indicate an association between magnesium deficiency and a poor prognostic outcome in patients who have had myocardial infarction. It therefore appears to be a reasonable prophylactic measure to monitor closely magnesium status in patients with coronary heart disease and other patients at risk of acute myocardial infarction, and to supplement with magnesium chloride when clinically necessary. In addition, recent studies pr…

Diuretic-induced CNS magnesium alteration and digoxin intoxication.

Male Wistar rats were injected intraperitoneally for 3 consecutive days with hydrochlorothiazide (HCTZ; 15 mg/kg), chlorothiazide (CTZ; 100 mg/kg), bendroflumethiazide (BFTZ; 1.5 mg/kg), chlorthalidone (CHLOR; 15 mg/kg), methyclothiazide (METH; 1.0 mg/kg) or metolazone (MET; 1.5 mg/kg). Magnesium content was measured in the hypothalamus, medulla oblongata, cerebral cortex, heart, skeletal muscle and serum. Although there was no consistent alteration of Mg in the serum, skeletal muscle and heart, there was a significant effect on hypothalamic and medullary Mg. Compared to a control value of 17.…

Decreased amniotic fluid magnesium concentration in diabetic pregnancy.

Amniotic fluid samples from 21 diabetic pregnant women were pair-matched for gestational age with 21 samples obtained from nondiabetic women, and analyzed for magnesium concentration. Mean +/- standard deviation amniotic fluid magnesium concentration (mg/dL) was 0.86 +/- 0.21 in the diabetic group and 1.06 +/- 0.22 in the control group (P less than .001). It is concluded that in the diabetic pregnancy, a state of fetal magnesium deficiency exists. This deficient state may contribute to neonatal hypocalcemia in infants of diabetic mothers.

cis-Diamminedichloroplatinum-induced hypomagnesemia and renal magnesium wasting.

Hypomagnesemia is a well-recognised complication of cis-diamminedichloroplatinum (DDP) treatment. We prospectively evaluated 50 patients with advanced malignant disease receiving DDP for the development of hypomagnesemia. Urinary magnesium excretion was measured in 24 patients. The mean serum magnesium fell from 0.79 mmol/l (normal 0.7-1.1 mmol/l) prior to therapy to 0.55 mmol/l 3 months after commencing DDP. All 50 patients had become hypomagnesemic by this time and 10% were symptomatic, requiring oral magnesium supplementation. At 6 weeks after commencing DDP only four patients had restricte…

Urinary magnesium loss in aging diabetic mellitus rats.

Past studies on urinary loss of magnesium (Mg) have focused on young diabetic rats. The aims of the present study were to determine the rapidity at which glycosuria and magnesuria occur after the induction of diabetes mellitus (DM) in old male rats, and the maximal amount of Mg and glucose (Glu) loss in the urine and whether or not the loss is persistent and (3) the most sensitive means of correlating the Mg and Glu loss in the urine. Three methods of expressing urinary Mg and Glu concentrations were selected: Method A: mg/24 h; Method B: mg/24 h/kg body weight (BW), and Method C: ratio of Mg …

Magnesium deficiency after ileal resections for Crohn's disease.

The magnesium status of the body was studied in 87 patients with various lengths of small-bowel resections for Crohn's disease. The urinary magnesium excretion decreased with increasing resection length, and so did the concentration of magnesium in muscle. Muscular fatigue, an early symptom of magnesium deficiency, was positively correlated to a pathologically low concentration of muscle magnesium. It was concluded that clinically important magnesium deficiency, which was not detected by determination of serum magnesium, occurred in patients with ileal resections exceeding 75 cm.

Magnesium deficiency following jejunoileal bypass operations for obesity.

Skeletal muscle biopsies, blood samples, and 24-hour urines, before and after magnesium infusions, were obtained from 12 patients who had undergone jejuno-ileal bypass surgery several years earlier, selected for probable magnesium deficiency on the basis of repeated hypomagnesemia. The patients retained significant amounts of the infused magnesium, and exhibited elevation of low skeletal muscle magnesium and potassium, with concomitant decreases of muscle sodium and chloride. These changes were accompanied by increased urinary calcium and sodium and decreased urinary phosphorus excretion.

Choline magnesium trisalicylate versus ibuprofen in rheumatoid arthritis.

A double-blind study compared choline magnesium trisalicylate (Trilisate tablets, Purdue Frederick) (CMT) and ibuprofen (IPF) in the treatment of rheumatoid arthritis. The seven-week trial in 134 ambulatory patients, of whom 68 received CMT and 66 received IPF, was conducted as a multicentre study. Both CMT and IPF were highly effective in reducing significantly all symptoms that had worsened following the discontinuance of previous therapy. The anti-inflammatory effect of CMT produced a significantly (P less than 0.05) greater reduction in the number of swollen joints, compared to IPF, in pat…
