Design and Testing of the Degree of Shoulder Involvement in Sports (DOSIS) Scale.

BACKGROUND: For athletes affected by shoulder problems, the most important expectation is to resume sporting activities. The ability to return to sport is related to several parameters, including the type and level of sport played. By focusing on these parameters, the Degree of Shoulder Involvement in Sports (DOSIS) scale allows for a better assessment of the involvement of the shoulder in sports. PURPOSE: To design the DOSIS scale and test its psychometric features. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: The DOSIS scale was developed as a patient self-administered scale

Moderate value of non-contrast magnetic resonance imaging after non-dislocating shoulder trauma.

PURPOSE: The purpose of the present study was to determine the value of shoulder magnetic resonance imaging (MRI) obtained in the community setting interpreted by musculoskeletal radiologists in patients with shoulder pain initiated by a single non-dislocating shoulder trauma. METHODS: In 56 of 61 consecutive patients who underwent shoulder arthroscopy due to pain after a single non-dislocating shoulder trauma, the data sets of non-contrast MRI were complete. These were retrospectively interpreted by three radiologists specialized on musculoskeletal MRI who were blinded for patients' history

Host response to xenograft ECM implantation is not different between the shoulder and body wall sites in the rat model.

In the context of tendon and ligament repair, mechanical loading and the presence of joint synovial fluid are known to profoundly influence the form and function of the repair tissue and potentially the host response to biomaterials. Previously, we demonstrated that a xenograft extra cellular matrix (ECM) scaffold implanted in the rat shoulder elicited a unique host response from that seen in the body wall. However, the host response to xenografts implanted in shoulders with a tendon/capsule injury was not different from xenografts implanted in shoulders with no injury. In the current study, w

[Rupture of larynx or trachea resulting from injuries sustained at birth].

Four newborn boys developed respiratory insufficiency and pneumothorax, pneumomediastinum or subcutaneous emphysema as the result of a laryngeal or tracheal rupture. These ruptures were due to birth injuries after difficult labour resulting from shoulder dystocia or a large lymphangioma and to a birth weight of at least 4500 g. The three children with shoulder dystocia also had a clavicular fracture, a Horner's syndrome, Erb paralysis or phrenic nerve paresis. Treatment consisted of surgical repair followed by a few days' intubation. The children with a shoulder dystocia recovered well, althou

Traumatic winging of the scapula.

Fourteen patients with traumatic winging of the scapula were reviewed, all of whom had had injuries producing sudden depression of the shoulder girdle from either a direct blow to the top of the shoulder or downward traction on the arm. The diagnosis was commonly missed for a considerable interval. Seven patients recovered spontaneously within six months of injury. Three of the other seven patients were treated by reattachment of the insertion of the sternal portion of the pectoralis major muscle via a fascia lata graft to the lower pole of the scapula. In one of these patients a reoperation w

[Functional comparison between arthroscopy and open surgery for rotator cuff injuries].

BACKGROUND: Rotator cuff injuries result in impaired functionality of the shoulder. The difference in shoulder functionality in patients treated with arthroscopy who required open surgery is still unknown. OBJECTIVE: To determine if there is a difference in the functional evolution of the shoulder in patients who underwent surgery due to rotator cuff lesion which began with arthroscopy and ended in open surgery. MATERIAL AND METHODS: A cohort, longitudinal, observational and comparative study was carried out. The Constant scale was applied to measure the functionality of the shoulder in pati

Aquatic Sports Injuries: Biomechanics, Injury Patterns, and Imaging in Swimming, Water Polo, and Diving.

Aquatic sports, such as swimming, water polo, diving, and artistic swimming, combine endurance, strength, and flexibility with unique biomechanical demands, often leading to sport-specific injuries. Although water is a low-impact environment, the repetitive and high-intensity movements required in these disciplines increase the risk of both acute and overuse injuries. Swimmers frequently encounter shoulder overuse syndromes, water polo players face injuries from throwing and physical contact, and divers are at risk for spinal and wrist injuries during entry impacts. Imaging modalities, particu

SLAP tears and return to sport and work: current concepts.

Superior labrum, anterior and posterior (SLAP) lesions are common and identified in up to 26% of shoulder arthroscopies, with the greatest risk factor appearing to be overhead sporting activities. Symptomatic patients are treated with physical therapy and activity modification. However, after the failure of non-operative measures or when activity modification is precluded by athletic demands, SLAP tears have been managed with debridement, repair, biceps tenodesis or biceps tenotomy. Recently, there have been noticeable trends in the operative management of SLAP lesions with older patients rece

'Physio's not going to repair a torn tendon': patient decision-making related to surgery for rotator cuff related shoulder pain.

