[Gardner syndrome. A report of 3 cases in members of the same family].

The aim of the present report is to describe the overall mandibular and extramandibular lesions correlated to Gardner's syndrome which were found in the same family (mother and two daughters). In two cases (mother and one daughter) mandibular lesions consisted of osteoma. In the latter, these were associated with composite odontoma. In the other daughter the lesions consisted of desmoid fibroma. The two daughters revealed single colonic polyposis whereas the mother was affected by multiple colonic polyposis and subcutaneous lipoma. Such a constellation of lesions may be attributed to Gardner's

Gorlin's syndrome with a cardiac lesion and jaw cysts with some unusual histological features. A case report and review of the literature.

A 25-year-old male with Gorlin's syndrome associated with an intracardiac lesion, presented with jaw cysts requiring surgical treatment. One of the cysts contained unusual histological components including fibrous tissue, myxomatous stroma and abnormal epithelial hyperplasia. Metabolic investigation showed normal parathyroid function and normal tissue responsiveness to parathyroid hormone, despite several clinical and radiological features suggestive of pseudo-hypoparathyroidism. The principal features of the syndrome are reviewed.

Comparison of the ultrastructure of a malignant tumor of the mandible containing giant cells with Paget's disease of bone.

Distinction between lesions of the jaws containing giant cells can be difficult. This study reports the results of the study of the ultrastructure of 30 patients with Paget's disease in comparison with a patient diagnosed as having malignant giant cell tumor of the mandible. The ultrastructure of the malignant giant cell tumor of the mandible was found to contain nuclear inclusions morphologically identical with the 12-15 nm tubules characteristic of the nuclei of osteoclasts in Paget's disease of bone. No clinical or laboratory signs of Paget's disease was found in this patient. A search of t

Adenomatoid odontogenic tumour (adenoameloblastoma).

Two cases of adenomatoid odontogenic tumour affecting the jaws of two young Nigerian Africans are described. One was diagnosed clinically as a cyst, the other as an ossifying fibroma, but their true nature was later unmasked by histopathological studies. Although considered rare, the adenomatoid odontogenic tumour should be borne in mind in the differential diagnosis of radiolucent lesions of the jaws since an incorrect diagnosis may lead to mutilating surgery whereas the lesion is amenable to a conservative surgical approach. It is also important to examine histologically all excised specimen

Diffuse large B-cell lymphoma with oral manifestations.

Non-Hodgkin lymphomas (NHLs) are a heterogenic group of lymphoproliferative diseases with their different behavior pattern and response to treatments. As a Hodgkin disease, NHL occurs from lymphoid tissues and can spread to other organs. Prognosis depends on histologic type, grade, and treatment. In the World Health Organization classification, aggressive B-cell lymphomas are diffuse large B-cell lymphoma, Burkitt lymphoma/leukemia, and precursor lymphoblastic leukemia/lymphoma. They may present either nodal or extranodal disease, which may be either localized or disseminated. Diffuse large B-

Ameloblastic fibro-odontosarcoma: a case report.

This paper reports one case, of an ameloblastic fibro-odontosarcoma (AFOS) affecting the mandible, in a 12-year-old girl. This neoplasm is a rare odontogenic neoplasm. To the authors' knowledge this is the fifteenth case of AFOS reported in English. The patient's chief complaint was a swelling in the face for 6 months. An incisional biopsy was performed diagnosing the case as an ameloblastic fibroma. After radiography ameloblastic fibro-odontoma was diagnosed. Computed tomography was performed and a stereolithography model made to plan the surgical procedures. A hemimandibulectomy followed by

Ewing sarcoma of the mandibular condyle: multidisciplinary management optimizes outcome.

BACKGROUND: Ewing sarcoma (ES) is a rare, primary malignancy of bone that occurs in childhood and early adolescence. Improved methods of diagnosis and treatment have dramatically increased survival over the last 20 years. Treatment mainstays are chemotherapy and surgical tumor resection. ES usually occurs in long bones of the axial skeleton; however, it may rarely arise in facial structures, particularly the mandible. In these cases, resection presents a challenging postsurgical reconstruction. METHODS AND RESULTS: We present the clinical findings and management of a case of ES that developed

Atypical hypercementosis versus cementoblastoma.

Hypercementosis is a non-neoplastic condition in which excessive cementum is deposited in continuation with the normal radicular cementum. Apart from the idiopathic nature of hypercementosis, this condition is associated with several local and systemic factors. Cementoblastoma is an odontogenic tumour characterized by the proliferation of functional cementoblasts that form a large mass of cementum or cementum-like tissue on the tooth root. Although hypercementosis and cementoblastoma are typical conditions with distinct clinical evolution, atypical cases may challenge their diagnosis. Because

Sialolithiasis or complex odontoma?

