Planetary health at a decade: from Bellagio to practice.

INTRODUCTION: In its first formally defined decade, the "planetary health" framing has had demonstrable impact but barriers to more radical action remain. OBJECTIVES: We reflect on progress during planetary health's first decade, identify barriers limiting its wider uptake and application, and propose pragmatic communication approaches for engaging health practitioners, policymakers and the public. METHODS: We have drawn on our experience as two practitioners working at the nexus of climate and health, merging our perspective with a targeted literature search. RESULTS: In its first ten year

Marxist concepts of alienation in relation to health: a narrative review of the evidence.

Public health scholars are increasingly interested in the Marxist concept of alienation to explain the existence of health inequities. Marx's use of the term stresses the crucial role of political economic structural factors in creating alienation. This article reports on a narrative review of literature that used Marx's theory of alienation and focuses on the mental and/or physical health effects of alienation. Our search resulted in 34 relevant articles. Collectively, they demonstrate the ongoing relevance of the concept of alienation to the creation of health inequities in contemporary soci

An audit of tuberculosis health services in prisons and immigration removal centres.

BACKGROUND: Tuberculosis (TB) is the second leading cause of death worldwide due to a single infectious agent. Rates of active TB in places of prescribed detention (PPD), which include Prisons, Young Offender Institutions and Immigration Removal Centres, are high compared with the general population. PPD therefore present an opportunity to develop targeted health programmes for TB control. This audit aims to assess current service provisions and identify barriers to achieving best practice standards in PPD across London. METHODS: Twelve healthcare teams within PPD commissioned by NHS England

Locked in and left out: the "prison penalty" for implementation of evidence-based interventions.

BACKGROUND: While the broader medical community grapples with the widely accepted notion that it takes an average of 17 years for research evidence to be incorporated into clinical practice, the implementation of evidence-based interventions in carceral settings (i.e., jails and prisons) faces longer delays, exacerbating health disparities. MAIN BODY: The "prison implementation penalty" describes the significant delay in and limited adoption of evidence-based healthcare practices in carceral settings. We explore the complex challenges of implementing evidence-based interventions in jails and

Complex systems thinking in emergency medicine: A novel paradigm for a rapidly changing and interconnected health care landscape.

INTRODUCTION: The specialty of emergency medicine is experiencing the convergence of a number of transformational forces in the United States, including health care reform, technological advancements, and societal shifts. These bring both opportunity and uncertainty. 21ST CENTURY CHALLENGES: Persistent challenges such as the opioid epidemic, rising health care costs, misaligned incentives, patients with multiple chronic diseases, and emergency department crowding continue to plague the acute, unscheduled care system. REDUCTIONISM AND COMPLEX SYSTEMS THINKING: The traditional approach to healt

Stigma and HIV testing behavior in a nationally representative sample of US adults: implications for public health campaigns and stigma monitoring.

Research has examined the impact of HIV stigma on people with HIV (PWH), but little work has explored how stigmatizing beliefs toward PWH and instrumental stigma - the misplaced fears of infection through everyday contact with PWH - may serve as a barrier to HIV testing. We analyzed responses from 3,271 US adults in the fall 2021 ConsumerStyles Survey to estimate the prevalence of these stigmas and examine associations with sociodemographic factors. Multivariate regression showed that adults who identify as heterosexual, those aged 65 years and older, those with lower educational attainment, t

Potential public sector cost-savings from over-the-counter access to oral contraceptives.

OBJECTIVE: This study estimates how making oral contraceptive pills (OCPs) available without a prescription may affect contraceptive use, unintended pregnancies and associated contraceptive and pregnancy costs among low-income women. STUDY DESIGN: Based on published figures, we estimate two scenarios [low over-the-counter (OTC) use and high OTC use] of the proportion of low-income women likely to switch to an OTC pill and predict adoption of OCPs according to the out-of-pocket costs per pill pack. We then estimate cost-savings of each scenario by comparing the total public sector cost of prov

Women's experiences with the use of medical abortion in a legally restricted context: the case of Argentina.

This article presents the findings of a qualitative study exploring the experiences of women living in Buenos Aires Metropolitan Area, Argentina, with the use of misoprostol for inducing an abortion. We asked women about the range of decisions they had to make, their emotions, the physical experience, strategies they needed to use, including seeking health care advice and in dealing with a clandestine medical abortion, and their overall evaluation of the experience. An in-depth interview schedule was used. The women had either used misoprostol and sought counselling or care at a public hospita

Impact of face washing and environmental improvement on reduction of active trachoma in Vietnam-a public health intervention study.

