Ultrasound Evaluatıon of Muscle Movement During Swallowing Maneuvers: Trials on 3 Maneuvers.

Ultrasound (US), a technology extensively utilized and deemed safe for medical purposes, represents an innovative approach that has recently been applied in the evaluation of swallowing functions as well as in the rehabilitation processes. Therapeutic interventions often prioritize swallowing maneuvers as preferred options. Nevertheless, the current body of literature indicates that the application of ultrasound in the assessment and treatment of swallowing disorders remains limited, with a scarcity of studies investigating muscle activation and laryngeal elevation during these swallowing mane

Sounding out Dysphagia: Using a Digital Stethoscope to Assess Audiometric Differences in Swallowing.

OBJECTIVES: A clinical swallow evaluation (CSE) is a subjective exam with limited diagnostic accuracy. We hypothesize that audiometric data collected superficially can enhance the detection of swallowing dysfunction. This study examines audiometric differences between normal and dysfunctional swallows on Flexible Endoscopic Evaluation of Swallowing (FEES) using a digital stethoscope. METHODS: In this single-center prospective study, adult patients undergoing FEES for dysphagia had a digital stethoscope placed on the left neck. Audiometric data were analyzed by consistency and compared to Pene…

Radiation Exposure During Contrast-Enhanced Videofluoroscopic Swallow Studies in Dogs and Cats.

Contrast-enhanced videofluoroscopic swallow studies are considered the gold standard diagnostic for determining underlying etiologies of dysphagia. Swallow studies are commonly performed procedures utilizing fluoroscopy in which personnel are exposed to ionizing radiation. The study objective was to document radiation exposure to individual personnel involved in performing gastrointestinal swallow studies in dogs and cats, compare radiation exposure based on the localization of disease, and evaluate radiation exposure based on the experience (i.e., year of training) of the diagnostic imaging r

Evaluation of Masticatory and Swallowing Functions Using Videoendoscopy and Observation of Facial and Mandibular Movements.

Background It is important to observe masticatory and swallowing movements to improve the eating habits of older adults with dysphagia. Observing facial and mandibular movements can be useful for such evaluations. However, the process models that occur while eating solid foods have not been adequately considered. Objective This study aimed to clarify the differences in facial and mandibular movements before and after bolus transport to the pharynx (stage II transport; STII) during the consumption of solid food. Methods As this was conducted as a pilot study, the participants included 20 health

Cerebral Infarction Triggered by Swallowing-Induced Compression of an Aberrant Right Subclavian Artery.

An 81-year-old man presented with recurrent cerebral infarctions despite the administration of dual antiplatelet therapy. Imaging revealed a severely stenotic, calcified, and aberrant right subclavian artery (ARSA) closely adjacent to the esophagus. Cerebral infarctions are repeatedly triggered by swallowing, and mechanical compression of the ARSA by food passage is considered to be a mechanism of cerebral infarction. Stenting of the ARSA and the left subclavian artery was performed to prevent any further events.

Recovery of Swallowing Function and Prognostic Factors Associated with Exacerbation of Post-stroke Dysphagia.

Post-stroke dysphagia is a common and debilitating complication affecting millions of people worldwide, often leading to malnutrition, pneumonia, and reduced quality of life. This study, an interim analysis of the Korean Study Cohort for Functional and Rehabilitation, aimed to identify long-term changes and predictive factors associated with post-stroke dysphagia at 3 years after stroke. A total of 4735 patients with acute first-ever stroke, including both ischemic and hemorrhagic subgroups, were followed, and dysphagia was assessed using the ASHA-NOMS scale. All the patients were then followe

Endoscopic Retrieval of Multiple Elongated Swallowed Foreign Bodies in a Patient With Psychiatric Illness.

Foreign body ingestion is common, with most passing uneventfully through the gastrointestinal tract. However, ingested long and rigid foreign bodies pose a significant challenge for endoscopic removal from the oesophagus and stomach. This case report describes a 19-year-old woman with a history of psychiatric illness who swallowed 12 elongated foreign objects, which were successfully retrieved via esophago-gastro-duodenoscopy (EGD). To our knowledge, this is one of the first documented case reports of multiple swallowed foreign bodies managed endoscopically without the need for surgical interv

Swallowing skill training approaches and underlying motor learning principles: A scoping review.

PURPOSE: While dysphagia rehabilitation has traditionally focused on muscle strengthening, there is an increasing shift towards harnessing motor cortical plasticity to modify swallowing motor output via swallowing skill training. We reviewed current swallowing skill training approaches and framed existing evidence in the context of motor learning principles. METHOD: Searches were conducted across six databases (Scopus, CINHAL, MEDLINE, PUBMED, Emcare, and Web of Science) and the grey literature. Two researchers independently screened studies for inclusion using Covidence. English publications

Effects of Pallidal Deep Brain Stimulation on Speech and Swallowing in Pediatric Patients with Dystonia.

BACKGROUND: Bilateral globus pallidus internus deep brain stimulation (GPi-DBS) is a proven safe and effective treatment in certain forms of idiopathic or inherited dystonia (ID/IN). Its effects in acquired dystonia such as in dyskinetic cerebral palsy (DCP) however vary widely. The impact of GPi-DBS on speech and swallowing, which significantly affect quality of life, remains poorly understood, especially in pediatric patients. OBJECTIVE: To evaluate GPi-DBS effects on speech and swallowing using the Frenchay Dysarthria Assessment 2 (FDA-2), in pediatric patients with dystonia, and assess ho

Therapeutic singing-induced swallowing exercise for dysphagia in advanced-stage Parkinson's disease.

BACKGROUND: With longer life spans and medical advancements, the rising number of patients with advanced-stage Parkinson's disease (PD) warrants attention. Current literature predominantly addresses dementia and fall management in these patients. However, exploring the impact of swallowing function on patients with advanced PD is crucial. Previous research has demonstrated notable enhancements in the quality of life related to voice for participants following a group singing-intervention program. To further elucidate the effect of individual singing-induced swallowing exercises, our study aime

Decisions on eating and drinking in older adults admitted with pneumonia and referred for swallowing difficulties.

PURPOSE: Older patients with pneumonia are commonly restricted from oral intake due to concerns towards aspiration. Eating and drinking with acknowledged risks (EDAR) is a shared decision-making process emphasising patient comfort. As part of our project to find the barriers and facilitators of EDAR, we aimed for this initial study to see how frequently EDAR was selected in practice. METHODS: We performed a retrospective cohort study at an acute hospital where EDAR was initially developed, of patients aged ≥ 75 years-old admitted with pneumonia and referred to speech and language therapy. Presence of postlaryngectomy pseudodiverticulum on barium swallow does not affect early dietary progression.

BACKGROUND: The presence of a pseudodiverticulum of the anterior pharyngeal wall, or prominent "pharyngeal bar," is a well-known phenomenon that occurs following total laryngectomy, which can be visualized by nasolaryngoscopy or videofluoroscopy. Among the different techniques of pharyngeal reconstruction, there is higher incidence following primary vertical multilayered closure. It has been postulated to cause dysphagia and lack of dietary progression despite a paucity of data. However, the direct impact of pseudodiverticulum is less clear and anecdotally its presence and severity does not ne
