Thoracoscopic sympathectomy for palmaris hyperhidrosis.

INTRODUCTION: Palmaris hyperhidrosis is a disorder mediated by the sympathetic nervous system. It causes excessive sweating. This study evaluated the safety, efficacy, and outcome after thoracoscopic sympathectomy in patients with palmaris hyperhidrosis. METHODS: We reviewed the medical records of 18 patients (10 male) who underwent bilateral thoracoscopic sympathectomy between July 1998 and June 2001. RESULTS: The patients' mean age was 34 years. No conversions to thoracotomy occurred. Three 2- to 5 mm trocars were used. The thoracic sympathetic chain was resected from ganglia T2-T4, except…

Endoscopic transthoracic sympathectomy for idiopathic upper limb hyperhidrosis.

A 5-year experience of 51 endoscopic transthoracic dorsal sympathectomies for idiopathic palmar hyperhidrosis in 26 patients is presented. Fifty-two percent complained of excessive sweating over their hands, 28% of axillary sweating and 20% over both areas, with a mean duration of 10 years. The second, third and fourth thoracic ganglia and their interconnecting fibres on the affected side were ablated using diathermy cautery. Over a mean follow up time of 26 months, this procedure was successful in curing or improving intractable sweating in 92%. However, axillary sweating was less well contro…

Case Report: Acquired Generalized Anhidrosis Caused by Brain Tumor: Review of the Literature.

PURPOSE: There has been limited focus on sweating failure in patients with brain tumor. We report two patients with generalized anhidrosis caused by germinoma. We also review previous reports of generalized anhidrosis due to brain tumor. CASE REPORTS: Patient 1 was a 12-year-old boy with repetitive heat shock-like episodes even in winter. Based on Minor's test, he was diagnosed with generalized anhidrosis. Magnetic resonance imaging (MRI) revealed the absence of high signal intensity of the posterior pituitary. He was initially diagnosed with central diabetes insipidus. However, an MRI scan p…

Special considerations for children with hyperhidrosis.

Hyperhidrosis, characterized by excess sweat production, affects children and adults. Primary focal hyperhidrosis affects any anatomic region with sweat appendages present. Primary hyperhidrosis has traditionally been considered a problem for adults, but approximately 1.6% of adolescents and 0.6% of prepubertal children are affected. Psychological and social development and well-being are often affected, leading to profound emotional and social distress. Quality of life can be improved by early diagnosis and therapy; however, underdiagnosis and lack of knowledge regarding therapeutic options h…

[Focal hyperhidrosis: disease characteristics and treatments].

Hyperhidrosis is a disease defined by the eccrine sweat glands production exceeding the needs of thermoregulation. In most of the cases there is no underlying disease and hyperhidrosis is characterised by an intense sweating or flow in the axillae or palms. Hyperhidrosis results in an important psychologic effect that can lead to a major socio-professional impairement. In the severe forms of hyperhidrosis, conservative treatments often are disappointing. Botulinum toxin is a successful treatment used in hyperhidrosis for more than ten years. It is an important therapeutic discovery that leads …

[Inducibility of olfactory sensitivity by odor exposure of persons with specific anosmia].

Insensitivity to single odors, called specific anosmia, has been repeatedly reported in the literature. The main question of the present study was whether olfactory sensitivity is inducable in subjects with specific anosmia. For this reason the olfactory sensitivity of women with specific anosmia to the volatile steroid androstenone was investigated by threshold measurements at two times, before and after repeated odor exposure. Androstenone is a compound that contributes to human body odor and is found at a higher concentration in male axillary sweat than in female sweat. The results show tha…

Artificial feeding of Amblyomma cajennense (Fabricius, 1787) (Acari:Ixodidae) through silicone membrane.

An artificial feeding system was used where citrated bovine blood was offered to male and female Amblyomma cajennense. Vestiges of blood, sweat, hair and exfoliated skin were used as phago-stimulants placed on the surface of the silicone membrane. The ticks were collected, as engorged nymphs, from naturally infested equines, with the ecdysis occurring in the laboratory. Four hundred ticks were used, 50% being female, at three to four weeks post-ecdysis. Vestiges of blood on the silicone membrane were the most efficient phago-stimulant and the association of vestiges of blood and sweat residue …

X-linked anhidrotic (hypohidrotic) ectodermal dysplasia is caused by mutation in a novel transmembrane protein.

