The Health Education During Root Canal Therapy Reduces the Use of Antibiotics.

Although antibiotics can effectively control infectious diseases, overuse of antibiotics can cause problems such as drug resistance. Pulpitis and apical periodontitis are among the most common infectious diseases in humans. To alleviate the intense tooth pain, patients often self-administer antibiotics. To guide patients in managing pulpitis and apical periodontitis more effectively, we designed a Root Canal Treatment Health Education Form and provided personalised guidance on antibiotic use during the treatment period. From January 2024 to March 2025, a total of 272 patients participated in t

Impact of Multiple Individualized Guides on the Management of Obliterated Root Canals in a Maxillary First Molar: A Case Report.

This report evaluated the impact of using multiple individualized endodontic access guides in the management of obliterated root canals in maxillary molars. A 70-year-old female patient presented with pain in the upper left first molar. Intraoral examination revealed an extensive resin composite restoration, with no signs of tooth mobility or marginal periodontal disease. The clinical diagnosis was symptomatic apical periodontitis. Cone beam computed tomography (CBCT) revealed a hypodense area in the mesiobuccal and palatal (P) roots, associated with signs of apical periodontitis and obliterat

The influence of intraoral cryotherapy on postoperative pain and substance P in symptomatic apical periodontitis: randomized clinical study.

Cryotherapy is widely utilized in medicine, particularly for pain management. This randomized clinical trial aimed to assess the effect of intraoral cold pack application (cryotherapy) on postoperative pain (POP) and the level of Substance P (SP) in patients with symptomatic apical periodontitis (SAP). Enrolled patients were randomly assigned to either cryotherapy or control group. After adequate anesthesia, access cavity, and biomechanical preparation of the root canal system were completed, the first apical fluid (AF) sample (S1) was obtained. A custom-made intraoral ice-gel pack was applied

Ferumoxytol nanozymes effectively target chronic biofilm infections in apical periodontitis.

Bacterial biofilms are pervasive and recalcitrant to current antimicrobials, causing numerous infections. Iron oxide nanozymes, including an FDA-approved formulation, ferumoxytol (FMX), show potential against biofilm infections via catalytic activation of hydrogen peroxide (H2O2). However, clinical evidence regarding the efficacy and therapeutic mechanisms of FMX is lacking. Here, we investigate whether FMX nanozymes can treat chronic biofilm infections and compare their bioactivity to that of the gold standard sodium hypochlorite (NaOCl), a potent but caustic disinfectant. Clinical performanc

Systemic inhibition of 5-lipoxygenase by MK-886 exacerbates apical periodontitis bone loss in a mouse model.

BACKGROUND: To investigate if 5-LO selective inhibitor (MK-886) could be used for systemic treatment of experimentally induced apical periodontitis in a mouse model. METHODS: Twenty-four C57BL/6 mice were used. After coronal opening, a solution containing Escherichia coli LPS (1.0 µg/µL) was inoculated into the root canals of the lower and upper right first molars (n = 72 teeth). After 30 days apical periodontitis was established, and the animals were treated with MK-886 (5 mg/kg), a 5-LO inhibitor, for 7 and 14 days. The tissues were removed for histopathological and histometric analyses, ev

Cross Talk between Apical Periodontitis and Metabolic Disorders: Experimental Evidence on the Role of Intestinal Adipokines and Akkermansia muciniphila.

INTRODUCTION: Infection and dysbiosis present a close relationship with metabolic diseases although the influence of apical periodontitis (AP) in this context needs further investigation. This study evaluated the influence of AP in a rat model of metabolic syndrome induced by 10% fructose supplementation. METHODS: Male Wistar rats were used. Animals that received a high-fructose diet (HFD, n = 30) or filtered water (control, n = 30) were subdivided into the following groups: (1) without induction of AP (no AP, n = 10 each), (2) with AP induction 2 weeks before euthanasia (AP 14 days, n = 10 e

Odontogenic orbital abscess: a case report and review of literature.

Odontogenic orbital abscess is a rare but well-documented complication of sinusitis and infections spreading from dental apical lesion. We report a case of orbital abscess with periorbital cellulitis, in a 35-year-old man with positive recent dental history of a periapical dental infection arising from the second upper left premolar spread into maxillary sinus. The patient has shown facial edema, ocular pain, ophthalmoplegia, proptosis, and initial visual symptoms. A surgical intervention to drain the abscess and a revision of the dental lesion and maxillary sinus were required. A review of li

Responses of the pulp, periradicular and soft tissues following trauma to the permanent teeth.