We present a case of a 53-year old woman admitted for treatment in the clinic after detection of radiopacity in the left half of the mandible. The patient was operated on with a tentative diagnosis of complex odontoma. No pathological lesion was found intraoperatively after trepanation of the mandibular bone. Additional clinical examination and contrast media sialography of the left submandibular salivary gland in PA and lateral oblique view were instrumental in making the correct diagnosis and administering the proper treatment. This case demonstrates that difficulties may arise in differenti

Eruption disturbances of mandibular permanent canines in Japanese children.

OBJECTIVES: The purpose of this study was to clarify the cause and prognosis of eruption disturbance of mandibular permanent canines. SAMPLE AND METHODS: Fifteen Japanese children who visited Niigata University Dental Hospital between 1979 and 1997 with unerupted and/or impacted mandibular permanent canines were examined. RESULTS: All cases showed unilateral disturbances. The eruption disturbance of 6 mandibular canines was associated with a calcified obstacle such as an odontoma (5 cases) or a supernumerary tooth (1 case). After removal of the obstacle, 5 canines were aligned within the den

Oromandibular reconstruction with the radial-forearm osteocutaneous flap: experience with 60 consecutive cases.

One of the more difficult problems in reconstructive surgery of the head and neck is replacement of bone and soft tissue lost because of injury, osteomyelitis, or malignancy. The radial-forearm osteocutaneous flap is an accepted choice for oromandibular reconstruction. This study was undertaken to review one center's experience with 60 consecutive cases of oromandibular reconstruction with the radial-forearm osteocutaneous flap. Records of the 38 men and 22 women (mean age, 60 years; range, 26 to 86 years) were reviewed for tumor location, defect and bone length, flap failure rate, recipient-

Mandibular reconstruction with free vascularized fibular flap.

Although various sources of vascularized bone/composite tissue have been used for mandibular reconstruction, the vascularized fibular flap has been reported to have many advantages over the others and to be the most suitable for bridging a long-span mandibular defect. In this paper, positive experience of 14 consecutive cases in which a free vascularized fibular graft with or without a skin paddle has been used to reconstruct a long mandibular defect is reported. Half of the cases were primarily reconstructed, the rest secondarily. The average length of the fibula graft harvested was 16.7 cm,

Transmigration of permanent mandibular canines. Case report.

Transmigration is the migration of a tooth across the midline of the jaw. This phenomenon is found only in relation to the permanent mandibular canines. Two cases are reported. In both, the cause of this deviation was an odontome found in the site normally occupied by the mandibular canine. The literature on transmigration is reviewed.

Primary hyperparathyroidism: evaluation of different treatments of jaw lesions based on case reports.

On the basis of three case reports, different treatment modalities of primary hyperparathyroidism of the jaws are presented. Surgical intervention made as the result of misdiagnosis in the first case caused an unnecessary bone defect and delayed bone regeneration for several months. Two other cases showed spontaneous regeneration of bone after parathyroidectomy. The second case disproved the earlier opinion that regeneration of the bone lesions could last for several years, and that the normal morphology could be restored. Complete resolution of the central giant-cell lesion was found 6 months

B-cell lymphoma in two monogamous homosexual men.

Opportunistic infections and malignant neoplasms have been described in homosexual men in association with immunologic abnormalities. We observed the development of malignant B-cell lymphomas in two homosexual men who had had a monogamous relationship for two years. Patient 1 had an aggressive, monoclonal, small, noncleaved, non-Burkitt's lymphoma ("undifferentiated lymphoma"), associated with severe immunocompromise. Patient 2 manifested a monoclonal, small, cleaved, follicular center cell lymphoma, with a follicular pattern, two months later. No common acute infection was detected. Staining

A retrospective evaluation of 23 reported cases of solitary plasmacytoma of the mandible, with an additional case report.

Twenty-four cases of solitary plasmacytoma of the mandible are reported in the literature. The criteria for the selection of cases and for their diagnosis are described. It is a rare condition, with a maximum occurrence in the fifth to seventh decades and a male to female ratio of 2:1. It predominates in the premolar and molar regions. Clinical and radiological features are non-specific. Diagnosis is by biopsy. Treatment of preference is surgery. Irradiation of greater than 5,000 Rads is also recommended. Investigations must be carried out for multiple myeloma. Long-term regular surveillance i
