Urinary catheterization in care homes for older people: self-reported questionnaire audit of catheter management by care home staff.

A self-administered questionnaire was used to determine care home staff's reported knowledge of the urinary catheter care standards published by the National Institute for Clinical Excellence (NICE) and the Association of Continence Care, and to see whether this differed in homes with higher catheterization rates. Seven hundred and fifty out of 1438 (52%) nursing and care staff from 37 randomly selected care homes with high, medium and low catheterization rates responded. There was no difference in reported practice in care homes in the three health districts sampled or those with differing ca

The self-lined superiorly based pull-through velopharyngoplasty: plastic surgery-speech pathology interaction in the management of velopharyngeal insufficiency.

We present a rationale and step-by-step description of a previously unpublished innovative surgical technique designed to overcome velopharyngeal insufficiency. This procedure maintains the anatomic integrity and physiologic function of the velopharynx by limiting interpalatal dissection and incorporates a method for flap attachment through the velum that is accessible, predictable, and versatile. The efficacy of this procedure in eliminating velopharyngeal insufficiency was evaluated by using rigorously controlled quantitative psychophysical scaling procedures of presurgical and postsurgical

Positive psychological resources, self-management behavior, and glycemic outcomes in adolescents with type 1 diabetes and elevated diabetes distress.

OBJECTIVE: Positive psychological constructs (e.g., benefit finding, resilience, and diabetes strengths) have been associated with greater engagement in self-management behaviors among teens with type 1 diabetes (T1D). However, for teens with T1D experiencing diabetes distress, these potentially protective associations are less understood. To explore the potential protective functions of positive psychological constructs, we investigated their associations with subjective and objective diabetes self-management behaviors and health outcomes among adolescents with diabetes distress. METHODS: Th…

Behavioral Mechanisms of a Digital Health Intervention for Self-Management in Type 2 Diabetes Mellitus: Prospective Longitudinal Cohort Study.

BACKGROUND: Sustaining self-management is critical for optimizing clinical outcomes in individuals with type 2 diabetes mellitus (T2DM). Although digital health interventions (DHIs) have shown benefits for glycemic control and self-care, much of this evidence has focused on efficacy, and the behavioral mechanisms through which DHIs produce sustained effects remain insufficiently understood. Clarifying these mechanisms could inform the development of theory-driven interventions. OBJECTIVE: This study examined the longitudinal behavioral pathways through which Artificial Intelligence-based Heal…

An Integrated, Evidence-Guided Framework for Chronic Kidney Disease Self-Management: Behavioral Mechanisms, Implementation Strategies, and Real-World Application.

BACKGROUND: Chronic kidney disease (CKD) affects over 850 million people worldwide and contributes substantially to cardiovascular complications, reduced quality of life, and high healthcare utilization. Self-management support can improve patient activation, self-management behaviors, and quality of life, yet application in routine CKD care remains inconsistent. Many existing programs lack explicit specification of behavioral mechanisms and implementation-relevant delivery approaches, limiting reproducibility and real-world scale-up. SUMMARY: We conducted a structured narrative review with f…

Prioritising modifiable risk factors for self-management of urinary incontinence in older men: integrating expert consensus with patient readiness.

BACKGROUND: Urinary incontinence (UI) affects up to one-third of men aged ≥65 years, yet evidence to guide the selection of targets for self-management interventions remains limited. Although many contributing risk factors are potentially modifiable, little is known about which are considered clinically important by experts and are also viewed by older men as feasible and worthwhile to address. This study combined expert consensus with older men's perspectives to identify candidate priorities for future self-management intervention development. METHODS: We conducted a sequential multi-methods…