Trauma to the permanent teeth involves not only the teeth but also the pulp, the periodontal ligament, alveolar bone, gingiva and other associated structures. There are many variations in the types of injuries with varying severity and often a tooth may sustain more than one injury at the same time. In more severe trauma cases, there are many different cellular systems of mineralized hard and unmineralized soft tissues involved, each with varying potential for healing. Furthermore, the responses of the different tissues may be interrelated and dependent on each other. Hence, healing subsequent

An overview on biologic medications and their possible role in apical periodontitis.

INTRODUCTION: Apical periodontitis (AP) is the expression of a deficient balance between infection and the host immune response. METHODS: If reducing the bacterial load from the root canal and preventing its reinfection may lead to clinical success, then the integrity of the nonspecific immune system has a relevant influence on the outcome of endodontic treatment. RESULTS: Compromised immune systems and/or genetic alterations of the host's response may as well play an important role on the development, progression, and healing of AP. Thus, immunomodulatory drugs might have the potential to i

Results after wisdom tooth transplantation. A retrospective study.

Wisdom tooth transplants offer youth the possibility of biologically fixed tooth replacement in cases of premolar agenesis or premature loss of a molar. In the present study, 57 transplants of third molars were reviewed and evaluated retrospectively on preoperative findings (root growth stages, extraction sites, indication for transplantation), on postoperative clinical findings (local gingivitis, periodontal probing values, tooth mobility, percussion sound and percussion pain) and on radiological findings (tertiary build-up of dentin, osseous periradicular conditions, progress of root growth)

Incision techniques and flap designs for apical surgery in the anterior maxilla.

Apical surgery is often a last resort, and is used to surgically preserve a tooth with a recurrent lesion of endodontic origin when conventional re-treatment is neither indicated nor clinically or financially feasible. Incision type and flap design are important factors to consider when outlining the surgical area: the first must ensure optimal access to the root end and the second must allow visibility. A variety of factors must be considered when choosing a specific incision technique, particularly in the anterior maxilla. These include status of the marginal periodontium, location and exten

[Local and potential systemic consequences of endodontic root infection].

In root infections, bacteria are present not only in planktonic cells but also in biofilms, which are more resistant to host defence mechanisms and disinfectans. Apical periodontitis, which may be radiographically undetectable, may develop or persist as a host defence mechanism to prevent the systemic spread of bacteria and their by-products to other sites of the body. The risk of spreading microorganisms and septic emboli is present especially in compromised hosts; furthermore, long-standing inflammation may have systemic effects and affect general health. Effective procedures should be devel

EM evaluation of bacterial biofilm and microorganisms on the apical external root surface of human teeth.

The aim of this study was to evaluate the presence of bacterial biofilm on the external surface of the root apex in teeth with pulp necrosis, with and without radiographically visible periapical lesions, and in teeth with a vital pulp. Twenty-one teeth were extracted, eight with pulp necrosis and periapical lesions, eight with pulp necrosis without radiographically visible periapical lesions, and five with a vital pulp. The roots were sectioned, and the root apexes (+/- 3 mm) were processed for scanning electron microscope evaluation. The surface of the apical root was evaluated for the presen

[Odontogenic brain abscess. 2 case reports].

The odontogen brain abscess is a relatively rare, but very serious complication of an infection of the teeth. Frequently the bacteria found by aspiration of the brain abscess are the only indication of a dental focus. The diagnostic problems in confirming a covert dental infection as the cause of the brain abscess are illustrated.

Capacity of anaerobic bacteria from necrotic dental pulps to induce purulent infections.

Combinations of bacteria isolated from the root canals of teeth with necrotic pulps and periapical bone destruction were tested for their capacity to induce abscess formation and transmissible infections when inoculated subcutaneously into guinea pigs. Transmissible infections could be induced with combinations obtained from teeth with purulent apical inflammation, but not with combinations from symptomless teeth with chronic apical inflammation. All combinations which gave transmissible infections contained strains of Bacteroides melaninogenicus or B. asaccharolyticus (formerly B. melaninogen

Hypercementosis Progression With Evidence of Acquired Concrescence: A Case Report.

This case report highlights the importance of periodic dental examinations in identifying progressive hypercementosis and its rare complication, root concrescence, which can complicate future dental extractions. Hypercementosis may be idiopathic or secondary to either local factors or systemic disorders. However, evolution to concrescence raises questions about future treatment approaches should the involved teeth become nonrestorable and require extraction. This article presents a case of generalized hypercementosis affecting multiple quadrants with progression. The severe hypercementosis led
