BACKGROUND: Osteoporotic fractures of the proximal humerus show an increasing incidence. Osteoporosis not only influences the fracture risk after low-energy trauma, but also affects the mechanical stability of internal fixation. Preoperative assessment of the local bone quality may be useful in the surgical treatment of patients sustaining these injuries. The aim of the present study was to present a method for the preoperative assessment of the local cancellous bone mineral density (BMD) of the proximal humerus using CT data.
METHODS: In the first part of the study, CT scans of 30 patients w

STUDY DESIGN: Nine patients with thoracolumbar burst fractures associated with horizontal fractures in the posterior column were studied retrospectively.
OBJECTIVES: To clarify the clinical and radiographic findings and evaluate the postoperative results for this type of fracture.
SUMMARY OF BACKGROUND DATA: It is not well recognized that burst fractures occasionally are accompanied by horizontal fractures of the posterior column such as those that result from seatbelt-type injuries. This type of burst fracture differs from flexion-distraction injuries combined with burst fracture, which are

Remote nailing of intertrochanteric and subtrochanteric fractures of the femur is a definite addition to the armamentarium in the management of these injuries. The advantages of the technique are too great to ignore. The surgeon should not be misled, however, into thinking that this is an easy operation or that it represents a panacea for difficult fracture situations. Considerable time must be spent learning the operative technique, its pitfalls and complications, and the postoperative management of patients. After the operation we allow our patients early weight bearing in the knee-extended

Postoperative infections after back operations can produce complex wounds with myonecrosis, deep dead space, and exposed orthopedic hardware, bone, and dura. Three ambulatory patients with complex postoperative back wounds that resulted from infections were treated successfully with antibiotics, debridement, irrigation, and closure of deep dead space with a superior gluteal muscle flap. Several surgical maneuvers can be performed to increase the length of the superior gluteal muscle flap. The inferior portion of the gluteus maximus was left intact to preserve gluteus maximus function. All thre

UNLABELLED: The incidence of trochanteric fractures is currently increasing. The introduction of new devices aims to secure the position of the reduction, and allow patients to mobilize early.
MATERIAL AND METHODS: 227 fractures in 223 patients were treated in our hospital with DHS between 1991 and 1993, and were followed up to fracture union. There were 87 men and 136 women; the average age was 77,4 years (21-101 years). Using Ender's classification 68,58 % of these fractures were type 1 or type 2.
RESULTS: The average operative time was 48 min; open reduction was only required for 54 fract

INTRODUCTION: Deep brain stimulation (DBS) is an effective treatment for multiple movement disorders and shows substantial promise for the treatment of some neuropsychiatric and other disorders of brain neurocircuitry. Optimal neuroanatomical lead position is a critical determinant of clinical outcomes in DBS surgery. Lead migration, defined as an unintended post-operative displacement of the DBS lead, has been previously reported. Despite several reports, however, there have been no systematic investigations of this issue. This study aimed to: 1) quantify the incidence of lead migration in a

BACKGROUND: Vertebral body tethering (VBT) is indicated for skeletally immature patients with progressive adolescent idiopathic scoliosis (AIS) who have failed or are intolerant of bracing and who have a major coronal curve of 40° to 65°. The vertebral body must be structurally and dimensionally adequate to accommodate screw fixation, as determined radiographically. The best indication for VBT is a flexible single major thoracic curve with nonstructural compensating lumbar and proximal thoracic curves (Lenke 1A or 1B). VBT allows for progressive correction of the deformity without spinal fusio

INTRODUCTION: Pectus excavatum (PE) is a congenital deformity essentially responsible for an unattractive aspect, much more rarely for compression problems. The classical treatments consist either in filling the excavation or in open thoracic reconstruction (the Ravitch technique). Alternatively, the treatment described by Nuss raises the sternum with a retrosternal metallic bar placed under thoracoscopic guidance. We present the preliminary results of a series of 25 children operated on using this technique.
HYPOTHESIS: The minimally invasive procedure described by Nuss is a valid surgical s

BACKGROUND: Chronic low back pain imposes substantial social and economic burdens due to its rising prevalence and associated disability. Intervertebral disc degeneration is the primary driver of discogenic back pain, though the mechanistic links between degenerative changes and persistent pain remain incompletely defined. Aberrant mechanical loading is recognised as a key trigger of disc degeneration. This work built a caudal spine compression rat model to explore how the P2Y2 receptor mediates load-induced disc degeneration and associated pain.
METHODS: Fifty-six eight-week-old male laborat

BACKGROUND AND OBJECTIVES: Atlantoaxial dislocation (AAD) is a high-risk condition that can lead to significant mortality. Lateral mass fusion offers structural stability and reduction, yet no standardized fusion device currently exists for consistent atlantoaxial fixation. This study compares the clinical outcomes of a novel anatomic cage vs iliac crest autograft in treating AAD.
METHODS: Radiographic, laboratory, and clinical outcomes were retrospectively compared between 2 surgical groups: one treated with an iliac crest autograft by a traditional posterior approach, the other treated with

BACKGROUND: In patients with valgus alignment and degenerative changes in the lateral compartment, both distal femoral osteotomy (DFO) and high tibial osteotomy (HTO) can be used to unload the lateral compartment. Prior studies have shown that in valgus knees, the tibial wear is posterior and DFO exerts the greatest effect in extension; however, its effect is decreased as flexion angle rises.
HYPOTHESIS: Medial closing-wedge (MCW) HTO would significantly decrease contact area, mean contact pressure (MCP), and peak contact pressure (PCP) in the lateral knee compartment through knee flexion to

OBJECTIVE: Intramedullary nails are commonly used for the purposes of internal fixation in the repair of tubular bone fractures, allowing for optimal fixation while minimizing intraoperative bleeding, surgical incision size, and associated stress. These nails are thus often utilized to repair hip and peripheral fractures, femur fractures, tibial fractures, humerus fractures, and clavicle fractures. Recent life expectancy increases, rising economic standards, and technological innovations have led to the application of a wider range of materials and techniques in the context of orthopedic devic

Introduction Spinal fusion surgery with pedicle screws is commonly performed to stabilize the spine of osteoporotic patients. However, securing a strong screw fixation in osteoporotic bone presents significant challenges due to the reduced bone density. This study aimed to compare the biomechanical performance in an osteoporotic bone model of pedicle screws inserted using two different techniques, the Jamshidi needle technique and the pedicle probe technique, as well as the influence of tapping on both these techniques. The research sought to determine if the surgical device used in aiding ped

BEST PRACTICE ADVICE 1: For all procedures, especially procedures carrying an increased risk for perforation, a thorough discussion between the endoscopist and the patient (preferably together with the patient's family) should include details of the procedural techniques and risks involved. BEST PRACTICE ADVICE 2: The area of perforation should be kept clean to prevent any spillage of gastrointestinal contents into the perforation by aspirating liquids and, if necessary, changing the patient position to bring the perforation into a non-dependent location while minimizing insufflation of carbon

OBJECT: The surgical approach and treatment of thoracolumbar osteoporotic vertebral collapse with neurological deficit have not been documented in detail. Anterior surgery provides good decompression and solid fusion, but the surgery-related risk is relatively higher than that associated with the posterior approach. In posterior surgery, the major problem after posterior correction and instrumentation is failure to support the anterior spinal column, leading to loss of correction of kyphosis. The aim of this study was to evaluate the efficacy of reinforcing short-segment posterior fixation wit
