Contactless surface registration of featureless anatomy using structured light camera: application to fibula navigation in mandible reconstruction.

PURPOSE: Mandibular reconstruction using fibula free flap is a challenging surgical procedure. To assist osteotomies, computer-assisted surgery (CAS) can be used. Nevertheless, precise registration is required and often necessitates anchored markers that disturb the patient and clinical flow. This work proposes a new contactless surface-based method adapted to featureless anatomies such as fibula to achieve a fast, precise, and reproducible registration. METHODS: Preoperatively, a CT-scan of the patient is realized and osteotomies are virtually planned. During surgery, a structured light came…

Superficial peroneal nerve accessory artery (SPNAA) flap for head and neck reconstruction: A cadaveric anatomical study and retrospective case series review.

BACKGROUND: Several different flaps can reconstruct intraoral defects or lower limb deficits after free fibula osteo-cutaneous flap harvesting for jaw reconstructions. However, commonly used options may not be available for various reasons and can be associated with significant morbidity. We hypothesized that flaps supplied by the superficial peroneal nerve accessory artery (SPNAA) could be a viable alternative reconstructive option. METHODS: We describe the SPNAA's anatomy using 20 human cadaveric leg dissections and report eight cases involving SPNAA-based perforator flap reconstructions (s

[Reconstruction of the atrophic edentulous maxilla for implant placement].

INTRODUCTION: Extreme maxillary atrophy results from partial or total maxillary bone defect, affecting the alveolar and basal segments. The residual bone is only a few millimeters high and does not allow retention of dental prostheses (complete edentulation), or placing implants. Bone reconstruction with cranial bone grafts usually allows obtaining enough bone volume in a single step to place implants for prosthetic rehabilitation, 3 months later. TECHNICAL NOTE: Reconstruction begins by inserting bone grafts on the sinus floor. The pre-maxilla is then rebuilt with bone grafts placed on the n

A patient with severe maxillary gingival exposure treated with combined compression osteogenesis of the anterior alveolar bone and conventional Le Fort I osteotomy.

Excessive gingival exposure at the maxillary anterior region during not only smiling (a gummy face) but also at rest creates both functional and aesthetic problems for patients. We herein introduce a unique treatment procedure for mandibular retrognathia with a gummy face. This procedure combines conventional Le Fort I osteotomy and following corticotomy at the anterior region of the maxilla. Subsequently, the anterior segment is continuously compressed (compression osteogenesis) in a posterior-superior direction until it reaches an ideal position. This procedure appears to safely and adequate

The Use of Microplates for Internal Fixation of Comminuted Mandibular Fractures.

BACKGROUND: Although clinical trials have successfully applied microplates for the internal fixation of single or double isolated mandibular fractures, the use of microplate systems in comminuted mandibular fractures is not widely accepted. This study aimed to evaluate the use of microplates for internal fixation of comminuted mandible fractures and to discuss their applicability. METHODS: Fourteen patients with comminuted mandibular fractures (10 at a single region, 3 at 2 regions, and 1 at 4 regions) were treated with open reduction and internal fixation using 0.5- or 0.6-mm-thick titanium

Three-dimensional topography of rat trigeminal ganglion neurons using a combination of retrograde labeling and tissue-clearing techniques.

The trigeminal nerve is the sensory afferent of the orofacial regions and divided into three major branches. Cell bodies of the trigeminal nerve lie in the trigeminal ganglion and are surrounded by satellite cells. There is a close interaction between ganglion cells via satellite cells, but the function is not fully understood. In the present study, we clarified the ganglion cells' three-dimensional (3D) localization, which is essential to understand the functions of cell-cell interactions in the trigeminal ganglion. Fast blue was injected into 12 sites of the rat orofacial regions, and gangli

Mandibular asymmetry: a three-dimensional quantification of bilateral condyles.

INTRODUCTION: The shape and volume of the condyle is considered to play an important role in the pathogenesis of the mandibular deviation. Curvature analysis is informative for objectively assess whether the shape of the condyles matches that of the glenoid fossa. In this study, a three-dimensional (3-D) quantification of bilateral asymmetrical condyles was firstly conducted to identify the specific role of 3-D condylar configuration for mandibular asymmetry. METHODS: 55 adult patients, 26 males (26 ± 5 yrs) and 29 females (26 ± 5 yrs), diagnosed with mandibular asymmetry were included. The e

Digital workflow for treating comminuted anterior mandibular fracture - A technical note.

Comminuted mandibular fracture poses a surgical challenge. Extensive comminution and absence of reference adds complexity particularly in anterior region. In this technical note, we describe a novel application of virtual surgical planning combined with 3D printing which grants precise anatomic reduction in such cases. Computed Tomography data (mandible) of another patient of same age and sex was utilized to create reference model by virtual surgical planning. Virtually created model was 3D printed and utilized for precontouring the reconstruction plate which assisted in achieving satisfactory

Distraction lengthening of the mandibular costochondral graft: a precautionary note.

Distraction osteogenesis of bone-grafted mandibles has been shown to occur without undue difficulties in bone regeneration and healing. In the patient with hemifacial microsomia for which early costochondral graft reconstruction has been done, secondary distraction lengthening may still be needed eventually. It is important to assess preoperatively that the superior portion of the graft is properly positioned against the residual glenoid fossa to prevent excessive superior movement of the proximal mandibular segment during distraction. The case presented clearly illustrates this unique distrac

Reconstruction of Posterior Mandibulectomy Defects in the Modern Era of Virtual Planning and Three-Dimensional Modeling.

BACKGROUND: Posterior mandibulectomy defects can be reconstructed using either soft tissue or vascularized bone. The authors hypothesize that advances in computer-aided design and manufacturing (CAD-CAM) have resulted in osteocutaneous free flaps now proving superior to soft-tissue flaps. METHODS: The authors conducted a retrospective review of all free flap reconstructions of posterior mandibulectomy defects where the condyle was resected from 2005 to 2016. RESULTS: Overall, 291 patients (mean age, 56.9 years; mean body mass index, 26.2 kg/m) underwent posterior mandible reconstruction with

Clinical Application of Individualized 3D-Printed Templates in the Treatment of Condylar Osteochondroma.

BACKGROUND: Osteochondroma (OC) is one of the most common benign tumors of the long bones, but it rarely occurs in the maxillofacial skeleton. However, mandibular condylar OC often leads to severe facial deformity in affected patients, including facial asymmetry, deviation of the chin, and malocclusion. This study aimed to explore the clinical application of individualized 3D-printed templates to accurately and effectively treat condylar OC. METHODS: A total of 8 patients with mandibular condylar OC were treated from July 2015 to August 2021. The enrolled patients (5 women and 3 men) had a me

Preclinical evaluation of patient-specific 3D-printed PDLLA/SrHAp implants in a minipig segmental mandibular defect model.

Large segmental mandibular defects remain difficult to reconstruct, and patient-specific biodegradable scaffolds produced by additive manufacturing may offer an alternative to permanent alloplastic implants. This exploratory study evaluated three 3D-printed poly(DL-lactide) (PDLLA)-based implant configurations in a periosteum-preserving minipig mandibular defect model: PDLLA, PDLLA containing 10 wt% strontium-modified hydroxyapatite microparticles (PDLLA + SrHAp-P), and a biphasic PDLLA-shell/SrHAp-core implant (PDLLA/SrHAp-C). Twelve adult minipigs received patient-specific implants and titan