BACKGROUND: Rotator cuff related shoulder pain (RCRSP) is a common and disabling shoulder condition and surgical management is becoming more common. The rates and costs of surgical interventions have been on the rise. Understanding decision-making related to surgery and providing adequate information to people with RCRSP may improve patient-centred care and potentially reduce rates of surgery. OBJECTIVES: To explore the decision-making processors of people who have undertaken surgery for RCRSP. DESIGN: An in-depth thematic analysis. METHOD: Interviews were conducted with patients from Melbo

Arthroscopic Bankart repair in the beachchair position: a cannulaless method using an intra-articular suture relay technique.

In shoulder arthroscopy, cannulas with large diameters (7-9 mm) are commonly used both in the lateral decubitus and the beachchair position because they ensure the easy introduction of instruments into the intra-articular space and provide a clear arthroscopic view through the application of high intra-articular pressure. On the other hand, fluid extravasation and the resulting inconvenience due to the narrowing of the intra-articular working space are also obvious issues. Consequently, this makes arthroscopic stabilization surgery all the more challenging. To avoid these shortcomings of this

Modified technique for arthroscopic Bankart repair using anchor sutures.

Bankart repair, or one of its modifications, is currently the gold standard procedure for treatment of anterior traumatic shoulder instability. It is now possible to perform the operation arthroscopically with the introduction of suture anchors. As described by Eugene Wolf, arthroscopic shoulder stabilization using the Mitek (Mitek Surgical Products, Ethicon, Edinburgh, U.K.) anchors requires two anterior portals and intra-articular knot tying. However, sliding the anchor on the inside limb of a suture loop could be challenging because the other limb could get tangled in the nitinol arc of the

Recent advances and perspectives on arthroscopic stabilization of the shoulder.

The goal of surgical stabilization of the unstable glenohumeral joint is to produce a stable yet mobile joint that can return to preinjury function. Open surgical procedures (in particular, the Bankart procedure) have evolved to a point where stability and mobility are being attained with a low complication rate, but the rate of return to preinjury function, particularly in overhead athletes, has not been ideal. Arthroscopic stabilization is an attractive alternative to open procedures, particularly in those patients who have a history of traumatic and unidirectional recurrent dislocations wit

An improved radiographic view of the glenohumeral joint.

In addition to the standard views of the shoulder (external rotation, internal rotation, and axillary), a 40-degree posterior oblique and a 60-degree anterior oblique projection were used to evaluate 105 consecutive shoulder examinations. Of the 32 abnormal shoulders evaluated, the diagnosis was established 97% of the time with a single 40-degree posterior oblique view, as opposed to a 78% rate with the three standard views. The 40-degree posterior oblique view of the shoulder is especially useful in evaluating the glenohumeral joint.

Static and Dynamic Stabilization of the Shoulder After Total Scapulectomy in an Electric Burn Patient.

An otherwise healthy 49-year-old man experienced a high-voltage electrical injury to the left shoulder resulting in total scapulectomy, partial calviculectomy, and a substantial soft tissue defect. The majority of the muscles around his shoulder were debrided because of necrosis, with only the pectoralis and latissimus dorsi muscles remaining attached to the humerus. Surprisingly, the patient's brachial plexus remained intact, and his left elbow, wrist, and hand function were preserved. A novel combination of 3 static and dynamic suspension techniques were used to stabilize his shoulder and pr

Bilateral luxatio erecta complicated by venous thrombosis.

Inferior shoulder dislocation or luxatio erecta is an exceedingly rare form of shoulder dislocation and compromises less than 0.5% of all shoulder dislocations. Furthermore, bilateral luxatio erecta is reported only nine times in the English literature. This paper documents the tenth case of bilateral luxatio erecta. This tenth patient suffered an axial load injury to his outstretched arms and displaced both humeral heads inferiorly. After closed reduction, the patient was discharged home on hospital day two. However, he developed an axillary vein thrombosis 3 days later and required anticoagu

The Effect of Booster Seat Use on Pediatric Injuries in Motor Vehicle Frontal Crashes.

BACKGROUND: Motor vehicle crashes are a significant source of pediatric mortality and morbidity. Studies indicate that booster seats significantly improve seat belt fit for children who have not attained a height of 145 cm (4' 9"). This study examined injuries occurring in booster age children up to age 12, as the majority of children do not attain 145 cm until this age. The purpose of the study was to identify differences in injuries due to the type of restraint used, with attention to musculoskeletal injuries. METHODS: Vehicle and occupant data were obtained from a publically available stat
