Abnormal hemodynamic response to Valsalva maneuver in patients with atrial septal defect evaluated by Doppler echocardiography.

Hemodynamic responses to the Valsalva maneuver were studied in eight healthy subjects (group 1) and eight patients with ASD (group 2) using Doppler echocardiography. The acute changes of aortic blood flow profiles during the Valsalva maneuver were investigated on a basis of beat-to-beat estimation. During the active strain phase (phase 2), group 1 showed a significant decrease in systolic blood pressure, SV and CO with reflex tachycardia; in group 2, there was a significant decrease in SV and CO with reflex tachycardia, but not systolic blood pressure. The percentage decreases in SV and CO in

Klumpke's Palsy Following Minor Trauma: A Rare Presentation in a Young Adult.

The brachial plexus is a complex network that transmits motor and sensory signals between the spinal cord and the upper limbs. Klumpke's palsy, a rare injury affecting the lower brachial plexus (C8-T1 roots), commonly occurs in newborns or adults after severe trauma and is characterized by a claw-hand deformity, sensory loss along the medial forearm and hand and weakness in the small muscles of the hand. Adult-onset Klumpke's palsy following minor trauma is exceptionally rare. We report a unique case of isolated right-sided Klumpke's palsy in a 31-year-old male after a trivial fall from bed. N

Case Report: Ocular Tilt Reaction with Internuclear Ophthalmoplegia and Multiple Cranial Nerve Palsies.

SIGNIFICANCE: Ocular tilt reaction (OTR) is an abnormal eye-head postural reaction that consists of skew deviation, head tilt, and bilateral ocular torsion. Understanding of the pathway of the vestibulo-ocular reflex (VOR) is essential because this will help to localize the pathology. PURPOSE: The aim of this study was to report a case of OTR with contralateral internuclear ophthalmoplegia (INO) and fifth and seventh cranial nerve palsies. CASE REPORT: A 51-year-old gentleman with underlying diabetes mellitus presented with sudden onset of diplopia for 3 days. On examination, his visual acui

[A 62-year-old man with an acute onset of consciousness disturbances].

We report a 62-year-old man who developed coma and died in a fulminant course. The patient was well until May 1, 1996 when he noted chillness, tenderness in his shoulders, and he went to bed without having his lunch and dinner. In the early morning of May 2, his families found him unresponsive and snoring; he was brought into the ER of our hospital. He had histories of hypertension, gout, and hyperlipidemia since 42 years of the age. On admission, his blood pressure was 120/70, heart rate 102 and regular, and body temperature 36.3 degrees C. His respiration was regular and he was not cyanotic.

The effect of dynamic visual acuity on rapid naming in school-aged children with dyslexia.

OBJECTIVES: A decrease in vestibulo-ocular reflex (VOR) gain can lead to retinal slip, negatively impacting visual acuity. During reading, eye movements are essential for smoothly transitioning between lines and words. Impaired VOR may disrupt these movements, resulting in additional corrective actions and affecting reading speed. This study aims to explore the relationship between dynamic visual acuity and rapid naming skills in children with dyslexia, compare dynamic visual acuity between children with dyslexia and neurotypical children, and assess gender-related differences. METHODS: Dynam

[A 32-year-old man who developed a posterior fossa mass 12 years after the radiation therapy for cerebellar arteriovenous malformation].

We report a 32-year-old man who developed cerebellar ataxia and a posterior fossa mass 12 years after the radiation therapy for a cerebellar arteriovenous malformation (AVM). The patient was well until 19 years of the age when he had an acute onset of vertigo and vomiting. A spinal tap was performed and the CSF was bloody. He was admitted to another hospital where an arteriovenous malformation was found in the cerebellum by angiography. Four years after the onset, he developed tingling sensation in the distribution of the second division of the right trigeminal nerve. He was admitted to the ne

The replica dilemma: older patients' views on virtual models made from their health data-a qualitative study.

BACKGROUND: Virtual models (digital twins, AI-simulated cohorts) built from patient data are transforming medical research, including applications in Transcatheter Edge-to-Edge Repair, yet the ethical perspectives of older patients, who are disproportionately represented in these datasets, remain critically unexplored. This study investigated how older adults with chronic conditions understand the use of their data for virtual model construction, what concerns emerge regarding digital replicas, and what rights they believe should accompany such data use. METHODS: A descriptive qualitative stu

Effect of fatigue on muscle latency, muscle activation and perceived discomfort when exposed to whole-body vibration.

Whole-body vibration and muscle fatigue have both been shown to delay the trunk muscle reflex response and increase trunk muscle activation, leading to an increased risk of low back injuries. However, the effects of whole-body vibration on previously fatigued trunk muscles have never been tested, despite studies showing that prolonged exposure to whole-body vibration can lead to muscle fatigue. The purpose of this research was to investigate the effects of muscle fatigue on muscle latency, muscle activation and perceived discomfort when exposed to whole-body vibration. The results showed that

Vagal nerve stimulation activates vagal afferent fibers that reduce cardiac efferent parasympathetic effects.

