[Study of the placebo effect in using the non-invasive ambulatory measurement of blood pressure].

The aim of this study is to evaluate the antihypertensive effect of placebo assessed by 24 hr non invasive blood pressure monitoring. 20 patients (16 males, 4 females, 55 +/- 10 years old) with primary hypertension (WHO stage I or II) were included with a diastolic blood pressure greater than or equal to 100 mmHg (mean blood pressure from three clinical readings). Casual blood pressure and blood pressure monitoring (Spacelabs - 4 measurements per hour during a 24 hr period) were established before and at the end of the placebo run in period (one placebo tablet given once daily at 8 h-8 h 30 a.

[Dopplerography of the venous return of the foot in the healthy subject. Preliminary to a study of ambulation].

The direction of venous return was monitored continuously by the Doppler method at 4 MHz, in order to study the effects of posture and certain kinesitherapeutic maneuvers on venous return in the foot of young, healthy and trained adults (kinesitherapy monitors, yoga assistants). It was noted that: the acceleration of venous return in the foot was expiratory in the recumbent position and inspiratory in the standing position; dorsiflexion (leg on foot or toes on heel) caused a more pronounced spurt phenomenon than plantar flexion; massage repeated at high frequency is less beneficial for venous

Ambulatory sleep-wake polygraphy in narcolepsy.

We have presented an ambulatory monitoring and off-line data analysis system for sleep-wake polygraphy and have illustrated its use for the diagnosis of narcolepsy. The advantages over conventional polysomnography have been discussed.

Reversion mode activation by myopotential sensing in a ventricular inhibited demand pacemaker.

Pacemaker follow-up in a 72-year-old woman revealed occasional failure to sense and pace, which was confirmed by Holter monitor. Neither reprogramming the pacemaker sensitivity nor repositioning the lead resolved the problem. A recheck of the Holter recordings revealed pacing and sensing failures were concurrent with "baseline artifact," suggestive of myopotentials. Furthermore, the inappropriate pacing spikes occurred at a rate of 90 pulses per minute (ppm). It was theorized that myopotential sensing was alternately inhibiting the pacer and activating the reversion mode, an asynchronous rate

Automated ambulatory blood pressure monitoring: a study in age-matched normotensive and hypertensive men.

Noninvasive automated techniques have been used in this study in 29 ambulatory normotensive and hypertensive men to monitor blood pressure during a full 24-hour period. The two groups of men were carefully matched for age. The data were analyzed with respect to possible differences in the circadian pattern of blood pressure, in the variability of blood pressure, and differences in the prevalence of abnormal blood pressures between the two groups. The circadian pattern of blood pressure in hypertensive patients was displayed parallel to that in normotensive control subjects but at a significant

Peri-operative management and administration of anaesthesia in children with congenital complete heart block. A case report and review.

The clinical presentation and natural history of congenital complete heart block (CHB) differ from those of acquired third-degree heart block. Although perioperative prophylactic cardiac pacing is considered mandatory in patients with acquired CHB, it is not usually necessary in children with asymptomatic congenital heart block. The anaesthetist should be able to identify which patients require temporary perioperative pacing, and should modify his anaesthetic technique appropriately for patients who do not. An 8-year-old patient with congenital CHB who required emergency surgery for acute appe

Home monitoring of heart failure patients at risk for hospital readmission using a novel under-the-mattress piezoelectric sensor: A preliminary single centre experience.

Introduction A piezoelectric sensor (PS) converts mechanical deformations into electrical signals. We used a contactless under-the-mattress PS to monitor physiological vibrations resulting from breathing, pumping of the heart, and body movements, among individuals at home following hospitalization for heart failure (HF). Our objectives were to assess acceptability of the device in the home, to assess physiological patterns, and to determine if altered patterns correlate with readmission. Methods We conducted a prospective observational study of 30 patients discharged home following HF hospital

Telehealth by Home Monitoring and Video Consultation for Children With Cystic Fibrosis: Qualitative and Quantitative Study.

BACKGROUND: Clinical outcomes for patients with cystic fibrosis have improved over the last decades, with a focus on enhancing the quality of life for children and their families. These improvements are expected to reduce the need for in-hospital visits, allowing for alternative care provisions that further enhance their quality of life. Telehealth can decrease the number of in-hospital visits and disruptions to everyday life. OBJECTIVE: This study aims to assess the feasibility of home monitoring with subsequent video consultations and to explore the experiences of families and health care p

Wearables for Telemonitoring in ATTR-Amyloidosis: Current Perspectives.

Wearable sensors enable continuous recording of electrocardiographic, photoplethysmographic, and inertial signals and have accelerated the development of digital biomarkers in cardiovascular medicine. Transthyretin amyloidosis (ATTR) is a progressive multisystem disease characterized by arrhythmia, conduction disturbances, hemodynamic impairment, autonomic dysfunction, and gait abnormalities, making it theoretically suitable for multimodal wearable monitoring. This review summarizes current knowledge on wearable applications in amyloidosis with ATTR serving as an illustrative case, evaluates t

Time-burden insults of neuromonitoring signals: practical implications for the management of acute brain injury.

PURPOSE OF REVIEW: To explore recent insights into measures of time-burden insults in intracranial pressure (ICP) monitoring, and potential implications for clinical management. RECENT FINDINGS: The ICP is an important therapeutic target in patients with traumatic brain injury (TBI) and some other brain injuries. Current clinical guidelines in TBI recommend starting treatment above a fixed ICP threshold of 22 mmHg. The concept of ICP burden was introduced recently, which takes both intensity and duration of an episode of elevated ICP into account. This burden of ICP is visualized in a colour-

Cardiac output measurements during high-risk Cesarean section using electrical bioreactance or arterial waveform analysis: assessment of agreement.

OBJECTIVE: Maternal hemodynamics change significantly during Cesarean section complicated by massive hemorrhage or severe hypertensive disease. Cardiac output (CO) monitoring aids early, goal-directed hemodynamic therapy. The aim of this study was to record hemodynamic changes observed during Cesarean section in pregnancies at high risk of hemodynamic instability, using invasive (LiDCOrapid™) and non-invasive (NICOM®) devices, and to assess agreement between the two devices in measuring CO. METHODS: Simultaneous intraoperative hemodynamic measurements were taken using the LiDCOrapid and NICOM
