This article reviews the pathogenesis and clinical and imaging findings in shoulder impingement syndrome. Shoulder impingement is caused by compression of the supraspinatus tendon underneath the coracoacromial arch, mainly in forward flexion of the arm. Different stages of impingement syndrome are described. Stage I relates to edema and hemorrhage of the supraspinatus tendon. Stage II is characterized by bursal inflammation and fibrosis, as well as tendinopathy. In stage III there is a tear of the rotator cuff. Clinical signs may overlap. Moreover, calcifying tendinitis, fractures and pain ori

INTRODUCTION: Clavicular hook plate fixation is widely used for the surgical management of high-grade acromioclavicular (AC) joint injuries and generally provides reliable clinical outcomes. Nevertheless, implant-related complications involving the acromion have been reported. Stress-related fractures at the base of the acromion following hook plate fixation are extremely rare and remain poorly described in the literature.
CASE REPORT: A 48-year-old woman sustained a high-grade AC joint injury and underwent open reduction and internal fixation with a clavicular hook plate. Two months after su

INTRODUCTION:: Proximal humerus fractures are the seventh most frequent fracture in adults, and the third in patients over 65 years old, 5.7% of whole diagnosed fractures. Most of these fractures can be treated conservatively and achieve good results. However, more and more frequently we are confronted with dislocated and multifragmentary fractures, and with elderly and high functional demanding patients. In patients with osteoporosis and poor general conditions external fixation can be performed as rapid and mininvasive procedure with good outcome and low complication rates. The authors inves

Peripheral nerve injury after shoulder trauma is an underestimated complication. The distribution of the affected nerves has been reported to be heterogeneous in previous studies. This study aimed to describe the distribution of peripheral nerve injuries in patients with a history of shoulder trauma who were referred to a tertiary care electrodiagnostic laboratory. A retrospective chart review was performed for all cases referred to a tertiary care electrodiagnostic laboratory between March 2012 and February 2020. The inclusion criteria were a history of shoulder trauma and electrodiagnostic e

A 42-year-old male police officer sustained multiple injuries in a motorcycle accident, including a comminuted fracture of the scapula and several rib fractures. Four months after his injury, the patient was prescribed outpatient physical therapy, after 2 months of which the patient's cervical and lumbar spine function were restored but his shoulder mobility deficits persisted. A second physical therapy opinion was sought 6 months post injury for the left shoulder. Given his functional deficits, including an inability to return to work, he was referred to an orthopaedist for additional imaging

BACKGROUND: Shoulder dislocation is a common reason for admission in the emergency room. Its recurrence rate may be as high as 96% due to the associated bone lesions. The purpose of this study is to identify the underreporting of lesions comparing those identified in the initial X-ray with those identified in the post-reduction X-ray.
MATERIAL AND METHODS: X-rays of 46 patients with suspected anterior glenohumeral dislocation were assessed for diagnostic purposes and to identify the initial bone lesions. After patients had undergone reduction, post-reduction serial bone X-rays (AP, Stryker, W

Proximal humerus fractures in the setting of osteoporosis can be technically challenging. Intraoperative challenges include comminution, thin cortical bone, and crushed cancellous bone that lead to difficulties in obtaining and maintaining a reduction and hardware fixation. Loss of fixation and varus collapse continue to be problems despite the utilization of modern locking plate fixation. A clearer understanding of predictors of fixation failure and the encouraging early results of reverse total shoulder arthroplasty (RTSA) have resulted in increased utilization of RTSA for the primary treatm

Four-fragment fractures have to be accorded great importance because they are so severe. The morphology allows differentiation of two types, the dorsolateral fracture type and the impacted fracture type. Over a 4-year time span, 13 of these fractures were treated with minimal osteosynthesis. This involve the open anatomical reduction and allogeneic spongiosa grafting and retention of the fracture with screws and, if necessary, K-wires and tension banding. After an average of 26.3 months the shoulder function was evaluated using the CONSTANT score. The dorsolateral fractures were scored of 87.2

