The embryology, classification, evaluation, and reconstructive management of complex craniofacial clefts, a group of exceedingly rare and highly variable congenital anomalies, is presented. Using the Tessier system as a structural framework, the characteristic features of each cleft subtype and highlights, key diagnostic considerations, including assessment of soft-tissue deficiency, skeletal disruption, and associated functional concerns are described. Emphasis is placed on the application of fundamental reconstructive principles: respect for facial subunits, restoration of symmetry, and stag

Cosmetic penile enhancement represents an expanding field characterized by increasing demand, limited evidence, and significant ethical complexity. Though reconstructive interventions remain essential in the management of anatomical and functional pathology, their extension to the augmentation of normal anatomy remains difficult to justify. In this context, the reconstructive urologist must adopt a measured and principled approach, prioritizing patient safety, rigorous assessment, and ethical restraint. Ultimately, responsibility lies not in the expansion of cosmetic practice, but in safeguard

INTRODUCTION: Tumors located near the clitoris in vulvar (pre)malignancies represent a distinct clinical category. Achieving tumor-free surgical margins often requires removal of a significant portion of the clitoris, potentially causing profound psychological and sexual impact. Clitoral reconstruction, used in patients with female genital mutilation/cutting (FGM/C), has shown positive outcomes in restoring sexual function and reducing pain.
OBJECTIVES: This scoping review presents an overview of the literature on clitoral reconstruction after treatment for vulvar (pre) malignancies, incorpor

The repair of small to medium-sized tongue defects is controversial among surgeons. This can be achieved using various local, regional, and microvascular free flaps. In this Technical Note, the authors introduce a new classification system for tongue defects based on the subunit principle and describe a technique using a modified local rhomboid flap, which is aimed at restoring both the volume and function of the anterior tongue.

Surgical reconstructive techniques continue to evolve as dermatosurgeons combine clinical expertise, technical skill, and innovation. In some cases, such innovation can result in better outcomes in terms of tension management, vascular supply preservation, and esthetic appearance. The double-opposing rotation advancement flap, also known as the Ying-Yang flap due to its yin-yang mode of action, is often applied in the scalp region, where it provides excellent results with minimal complications. However, data supporting its application in less conventional anatomical sites remains limited. We p

BACKGROUND: Scalp burns pose a unique challenge to the reconstructive surgeon given the anatomical characteristics of the head and neck. This challenge is magnified in severe burn injuries extending to the calvarial bone. Several algorithms for scalp and calvarial reconstruction have been presented in the past. However, no consensus exists about the optimal reconstructive approach. Our aim is to develop a novel algorithm for scalp burn reconstruction based on the relevant literature as well as case observations from the growing multidisciplinary field of neuroplastic surgery.

Orbital fractures, commonly resulting from blunt trauma like accidents or assaults, comprise a significant proportion of facial fractures. Effective management involves a multidisciplinary approach and computed tomography imaging with 3D reconstruction. Surgical repair aims to restore orbital volume and correct visual deficits, with conflicting data to suggest optimal timing for intervention. Implant materials range from autologous grafts to alloplastic options like titanium mesh. Recent advancements include intraoperative navigation and patient-specific implants, improving precision and outco

BACKGROUND: External auditory canal (EAC) atresia is a congenital anomaly leading to hearing impairment. Various techniques have been used for its surgical correction, but no gold standard exists.
METHOD: This paper describes a fascia-skin composite grafting technique utilizing temporal muscle fascia and postauricular skin for EAC reconstruction.
CONCLUSION: Temporal fascia-skin composite grafting offers a reliable and effective option for EAC reconstruction with favorable results and minimal complications.

The Abbe flap is a valuable reconstructive option for secondary deformities of the upper lip following cleft lip repair. In this report, the author present four cases successfully treated with the Abbe flap, demonstrating its reliability and aesthetic outcomes. Each procedure was performed under local anesthesia, utilizing a flap based on the left labial artery. The technique proved effective in addressing whistling deformity, philtrum loss, and upper lip retraction. Even surgeons without prior experience in this flap can achieve functional and cosmetic success when the indication is correct.

Facial symmetry is paramount in societal perceptions of attractiveness, with symmetric faces receiving higher ratings. This is particularly relevant for individuals with microtia, a congenital condition affecting external ear formation, who often experience psychosocial challenges such as anxiety and depression. Auricular prostheses and High-density porous polyethylene (MEDPOR®) offer an aesthetic solution. However, they are related to disadvantages like color mismatches, periodic replacement, and skin infections. Currently, the Nagata technique, regarded as the "gold standard" for microtia tr

Chronic proximal interphalangeal (PIP) joint instability is a rare and difficult problem to treat, with the main options including collateral ligament reconstruction and joint arthrodesis. To date, there are minimal described techniques for collateral ligament reconstruction. We describe a novel technique utilizing a slip of the flexor digitorum superficialis (FDS), harvested through a single minimal incision. This technique can be used for collateral ligament reconstruction with multiple applications, including chronic recurrent PIP joint dislocations and instability during PIP joint arthropl

There are no standardized guidelines for reconstructive surgery of large temporal bone defects following lateral temporal bone resection for external auditory (acoustic) meatus carcinoma. Filling the defect with well-vascularized tissue is important for large tissue defects to promote wound healing and prevent infection postoperatively. Patients with malignant tumors of the external acoustic meatus requiring lateral temporal bone resection may sometimes necessitate postoperative adjuvant chemoradiotherapy. Therefore, it is essential to facilitate wound healing and initiate adjuvant therapy pro

BACKGROUND: The facial artery musculomucosal (FAMM) flap is a versatile flap based on the facial artery. It can be used to reconstruct the floor of the mouth, palate, tongue and alveolar ridge. The flap can be designed in various orientations and modified as an islanded or tunnelised flap. This study aimed to review the application of FAMM flap in the reconstruction of defects of the tongue and oral cavity, with specific focus on success rates and total complications.
METHODS: A literature search was conducted by 2 independent reviewers on PubMed, Dynamed, DARE, EMBASE, Cochrane and British M

BACKGROUND: Conservative treatment remains a well-established concept for managing chronic lymphedema; however, it substantially impacts the patients' quality of life owing to the considerable time investment and strict adherence required. Therefore, lately lymphatic reconstructive procedures have gained popularity. We aimed to provide a detailed overview of the outcomes of lymphatic reconstruction and evaluate the efficacy of surgical interventions.

Various surgical techniques have been utilized for thumb reconstruction in the setting of aphalangia. Given abnormal proximal anatomy, nonvascularized toe phalangeal transfer has been a popular surgical technique in a subset of patients with monodactylous symbrachydactyly. However, altering the appearance of a noninvolved foot is often unappealing to many parents. Here we detail a surgical technique of thumb reconstruction utilizing the index metacarpal as a nonvascularized graft to improve thumb length and stability. This technique avoids the donor site morbidity associated with toe phalangea

Vulvar cancer is a rare malignancy, accounting for approximately 5% of all gynecological cancers, but its incidence has increased. The gold standard treatment is complete surgical resection with safety margins, which may vary according to histological subtype. This surgery often results in large defects, which may be challenging to repair international guidelines recommend that reconstructive procedures after vulvar cancer surgery should always be considered in cases where it will guarantee better functional and/or cosmetic results or when wound closure will be challenging.
