Deep brain stimulation (DBS) in movement disorders management: exploring therapeutic efficacy, neurobiological mechanisms, and clinical implications.

INTRODUCTION: Deep brain stimulation (DBS) has emerged as a promising therapeutic approach, offering targeted neuromodulation for movement disorders refractory to medical management or stereotactic surgery. However, assessing its benefits against potential risks is essential. This meta-analysis aims to evaluate the efficacy and safety of DBS in movement disorders, shedding light on its role as an alternative therapeutic option. METHODS: A comprehensive search of databases after systemic review yielded studies published in English from 2000 to the present. Data selection, screening, extraction…

Differential resting-state functional connectivity patterns in functional movement disorders: Evidence for gait-disorder-specific aberrations.

BACKGROUND: The clinical presentation of Functional Movement Disorders (FMD) is highly variable, encompassing gait disturbances and a wide range of hyper- and hypokinetic movement abnormalities. The neurobiological correlates distinguishing different phenotypes, particularly functional gait disorders, remain poorly understood. OBJECTIVE: To investigate whether functional gait disorders are associated with specific patterns of resting-state functional connectivity that distinguish them from other FMD phenotypes. METHODS: Thirty-eight FMD patients (9 with isolated gait disorders, 9 with combin…

Mixed Methods Survey to Identify Barriers to Accessing Deep Brain Stimulation for Movement Disorders in Canada.

BACKGROUND: Movement disorders (Parkinson's disease, essential tremor, dystonia) are debilitating, progressive conditions that profoundly impact patients' quality of life. Surgical therapies, such as deep brain stimulation (DBS) can provide tremendous relief to patients but remain costly and, therefore, limited in availability. It is critical to understand regional barriers to accessing this service to improve access for all patients who may benefit from it. METHODS: This is a mixed methods survey of stakeholders (patients/family members, advocacy groups, family physicians, neurologists, neur…

Direct targeting for focused ultrasound thalamotomy in the treatment of movement disorders: a retrospective cohort study.

BACKGROUND: Accurate targeting of the ventral intermediate nucleus (Vim) remains a critical challenge in stereotactic thalamotomy for essential tremor (ET) and tremor-dominant Parkinson's disease (TDPD). Indirect atlas-based methods suffer from interindividual anatomical variability and poor visualization of thalamic substructures. We evaluated the clinical impact of a direct targeting strategy enabled by fast gray matter acquisition T1 inversion recovery (FGATIR) imaging in MR-guided focused ultrasound (MRgFUS) thalamotomy. METHODS: We conducted a retrospective cohort study of adult patients

Development of the Movement Disorders Interpretation Bias Scale and psychometric evaluation in adults with Huntington's disease.

INTRODUCTION: Anxiety is widespread but undertreated in Huntington's disease (HD). Targeting negative interpretations of ambiguity may reduce anxiety in HD, but no valid measure of negative interpretation biases in HD exists. We developed the Movement Disorders Interpretation Bias Scale (MDIB) to measure negative interpretation biases in movement disorders (e.g., HD, Parkinson's disease [PD]) and evaluated its validity in adults with HD (preregistration: https://osf.io/26a3h). METHODS: We developed the MDIB based on qualitative interviews and expert review, then invited adults with HD (N = 65…

Relationship between drug-induced movement disorders and psychosis in adults living in precarious housing or homelessness.

BACKGROUND: Studies have reported positive associations between drug-induced movement disorders (DIMDs) and symptoms of psychosis in patients with schizophrenia. However, it is not clear which subtypes of symptoms are related to each other, and whether one symptom precedes another. The current report assessed both concurrent and temporal associations between DIMDs and symptoms of psychosis in a community-based sample of homeless individuals. METHODS: Participants were recruited in Vancouver, Canada. Severity of DIMDs and psychosis was rated annually, allowing for the analysis of concurrent as…

Automatic two-dimensional & three-dimensional video analysis with deep learning for movement disorders: A systematic review.

The advent of computer vision technology and increased usage of video cameras in clinical settings have facilitated advancements in movement disorder analysis. This review investigated these advancements in terms of providing practical, low-cost solutions for the diagnosis and analysis of movement disorders, such as Parkinson's disease, ataxia, dyskinesia, and Tourette syndrome. Traditional diagnostic methods for movement disorders are typically reliant on the subjective assessment of motor symptoms, which poses inherent challenges. Furthermore, early symptoms are often overlooked, and overlap…

Limitations of Multigene Next-Generation Sequencing Panel for "Cerebral Palsy" Phenotype and Other Complex Movement Disorders.

In the past couple of decades, literature in pediatric neurology and clinical genetics has identified hundreds of monogenic disorders that can masquerade as infantile cerebral palsy (CP). Accurate and prompt diagnosis in such cases may be challenging due to several reasons. There are commercial multigene CP panels, but their diagnostic yield is often limited compared with exome sequencing because of diverse etiologies that may mimic CP. We report one such case where a patient with spastic hemiplegia underwent a long diagnostic journey before genetic diagnosis was established with exome sequenc

Low-Intensity Transcranial Ultrasound Stimulation: Mechanisms of Action and Rationale for Future Applications in Movement Disorders.

Low-intensity transcranial ultrasound stimulation (TUS) is a novel non-invasive brain stimulation technique that uses acoustic energy to induce changes in neuronal activity. However, although low-intensity TUS is a promising neuromodulation tool, it has been poorly studied as compared to other methods, i.e., transcranial magnetic and electrical stimulation. In this article, we first focus on experimental studies in animals and humans aimed at explaining its mechanisms of action. We then highlight possible applications of TUS in movement disorders, particularly in patients with parkinsonism, dy…

Randomized controlled trials on the use of cannabis-based medicines in movement disorders: a systematic review.

Anecdotal references, preclinical, and non-randomized studies support the therapeutic potential of cannabinoids for movement disorders (MD). To create an evidenced-based point of view for patients and physicians, we performed a systematic review of randomized controlled trials (RCT) on the use of cannabinoids in MD. The seven RCTs found on PD used different cannabis formulations. No improvement of motor symptoms was shown in any of the two RCTs with this as primary outcome (PO), but in the nabilone group, an improvement in quality of life was documented. Of the three RCTs having levodopa-induc…
