PURPOSE/OBJECTIVES: To develop evidence-based practice guidelines for and standardize the care of radiation skin reactions.
DATA SOURCES: Peer-reviewed scientific journals and texts and a survey of the guidelines in use at leading cancer treatment facilities in Canada, the United States, the United Kingdom, and Australia.
DATA SYNTHESIS: A formal reference document with recommended guidelines was developed. Consensus was obtained from all relevant disciplines, and the guidelines were implemented successfully into practice.
CONCLUSIONS: The document introduced a major change in practice from

We report the first case of bullous pemphigoid complicating radiation therapy for vulvar cancer. Shortly after completion of postoperative radiation therapy for a TIN1 vulvar carcinoma, the patient presented with a rash that started within, but continued to extend, well beyond the radiation field. A biopsy of the lesions confirmed the diagnosis of bullous pemphigoid, and she had prompt clinical resolution with systemic tetracycline and steroids.

PURPOSE: Radiotherapy (RT) can prevent progression of Dupuytren's contracture (DC). It is unknown whether there is a dose response and which dose is sufficient. Herein, the 1-year results of a prospective randomized trial are presented which compared two different RT dose concepts with each other.
METHODS: 129 patients (67 males; 62 females) were entered in this study: 69 had bilateral and 60 uni-lateral involvement of DC accounting for 198 irradiated hands. According to Tubiana's classification, 73 hands had Stage N (nodules/cords, no extension deficit = flexion deformity), 61 had Stage N/I

Radiation-induced acne is a rare, clinically and pathogenetically ill-defined acneiform dermatosis with special features that may occur in irradiated skin areas especially after high doses of deeply penetrating radiation. We report on a patient with an oropharyngeal carcinoma who developed severe radiation-induced acne including comedones and cysts as well as few inflammatory papules and pustules in a skin area irradiated with up to 63 gray of a 6 MeV photon beam. Acnegenic drugs may precipitate the disease; our patient was on longterm therapy with carbamazepine whose acnegenic potency is less

Acute and chronic exposures to non-physiological doses of UV-light lead to a variety of changes of the skin. The most significant of these changes are a premature aging of the skin, UV-induced hyperkeratosis or atrophy, provocation of skin diseases and neoplasms of the skin. Furthermore, the local and systemic immune response can be negatively modulated. As long as society views tanned skin as a sign of beauty and success, and provided no behavioral changes occur, it will be required to emphasize prevention through education and the use of external and internal sun protection products. It is p

In experiment with mice and rats radioprotective properties of direct alpha-adrenomimetic drugs: indraline, mesaton (phenylephrine) and naphthyzine (naphazoline) have been investigated by local early and late radiation injuries. The drugs were S. C. and C. administrated at the site of the thigh of the hind of the animal at intervals of 5-8 min locally was irradiated with gamma-60Co-rays in the doses of 26.3-53.7 Gy at a rate of the dose of 1.31-1.54 Gy/min. When S. C. injected the topical effect of the adrenomimetics was higher than the systemic one. In experiment with mice radioprotective eff

PURPOSE/OBJECTIVES: To develop a method of predicting the severity of acute radiation skin reactions.
DESIGN: Prospective, descriptive correlational with repeated measures.
SETTING: Outpatient radiation oncology department in a metropolitan public teaching hospital in Western Australia.
SAMPLE: 126 adult women aged 30-78 receiving postlumpectomy radiation therapy for breast cancer.
METHODS: Data about potential predictive factors were collected during the first week of radiation. Weekly observations of the skin reaction using the Radiation Therapy Oncology Group scoring system were recorde

Sub-cutaneous and muscular fibrosis are common and irreversible late effect of radiation on normal tissues. An experiment was designed to test the effectiveness of superoxide dismutase in reducing late radiation injury. This study was performed in an experimental porcine model of acute localized gamma irradiation simulating human accidental overexposure: 12 Large White pigs were irradiated on the thigh with a collimated gamma 192Ir source, so that the dose was 160 Gy/skin (100%) and 40 Gy/2 cm depth (25%). In this model, fibrosis appears in 4 to 5 months. The heterogeneous sclerotic tissue is

The management of local radiation injuries is influenced by the degree of vascular compromise within the skin and underlying tissues. Other authors have used thermography and angiography in assessing the blood flow to radiation damaged tissues. This report describes the use of radionuclide imaging in the evaluation of a patient who developed necrosis of his distal digits following a radiation accident. In addition to determining the vascular status of the hands, imaging helped indicate an appropriate level for amputation.

By definition de-epithelization is a technique by which a graft or a flap is thinned out of the split thickness or full thickness skin layer. The underlying fat tissue is thus covered more or less by corium only. The indications for this procedure are as follows: 1. A subcutaneous defect. 2. Resurfacing defects in a cavity (for example in the pharynx) 3. Subcutaneous implantation of a flap pedicle. The advantages of de-epithelization are: 1. Widening the scope of surgical technique. 2. In many cases second surgery can be avoided. 3. The way from the donor to the recipient site is shorter. 4. A

Skin reaction in the rat's foot was used as an experimental method to evaluate the protective potential of profound temporary hypoxia against roentgen radiation produced by a linear accelerator. Hypoxia was induced by intraarterial injection of degradable starch microspheres. The irradiation dose needed to evoke a certain reaction in the hypoxic feet was 2.0 times the dose giving the same reaction in a control group. The results point towards a new technique of inducing local hypoxia in healthy tissues which need to be protected in an irradiation field.

Radon seeds, formerly used for vascular and neoplastic tumors, acne, and other dermatological disorders, are rarely, if ever, used today. Because the half-life of radon is 3.83 days, these hollow gold seeds filled with radon gas are usually left in situ permanently. A case is reported of a woman who had seeds implanted 33 years ago for a vascular lesion. The seeds were removed and found to have minute amounts of residual radiation but not sufficient to cause radiation damage. Since seeds are foreign bodies, removal is recommended if they are easily accessible.
