Hysteroscopic tubal sterilization: an evidence-based analysis.

BACKGROUND: Hysteroscopic tubal sterilization is a minimally invasive alternative to laparoscopic tubal ligation for women who want permanent contraception. The procedures involves non-surgical placement of permanent microinserts into both fallopian tubes. Patients must use alternative contraception for at least 3 months postprocedure until tubal occlusion is confirmed. Compared to tubal ligation, potential advantages of the hysteroscopic procedure are that it can be performed in 10 minutes in an office setting without the use of general or even local anesthesia. OBJECTIVE: The objective of t

Vitamin B12 intramuscular injections versus oral supplements: a budget impact analysis.

BACKGROUND: Vitamin B12 deficiency can lead to adverse health effects such as anemia and, in some cases, permanent neurologic damage. In Canada, patients with vitamin B12 deficiency are typically given intramuscular injections, which incur considerable cost and inconvenience. The clinical evidence-based analysis has found that oral supplementation is as effective as intramuscular injections. OBJECTIVES: This economic analysis aimed to estimate the cost savings of switching from intramuscular injections to high-dose oral supplements for patients aged 18 years and older with confirmed vitamin B

Addressing Limited Evidence in Guideline Development.

BACKGROUND: Evidence-based guidelines are essential to optimize healthcare decision-making, yet in contexts such as rare diseases, emerging conditions, or specific subpopulations, direct evidence to estimate the magnitude of intervention effects may be minimal or absent. In these situations, guideline panels must formulate recommendations despite very low certainty evidence on benefits and harms. To address this dilemma, we explore three strategies that guideline panels may implement. RESULTS: The first involves systematic recollection of expert clinical experiences, a structured, timely, and

Enhancing prediabetes and diabetes detection through a machine learning-enabled self-assessment approach.

OBJECTIVES: Reliable, accessible, noninvasive self-assessment screening for prediabetes/diabetes is lacking, leading to missed opportunities for early intervention. We aimed to develop and externally validate a machine learning (ML)-derived self-assessment system to predict the likelihood of prevalent prediabetes or diabetes using easily accessible health parameters. STUDY DESIGN AND SETTING: We analyzed 30 years (1988-2018) of National Health and Nutrition Examination Survey (NHANES) data (N = 17,458). ML models predicting prediabetes/diabetes risk (composite outcome: fasting plasma glucose

Feasibility and image quality evaluation of accelerated T2-weighted and time-of-flight MRI using AI-assisted compressed sensing and deep reconstruction.

BACKGROUND: Magnetic Resonance Imaging (MRI) provides high-resolution, non-invasive diagnostic images, but long acquisition times increase patient burden and motion artefacts. Compressed sensing helps reduce scan duration, yet further image quality improvements remain necessary. Recent advances in artificial intelligence (AI) have shown promise in enhancing image quality and accelerating MRI acquisition. In this study, we aim to evaluate the feasibility of accelerated MRI acquisition using AI-assisted compressed sensing (ACS) and deep reconstruction by assessing scan time, quantitative image q

Evaluating the integration of the reported treatment mechanisms and goal of treatment in randomized controlled trials for low back pain: protocol for a meta-epidemiological analysis.

OBJECTIVES: Treatment mechanisms for exercise, cognitive behavioral therapy (CBT) and spinal manipulative therapy (SMT) are broadly theorized and examined in basic science studies; however, they are not typically incorporated in the design of randomized controlled trials (RCTs) for low back pain (LBP). The overall aim of this study is to answer the question, "what is the role of mechanisms and goal of treatment in RCTs for LBP?" Specifically, the objective is to answer: (1) how many RCTs of LBP consider an intervention-specific mechanism?, (2) are these mechanisms incorporated in their study d

Melatonin as a Neuroprotective Adjunct in Bipolar Patients Undergoing Electroconvulsive Therapy: A Double-blind Randomized Controlled Trial.

INTRODUCTION: Bipolar disorder (BD) is frequently treated with electroconvulsive therapy (ECT), which, despite its efficacy in severe mood episodes, is often complicated by cognitive impairment. To our knowledge, no standard prophylactic therapy currently exists to prevent these adverse effects. Melatonin, with established antioxidant and neuroprotective properties, may attenuate ECT-related cognitive dysfunction. METHODS: In this randomized, double-blind, placebo-controlled trial, 46 inpatients with BD undergoing ECT were allocated 1:1 to receive either melatonin (3 mg nightly) or identical

Optimizing the therapeutic ratio of PSMA-PET-guided para-aortic radiotherapy for prostate cancer: A systematic review and structured quantitative synthesis.

BACKGROUND: The increasing use of prostate-specific membrane antigen positron emission tomography (PSMA-PET) has transformed the detection of para-aortic lymph node recurrence in prostate cancer, creating new opportunities for metastasis-directed radiotherapy. However, the optimal balance between oncologic benefit and treatment-related toxicity remains uncertain, particularly when comparing focal stereotactic body radiotherapy (SBRT) with elective nodal irradiation (ENI). PURPOSE: To systematically evaluate the clinical, dosimetric, and radiobiological evidence regarding PSMA-PET-guided para-

Attention-Deficit/Hyperactivity Disorder Subtypes Defined by Cognition Have a Distinct Neural and Clinical Profile and Differ in Response to Atomoxetine.

OBJECTIVE: Attention-deficit/hyperactivity disorder (ADHD) is clinically and etiologically heterogeneous across multiple levels of organization. This study sought to parse the heterogeneity of ADHD by identifying cognitive subtypes based on common features of ADHD, including inhibitory control, sustained attention, and working memory. To validate the subtypes, constructs at other meaningful levels of organization were used: quantitative electroencephalogram (qEEG), clinical symptoms, and response to treatment with atomoxetine and methylphenidate. METHOD: Hierarchical cluster analysis of cogni

Workplace violence experienced by occupational therapists who visit people in their own homes.

BACKGROUND: Workplace violence is a common experience for many health professionals. However, little is known about the experience of occupational therapists, specifically those that visit people in their own homes, and experience violence in that context. Home visiting is a fundamental component of practice for many occupational therapists and often takes place with the occupational therapist being alone. This study aimed to explore the experience of this group of occupational therapists, their responses to workplace violence such as reporting, and the impact of their experience on their well

Metformin use in women with polycystic ovary syndrome (PCOS): Opportunities, benefits, and clinical challenges.

Metformin, a synthetic biguanide, is widely used to manage type 2 diabetes, and is commonly prescribed in polycystic ovary syndrome (PCOS) to address insulin resistance and associated metabolic and reproductive disturbances. PCOS is characterised by hormonal imbalances such as hyperandrogenism and anovulation, metabolic abnormalities including insulin resistance and increased cardiometabolic risk, and higher rates of pregnancy complications. However, the role of metformin in the multifaceted nature of PCOS remains debated. This review synthesises the mechanisms of action of metformin and its e