Vagal nerve stimulation (VNS) has been shown to have antiarrhythmic effects, but many of these benefits were demonstrated in the setting of vagal nerve decentralization. The purpose of this study was to evaluate the role of afferent fiber activation during VNS on efferent control of cardiac hemodynamic and electrophysiological parameters. In 37 pigs a 56-electrode sock was placed over the ventricles to record local activation recovery intervals (ARIs), a surrogate of action potential duration. In 12 of 37 animals atropine was given systemically. Right and left VNS were performed under six cond

[Total right hemispherectomy: neurophysiological study after twenty-six years].

Twenty-six years after a total right hemispherectomy for tumor when thirteen years old, a right-handed patient presented with disturbances of equilibrium, urinary incontinence, and a marked reduction in motor impulses, in spite of a subnormal intelligence quotient (0.85). Computed tomography examinations suggested the presence of a normal pressure hydrocephalus. However intracranial pressure monitoring and isotopic and cerebrospinal fluid perfusion tests showed the absence of any cerebral obstruction, and free communication between the cerebral and spinal spaces, though intracranial volume cap

A Rare Case Report of Neuroimaging, Electrophysiological, and Dynamic Laryngoscopy Studies in a Stroke Patient of Foix-Chavany-Marie Syndrome.

Foix-Chavany-Marie Syndrome (FCMS), also known as opercular syndrome, is a rare neurological disorder caused by an ischemic stroke that results in autonomic-voluntary dissociation. In this case report, we describe a patient who presented with an acute right frontal and parietal cortical lesion, in addition to a remote infarction in the left lateral paraventricular region. The patient exhibited a masked face, facial paralysis, dysarthria, dysphagia, and apraxia of eyelid closure while automatic pathways, such as crying and laughing, remained intact. MRI revealed an acute ischemic lesion in the

Brief periods of recurrent reproducible asystole during tibial nailing: a possible variant of the Gallagher Wilkinson reflex.

We present the case of an elderly man who was admitted to hospital following a road traffic accident for fracture fixation of the right tibia. He presented with the comorbid conditions of type 2 diabetes mellitus, systemic hypertension, bronchial asthma, organic brain disorder and probable coronary artery disease. He was administered general anaesthesia for the operation. Intraoperatively, a total of 6 periods of severe bradycardia/asystole were noticed, lasting from 2 s to 6 s, which resolved spontaneously. This correlated with the timing of intramedullary reaming and returned to sinus rhythm

Infantile hemiparesis secondary to periventricular leukomalacia: clinical findings and multidisciplinary rehabilitation outcomes.

A middle childhood child with infantile hemiparesis is the subject of this case study. He has left-sided weakness and delays in both gross and fine motor milestones, particularly in the upper limb. The child's delayed cry and need for NICU admission, despite a full term and vaginal delivery, suggested perinatal hypoxia. The results of the MRI showed slight right lateral ventricular asymmetry and periventricular leukomalacia, which are signs of hypoxic-ischaemic encephalopathy. Clinical assessment revealed isolated motor impairment, hypertonia and elevated deep tendon reflexes. The start of phy

Atypical Presentation of Creutzfeldt-Jakob Disease: A Stroke Mimic.

A 56-year-old woman presented with a five-week history of unsteadiness, progressive diplopia, tremulous speech, and mild cognitive decline. Although initial computed tomography (CT) imaging was normal, magnetic resonance imaging (MRI) revealed bilateral diffusion restriction in the basal ganglia and thalami, raising suspicion for prion disease, which was subsequently confirmed by cerebrospinal fluid real-time quaking-induced conversion (CSF RT-QuIC) testing. A neurological examination showed saccadic intrusions, persistent horizontal diplopia, mild limb hypertonia, right-sided dysmetria, absen

Hyperhomocysteinemia as a Cause of Acute Ischemic Stroke in a Four-Year-Old Child: A Report of a Rare Case.

Homocysteine is a type of amino acid that isn't genetically encoded by the human body. This amino acid is capable of causing oxidative damage to the endothelial cells, leading to the onset of thrombosis. Moreover, it can also inflict harm to neurons by activating pro-apoptotic factors, causing DNA damage, and inducing oxidative stress, as observed in various animal models and cell cultures. This case report highlights a four-year-old girl who exhibited signs of an ischemic stroke. The neurological examination revealed several symptoms, including anisocoria, decreased tone, decreased power, abs

[A severe, reversible encephalopathy after prolonged use of valproic acid].

Valproic acid is an effective mood stabilizer, registered for the treatment of bipolar disorder and epilepsy. Side effects of valproic acid are transient and generally well tolerated. A rare side effect is a valproic acid-induced encephalopathy. We saw a case of valproic acid-induced encephalopathy without hyperammonemia in a 71-year-old woman. She had used valproic acid as a mood stabilizer over the course of 16 years for a bipolar I disorder. The following clinical symptoms were observed: staring eye contact, somnolence, disorientation, hypotenacity, bradyfrenia, mutism and akathisia. Severe