Ectodermal dysplasias comprise over 150 syndromes of unknown pathogenesis. X-linked anhidrotic ectodermal dysplasia (EDA) is characterized by abnormal hair, teeth and sweat glands. We now describe the positional cloning of the gene mutated in EDA. Two exons, separated by a 200-kilobase intron, encode a predicted 135-residue transmembrane protein. The gene is disrupted in six patients with X;autosome translocations or submicroscopic deletions; nine patients had point mutations. The gene is expressed in keratinocytes, hair follicles, and sweat glands, and in other adult and fetal tissues. The pr…

Congenital bilateral absence of vas deferens in the absence of cystic fibrosis.

The high frequency of mutations in the cystic fibrosis gene in patients with congenital bilateral absence of vas deferens (CBAVD) has raised the question whether all of them have a genital form of cystic fibrosis. We investigated 47 CBAVD patients by ultrasonography, 10 (21%) had renal malformations and 37 (79%) did not. In the former group, no cystic fibrosis mutations were found and sweat chloride concentrations were normal. In the latter group, 18 patients (49%) carried at least one cystic fibrosis mutation and sweat chloride was high in 17 of 26 tested (65%). Our findings suggest that CBAV…

Patient's estimation of efficacy of various hyperhidrosis treatments in a dermatological clinic.

Hyperhidrosis is a common ailment, which is accompanied by a low quality of life. There are many current treatment options. The aims of this study were to investigate: (i) the characteristics of sweating in patients referred for hyperhidrosis; (ii) their quality of life, using the Dermatology Life Quality Index and the Hyperhidrosis Disease Severity Scale; and (iii) their estimation of efficacy of the treatments given, using a patient's global assessment score and the quality of life scores. In 47 of 94 patients (50%) responding to the questionnaires, hyperhidrosis was confined exclusively to …

Control of palmar hyperhidrosis with a new "dry-type" iontophoretic device.

BACKGROUND: Palmar hyperhidrosis is characterized by excessive sweating on the palm, and among the various treatment modalities, tap water iontophoresis has been widely used. OBJECTIVE: The objective of this study was to assess the effect of a new "dry-type" iontophoretic device that was locally manufactured and did not use tap water to control sweating. METHODS: Ten subjects with palmar hyperhidrosis were enrolled in this study. To be treated the patients were instructed that they only have to grasp the device. Only one palm was treated for 2 weeks, and then the treatment was discontinued t…

Sympathotomy instead of sympathectomy for palmar hyperhidrosis: minimizing postoperative compensatory hyperhidrosis.

OBJECTIVE: To determine whether anatomical modification of sympathectomy diminishes severe postoperative compensatory hyperhidrosis. PATIENTS AND METHODS: From January 1, 2000, to June 1, 2001, we prospectively studied 10 consecutive patients with primary palmar hyperhidrosis, aged 14 to 42 years. Medical therapy had failed in all patients. A preoperative and postoperative evaluation and thermoregulatory sweat testing were conducted in all patients. All patients underwent sympathotomy (bilateral simple disconnection) of the second thoracic ganglion input into the brachial plexus, and no sympa…

An artifact-resilient multimodal bioelectronic patch for monitoring biochemical and motor states in Parkinson's disease.

Personalized management of Parkinson's disease (PD) requires continuous tracking of motor fluctuations and medication-associated biochemical dynamics during daily living. Here, we report the Biochemical and Inertial Multimodal Assessment Patch (BIMAP), a fully integrated multimodal bioelectronic patch that combines multiplexed sweat biosensing of levodopa (L-Dopa), glucose (Glu), and ascorbic acid (AA) with triaxial accelerometry for biochemical-kinematic monitoring of PD-related states. A microfluidic sampling interface is incorporated to stabilize sweat transport and maintain sensor-fluid co…