PURPOSE: A public health intervention study in Vietnam was conducted between 2002 and 2005 to assess the impact of improved water and sanitation facilities and increased awareness about active trachoma in the community. METHODS: In My Thon village (MT), all four components of the SAFE strategy were implemented. In Xom Ngoai village (XN), only the S & A components of the trachoma control strategy were implemented. The villagers improved water and sanitation facilities in MT. Women's Union and school authorities assisted in increasing awareness in the community. Ophthalmologists examined the re

Best practice in dementia health care: Key clinical practice pointers from a national conference and innovative opportunities for pharmacy practice.

OBJECTIVE: Sub-optimal care of people living with dementia has serious consequences for older populations. The 2021 Australian Royal Commission noted that a large proportion of older adults in aged care live with dementia, yet there are limitations in the knowledge and understanding of staff who care for them. In the pursuit of educating pharmacists, physicians, allied health care professionals, researchers, academics, people living with dementia and their carers, and the public, who are facing the challenges of dementia management, the 'Best Practice in Dementia Health Care' conference was he

An Enquiry on the Ethics of One Medicine.

One Medicine is not One Health. Although both concepts connect animal and human health, One Health has arguably become a frame for public health. This paper argues that One Medicine uniquely centres on animal and human patients and speaks to the equal interdependence of their health, though it has also (historically) been adopted as a response to the circumstances of living in and with nature. The evolution of One Medicine can be seen as 'jumping together,' or the consilience of ideas and disciplines over time, giving a new sense in theory that unifies previously unconnected phenomena found in

Medical debt during epidemics: A case for resolving the situation in low- and middle-income countries such as Kenya.

This paper evaluates the problem of medical debt in Kenya during the COVID-19 pandemic. The medical debt problem is compounded during pandemics such as COVID-19 when patients seek treatment and end up in insurmountable debt because illnesses related to the pandemic are not covered by the Kenyan National Health Insurance Fund (NHIF), the public health coverage body under government control. As a result, discharged patients may be detained in hospitals and dead bodies are locked away in mortuaries, until relatives and friends fundraise and clear the bills. Apart from causing vulnerability, fear,

Gambling and the Health of the Public: Adopting a Public Health Perspective.

During the last decade there has been an unprecedented expansion of legalized gambling throughout North America. Three primary forces appear to be motivating this growth: (1) the desire of governments to identify new sources of revenue without invoking new or higher taxes; (2) tourism entrepreneurs developing new destinations for entertainment and leisure; and (3) the rise of new technologies and forms of gambling (e.g., video lottery terminals, powerball mega-lotteries, and computer offshore gambling). Associated with this phenomenon, there has been an increase in the prevalence of problem an

Dividends and Disconnects: Involving Public and Stakeholder Groups in the Development of a Modelling Study Exploring Income Policy and Health.

BACKGROUND: Policy models increasingly guide policy decision-making. Incorporating public and stakeholder perspectives into modelling processes can produce evidence that is more relevant, appropriate and acceptable to the public. However, public and stakeholder involvement (PSI) in modelling studies is an emerging practice in the field of health inequalities, with little guidance available to support researchers. OBJECTIVE: We present a methodological reflection on a PSI process supporting development of a policy model estimating the health impacts of major income supplementation policies. We

Establishing maternal and child health data collection priorities for state and local oral health programs.

OBJECTIVE: This paper identifies specific data items for use by state and local agencies in a maternal and child oral health needs assessment model. METHODS: A modified Delphi approach was used to develop consensus on items for inclusion in the data set and their relative importance. Initially, 31 data items were chosen from several national sources. All state dental directors, along with other selected administrators and advisory committee members for this process, were asked to categorize each of the data items as core (essential), important but optional, or of lesser importance. John Snow: Anesthesiologist, Epidemiologist, Scientist, and Hero.

A 19th century physician was crucial to the establishment of 2 medical specialties-anesthesiology and public health. Everyone whose interest in public health has increased in the last year will be amazed at Dr John Snow's career in anesthesiology. Those who recognize him as the first full-time physician anesthetist will be struck by his development of medical mapping during the Cholera Pandemic of 1848, resulting in one of the fundamental techniques of epidemiology and public health that has continued through today. Snow's accomplishments in anesthesiology and epidemiology reflected a concaten

The cycle of homelessness in the lives of young mothers: the diagnostic phase of an action research project.

This paper presents the findings of the first stage of an action research project designed to explore the experience of repeated homelessness in the lives of young women. An action research strategy was selected because it offered a framework within which to work with and for rather than on this vulnerable group of service users. The women participating in the research developed a conceptual model that captured the essence of their experience, which they named 'Our Cycle'. They also proposed a system for service delivery, 'From trauma to recovery: a non-professional model'. The diagnostic phas