The purpose of this retrospective study is to evaluate the perinatal outcome in cases with birth injuries, suggesting shoulder dystocia. This survey involves 92 cases of live newborns (gestational age between 37 and 42 weeks) with shoulder injuries. Another control group of 120 cases of live mature newborns without any shoulder injuries is studied for comparison. There are no significant differentialities between those two groups, regarding age, parity, weight and height of the parturients. Shoulder dystocia is most frequently found in newborns of 3500-4000 g birthweight. Antepartum, risk fact

BACKGROUND: Treatment of lateral fractures of the clavicle is challenging and has been controversially discussed for a long time due to high non-union rates in non-operative treatment and high complication rates in surgical treatment. Acromioclavicular joint instability due to the injury of the closely neighbored coraco-clavicular ligaments can result in a cranialization of the medial clavicle shaft. A recently developed implant showed a promising functional outcome in a small collective of patients.
METHODS: In this prospective study, 20 patients with a mean age of 40.7 ± 11.3 years with a d

Two cases of acromioclavicular dislocation associated with fracture of the coracoid process are described. This association is unusual and the coracoid fracture may only be recognised on an axillary radiograph of the shoulder. Both the cases were treated conservatively, with good results. The management and outcome of other cases described in the literature are reviewed and discussed.

BACKGROUND AND PURPOSE: Proximal humeral fractures (PHF) can be managed surgically or non-surgically. Locking plates have been the preferred head-preserving surgical technique while hemiarthroplasty (HA) or reverse shoulder arthroplasty (RSA) have been used in joint replacement surgery. We describe the epidemiology and trends in management of acute PHF in Denmark with a focus on (i) changes in the incidence of PHF; (ii) changes in the proportion of surgical cases; and (iii) changes in preferred surgical techniques.
PATIENTS AND METHODS: Data on diagnoses and interventions was retrieved from t

Proximal humerus fractures (PHF) are a common orthopedic injury; however, their treatment remains largely controversial with evidence supporting a wide array of treatments. Although many injuries can be treated nonoperatively, there has been much debate about surgical management of PHF. A detailed review of the literature was performed relative to operative management options specifically related to implant choices. Although no definitive answers are available regarding best practice, there is literature to guide operative decision-making and implant selection based on both patient- and surgeo

BACKGROUND: Fractures of the proximal humerus are common. The most frequent surgical treatment option is open reduction and locking plate fixation. Multifragmentary fractures, including 3- and 4-part fractures, are especially challenging to treat because they correlate with an increased risk of fixation failure. In the past, several mechanisms of additional fixation were investigated, but none directly addressed the lesser tuberosity (LT). The goal of this study was to investigate the biomechanical impact of additional anterior fracture fixation in lateral locked plating (LLP) of 4-part proxim

Periarticular fractures are common, especially in the elderly. Decision-making is dependent on fracture pattern as well as on patient- and surgeon-related factors. This article explores current concepts in decision-making for either reconstruction or primary prosthetic replacement of the proximal humerus. Selection criteria were relevance of data, number of cases greater than 20, and comparability of results.

Plate fixation for unstable fractures of the proximal humerus has seen mixed results as evidenced by the trials of new methods of fixation. The deltopectoral surgical approach is most frequently used and requires significant muscle retraction and soft tissue stripping to expose the lateral humeral neck. This may contribute to avascular necrosis and fixation failure. Lateral approaches have been limited to 5 cm distal to the acromion because of the course of the anterior branch of the axillary nerve. A recent anatomic study has demonstrated the predictability of the position of the axillary ner

Splitting fractures of the humeral head are rare; part of the humeral head dislocates and the unfractured part remains attached to the shaft. We report eight cases in young patients. In five the diagnosis was made at presentation: three had minimal internal fixation using a superior subacromial approach, one had a closed reduction and one a primary prosthetic replacement. All five patients regained excellent function with no avascular necrosis at two years. In three the injury was initially unrecognised; two developed a painless bony ankylosis and one is awaiting hemiarthroplasty. It is import
