Cataract surgeries in posterior polar cataracts (PPCs) are always challenging for an ophthalmologist, in spite of multiple techniques described for phacoemulsification surgery. The most important objective of cataract surgeries in PPCs is to achieve maximum removal or debulking of the nucleus so that, if any complications happen, they can be easily manageable. We describe a new technique to manually crush the nucleus after it is manually prolapsed out of the bag into multiple pieces with the help of two Y-rotators which were then removed by using a phacoemulsification probe so that complicatio…

INTRODUCTION: The requirement for very deep akinesia has decreased with the use of modern phacoemulsification technique for cataract surgery. The use of topical anesthesia has increased as a way to reduce complications associated with anaesthesia with injection and to allow the most rapid visual recovery. The objective of this study was to assess the patient reported pain in phacoemulsification cataract surgery under topical anaesthesia versus peribulbar anaesthesia administered using an injection.
MATERIALS AND METHODS: The subjects for this study were the patients undergoing phacoemulsifica…

Using the Topographic Modeling System (TMS), we studied the early changes in anterior corneal curvature following cataract surgery. Thirty patients who had planned extracapsular cataract extraction (p-ECCE) or phacoemulsification were evaluated with TMS at three days, and one, two, three, and five weeks postoperatively. After p-ECCE, the upper part of the cornea which corresponded to the wound and the lower cornea markedly steepened and showed a bow-tie pattern of astigmatism. Eight (57%) of 14 cases showed little reduction in the corneal steepening at five weeks postoperatively. In five cases…
