Narcolepsy: a neuropathologic study.

This article describes the neuropathologic findings in a woman who died suddenly at the age of 48 after having had symptoms of narcolepsy for 1 1/2 years. Gross examination of the brain showed an arachnoid cyst. The significant light microscopic findings consisted of focal gliosis of the ventrolateral caudal pons and the periventricular anterior hypothalamus. Although cases of symptomatic narcolepsy have been previously reported, to our knowledge structural brain changes in idiopathic narcolepsy have not been previously described. In the present case, there is no evidence that the narcoleptic …

Pontomedullary encephalitis and basal meningitis due to Listeria monocytogenes: report of a case.

A case of CNS listeriosis in a 49-year-old man, previously in good health, is described. The illness showed a biphasic clinical pattern with a prodromal phase characterized by headache, fever and leukocytosis and subsequent development of pontomedullary cranial nerve paresis. The infection was rapidly progressive and fatal. Gram-positive rods, identified as Listeria monocytogenes, were detected in CSF samples only at the end stage of illness. Neuropathological abnormalities consisted of rhombencephalitis and basal meningitis. The major difficulties encountered in the early diagnosis of CNS lis

Recovery after central pontine myelinolysis.

Central pontine myelinolysis (CPM) is known to be almost universally fatal. Most published cases are based on autopsy reports, whereas antemortem diagnosis is rare. We present a case in which the diagnosis of CPM was confirmed by computed tomography (CT); the patient had two symptoms not previously reported and made a remarkable recovery.

Clinicopathological study of pontine hemorrhage.

This report concerns a clinico-pathological study of 60 patients afflicted with primary pontine hemorrhage. The illness was fatal in 43, 17 patients survived. Ophthalmic signs, autonomic disturbances and transient visual hallucination were observed and discussed. A ruptured microaneurysm within the border of a pontine hematoma was detected in this study, and in the first report of such a finding.

["Locked-in" syndrome in intensive care patients (author's transl)].

The "locked-in" syndrome constitutes a potential source of diagnostic and prognostic errors, especially in intensive care patients. The pathologicoanatomical, pathophysiological and clinical features of the syndrome are described and differentiated from other syndromes. Means of establishing contact are reviewed which would allow simple wishes on the part of the patient to be satisfied and thus prevent the psychic isolation of the (conscious) patient.

Ascending projections to the hypothalamus and limbic nuclei from the dorsolateral pontine tegmentum: a biochemical and electron microscopic study.

Axons arising from the dorsolateral pontine tegmentum of the rat were traced in various hypothalamic and limbic nuclei by the electron microscopic degeneration method (0.5-8 day survival times) and by measuring regional norepinephrine (NE) concentrations after 12 days of survival using a radioenzymatic method. Significant reductions (41-85%) in NE contents were observed in the supraoptic, arcuate, basal and lateral amygdaloid nuclei and in the hippocampus 12 days after the bilateral electrolytic lesions of the locus coeruleus. No changes in NE concentrations were observed in the ventromedial,

A case of narcolepsy-cataplexy associated with distemper encephalitis.

Narcolepsy associated with localized brain lesions is described in a 10-month-old Argentine Dogo. Neurological examination and MRI study suggested an inflammatory lesion of the left frontal lobe. Postmortem examination revealed diffuse encephalitis in the forebrain and marked necrotic lesions in the ventral pontine area. Immunohistochemistry for distemper virus antigen showed positive staining of the cytoplasm of many neurones of the pons and cerebral cortex. The pathological pattern was suggestive of post-vaccinal distemper encephalitis and the localization of the lesions was consistent with …

Metabolic studies of the edematous cerebral cortex of the pyrithiamine-treated thiamine-deficient rat.

The pyrithiamine-treated rats produced by our method histopathologically exhibited the edematous lesion not only around the third ventricle and at the tegmentum of pons, as expected, but also in the cerebral cortex. These animals were, therefore, used in a biochemical study of the cerebral cortex, in particular, glucose metabolism. The results indicate an accumulation of lactate and severe impairment of the lipid metabolism in the edematous lesion of the cerebral cortex among the pyrithiamine-treated rats. The fact that oxygen consumption was hardly diminished, however, strongly suggested that…

Neurochemical changes in experimental African trypanosomiasis in voles and mice.

Dopamine, norepinephrine, serotonin and related metabolites were measured in the brain of meadow voles and mice after infection with Trypanosoma brucei gambiense with the goal of understanding the neurochemical changes accompanying infection with this parasite. Serotonin and 5-HIAA levels both fell by 26%, and HVA levels rose by 56%, in vole brains during infection. Dopamine, DHPG and norepinephrine levels, however, remained unchanged. In mice, serotonin levels dropped by 17 and 23%, respectively, in the pons-medulla and midbrain regions after infection; 5-HIAA and dopamine levels remained una…

Aneurysms of the basilar artery trunk.

Ten cases of basilar artery trunk aneurysms are reported; nine were operated on by a middle subtemporal approach and one by a pterional transsylvian technique. The middle subtemporal approach facilitated clipping of aneurysms located as deep as 25 mm below the posterior clinoid process as seen on the lateral views of the angiogram, such that the junction of the vertebral arteries was visible in most cases. The authors emphasize the usefulness of direct retraction of the trigeminal nerve, the pons, and the aneurysm itself; occasionally, drilling off the pyrimidal edge is also valuable. Surgical…

Involvement of noradrenergic systems in the effects of conditioning stimulation of the lower brain stem on the lumbar spinal reflex in rats.

The effects of conditioning stimulation at various positions in the pons and the medulla on the lumbar spinal monosynaptic reflex (MSR) were examined in rats. The conditioning stimulation facilitated the MSR. Facilitation with long latency was completely blocked by phenoxybenzamine, chlorpromazine or haloperidol. Phenoxybenzamine and chlorpromazine inhibited the MSR. Haloperidol, on the other hand, augmented the MSR, and this effect became more potent after the brain stem had been transected at the upper part of the medulla oblongata. These results suggest that pontine noradreneric systems hav…

Partial dorsal mesencephalic hemorrhages: report of three cases.

We studied three patients with spontaneous dorsal mesencephalic hemorrhages. One patient had Parinaud's syndrome with a unilateral hemorrhage confined to the rostral tectal plate. The second patient had vertical gaze palsy, skew deviation, and bilateral Horner's syndrome due to a unilateral hemorrhage that involved the superior colliculus and extended anteriorly into the midbrain tegmentum and inferiorly to the rostral dorsal pons. The third patient had a hemorrhage in the caudal tectal plate, with bilateral fourth cranial nerve palsies, unilateral Horner's syndrome, and ataxia. There was hydr…

Progressive idiopathic strio-pallido-dentate calcinosis (Fahr's disease) with autosomal recessive inheritance. Report of three siblings.

3 siblings with symmetrical calcifications in the strio-pallido-dentate system are described. Parathyroid function was normal and there were … that the calcifying process is a progressive disorder. It seems to start in the dentate nuclei and pons, and subsequently extends to the basal ganglia and to the radiation of the corpus callosum.

Temporal bone histopathology associated with occlusion of vertebrobasilar arteries.

We have presented the histological findings in the inner ear of a patient who experienced occlusion of the vertebral and basilar arteries 17 days before death. A large infarction was observed in the medulla and pons on the right side. There was severe degenerative change in the right membranous labyrinth as well as in the right cochlear and vestibular nerves, which was presumably caused by ischemia from occlusion of the right labyrinthine artery. The organ of Corti and sensory epithelia of the vestibular labyrinth showed the most advanced state of degeneration. The histopathological appearance…
