Congenital flexion deformities of the thumb.

A congenital flexion deformity of the IP joint of the thumb is usually due to a trigger finger. The diagnosis is based on the thumb being no … tly respond to cast immobilization in the very young but can otherwise be treated by an appropriate tendon transfer to re-institute active extension. Complex clasped thumb deformities require elucidation of the specific pathologic features of the deformity, based both on clinical evaluation and surgical exploration. These digits will require not only an extensive …

Transposition of the posterior tibial tendon in spastic equinovarus.

Eleven posterior tibial tendon transfers were performed in eight patients with spastic equinovarus with an average follow-up of 3 years (range 1 1/2-6 1/2 years). The main goals--to eliminate the need for braces and to improve gait--were achieved in all patients. An adequate operation technique kept the well-known postoperative complications, e.g., valgus and calcaneal deformity, to a minimum.

Retrofibular transfer of tibialis posterior to control residual adduction deformity of clubfoot.

The tibialis posterior muscle was transferred behind the fibula through the peroneal muscles and anchored in the fourth intermetatarsal cleft as an adjuvant step in postero-medial release to control the adduction deformity of the forefoot in clubfeet. The results of this procedure were compared after five years with conventional methods, including postero-medial release alone, in ten identical bilateral clubfeet. The immediate and late correction of the adductus deformity were significantly improved by this additional operation.

In post-traumatic brachial plexus lesions in adults, early repair will necessitate a variety of nerve grafting and nerve transfer procedures … reanimate the shoulder. In C5 C6 and C7 palsies, extension of the wrist and fingers is provided by tendon transfers. In chronic palsies, elbow flexion and extension loss is treated by means of free muscle transfers, (latissimus dorsi or gracilis) combined with nerve transfers (intercostals or spinal accessory). Secondary procedures are routinely necessary follow …

Fungating malignant peripheral nerve sheath tumor arising from a slow-growing mass in the forearm: a case report and review of the literature.

BACKGROUND: Malignant peripheral nerve sheath tumor is a rare and aggressive form of sarcoma that arises from a peripheral nerve, mostly in association with neurofibromatosis type 1. Half of the cases were reported in the extremities, with the lungs being the most common site of metastasis. We report a rare case of successful limb salvage surgery performed for a large exophytic malignant peripheral nerve sheath tumor of the right forearm with wide surgical margins followed by split-thickness skin graft and later a flexor carpi radialis tendon transfer to extensor digitorum communis tendon. CA…

Minimally invasive repair of the tibialis anterior tendon using a semitendinosus autograft.

BACKGROUND: Ruptures of the tibialis anterior tendon are rare but can cause substantial functional deficiencies. The literature regarding the treatment of such injuries is very limited. Atraumatic ruptures often occur in the presence of an abnormal tendon structure, and retraction often makes end-to-end repair impossible. With traumatic lesions, the risk of developing both adhesions and scar tissue is high. METHODS: This study assesses the results of a surgical reconstruction using the interposition of a semitendinosus tendon autograft in 12 patients. Three patients had a traumatic rupture, a

Closed tendon rupture as a result of Kienböck disease.

Closed tendon rupture is a well-known complication of Kienböck disease, but only 11 cases have been reported. We reviewed six cases of Kienböck disease with subcutaneous rupture of a tendon. There were five cases of 4th extensor tendons and one case of the flexor digitorum profundus tendons of the ring finger and little finger. Radiographs showed protrusion of the segmented lunate or the deformed lunate in all cases. Intraoperative findings confirmed rupture of the wrist joint capsule by these lunate lesions. We then reviewed 11 reported cases in English and 48 cases in Japanese and confirmed

Operative management of distal tibialis anterior tendinopathy.

BACKGROUND: The results of operative management for distal tibialis anterior tendinopathy (DTAT) without rupture have not previously been described in the literature. We present the results of 12 operative procedures. MATERIALS AND METHODS: Of 40 patients diagnosed clinically and radiographically with DTAT, we reviewed 11 patients who underwent surgery for failure of nonoperative management. Assessment included pre and postoperative AOFAS midfoot scores, clinical examination and postoperative VAS pain scoring, at a mean followup of 29 months. RESULTS: Ten women (ten feet) and one man (2 feet

Autogenic heterotopic vascularized proximal interphalangeal joint transplantation in children.

The proximal interphalangeal joint (PIP) joint is the most crucial joint for the functionality of a finger. For a child with complex injury of the hand every effort should be exercised to maximize function restoration. If the PIP joint is irreparably damaged, its reconstruction is indicated. The technique of autogenic heterotopic vascularized toe joint transplantation provides unique advantage of a composite transfer of skin, tendons, bone and joint alone with growth plate and its efficacy has been affirmed in children. It has been suggested that such transfers require intact flexor tendon to

Shoulder reconstruction in patients with chronic brachial plexus birth palsy. A case control study.

Patients with chronic brachial plexus birth palsy and persistent peripheral neurologic deficits frequently have problems related to their shoulder. Specifically, internal rotation and adduction contractures develop because of the loss of muscle balance about the glenohumeral joint. With time, progressive and predictable deformity of the glenohumeral joint occurs. The authors reviewed their results in treating patients with persistent functional deficits with either soft tissue procedures (tendon transfers and muscle releases) or rotational humeral osteotomies based on criteria incorporating pa…

Adductor magnus muscle transfer to restore knee extension: Anatomical studies and clinical applications.

BACKGROUND: Loss of knee extension causes significant impairment. Though nerve-based reconstruction is preferable in cases of femoral nerve palsy or injury, these surgeries are not always appropriate if the pathology involves the quadriceps muscles or presentation too late for muscle reinnervation. Muscle transfers are another option that has been underutilized in the lower extremity. We describe the successful restoration of knee extension by adductor magnus muscle transfer without functional donor morbidity, along with anatomical considerations. METHODS: Ten fresh frozen cadaveric lower lim

Parsing Through the Data on Achilles Tendon Rupture Management, Rehab and Sports Return Criteria: a Current Literature Review.

PURPOSE: To evaluate and discuss controversies that have arisen as management of acute and chronic Achilles tendon ruptures continues to evo … ntion after injury shows improved functional outcomes. For chronic ruptures, flexor hallucis longus tendon transfer is the workhorse of secondary reconstruction. Early functional rehabilitation has been shown to be safe and effective across treatment strategies. The management of Achilles tendon injuries remains a complex clinical challenge that demands a compreh …

Editorial Commentary: Endoscopic and Open Techniques Can Achieve Successful Outcomes With Hip Abductor Tendon Repair but Retracted Tears With High-Grade Fatty Atrophy May Benefit From an Open Approach.

The gluteus medius and minimus tendon unit are commonly referred to as the "rotator cuff of the hip." Tears of the gluteus medius and/or minimus can range from interstitial, partial-thickness tears to retracted, full-thickness tears, and may result from trauma but are more commonly associated with chronic degeneration. Patients commonly present with lateral hip pain exacerbated by weight-bearing and sleeping on the affected side. In patients with persistent lateral hip pain, despite nonoperative management, both open and endoscopic gluteal tendon repair techniques have demonstrated excellent s
