Pickle Juice Intervention for Cirrhotic Cramps Reduction: The PICCLES Randomized Controlled Trial.

INTRODUCTION: Muscle cramps are common among persons with cirrhosis and associated with poor health-related quality of life. Treatment options are limited. We sought to determine whether pickle juice can improve muscle cramp severity. METHODS: We enrolled 82 patients with cirrhosis and a history of >4 muscle cramps in the previous month from December 2020 to December 2021. Patients were randomized 1:1 to sips of pickle juice vs tap water at cramp onset. Our primary outcome assessed at 28 days was the change in cramp severity measured by the visual analog scale for cramps (VAS-cramps, scaled 0

Maintaining Continuity of Care for Expectant Mothers in Kenya During the COVID-19 Pandemic: A Study of MomCare.

In Kenya, early coronavirus disease (COVID-19) modeling studies predicted that disruptions in antenatal care and hospital services could increase indirect maternal and neonatal deaths and stillbirths. As the Kenyan government enforced lockdowns and a curfew, many mothers-to-be were unable to safely reach hospital facilities, especially at night. Fear of contracting COVID-19, increasing costs of accessing care, stigma, and falling incomes forced many expectant mothers to give birth at home. MomCare, which primarily serves communities in remote areas and urban slums, links mothers-to-be with pay

HCV hotline facilitates Hepatitis C elimination during the COVID-19 pandemic.

The COVID-19 pandemic necessitates healthcare restrictions that also affected ongoing hepatitis C virus (HCV) elimination efforts. We assessed the value of a physician-operated HCV hotline on treatment and cure rates throughout the pandemic. All HCV patients undergoing HCV therapy at the Vienna General Hospital from 2019 to 2021 were included. An HCV hotline was established in 2019 and provided services including phone calls, text messages and voicemails. Patients were stratified by date of HCV therapy: 2019 (pre-COVID) vs. 2020/2021 (during-COVID) and use of the HCV hotline: users vs. non-use

Macrolide-Resistant Mycoplasma genitalium Impairs Clinical Improvement of Male Urethritis After Empirical Treatment.

BACKGROUND: Mycoplasma genitalium (MG) is associated with urethritis in men and could play a role in clinical outcome. We examined clinical improvement of symptoms in men receiving empirical treatment for urethritis and correlated the outcome with Neisseria gonorrhoeae (NG), Chlamydia trachomatis (CT), MG, and MG macrolide resistance-associated mutations (MRAM) status. METHODS: At the sexually transmitted infection clinic in Amsterdam, the Netherlands, empirical treatment for gonococcal urethritis is 1 g ceftriaxone and for nongonococcal urethritis 1 g azithromycin. In 2018 to 2019, we tested

Bluetooth-Enabled Implantable Cardiac Monitors and Two-Way Smartphone Communication for Patients With Hypertrophic Cardiomyopathy.

BACKGROUND: Sudden cardiac death (SCD) risk stratification in hypertrophic cardiomyopathy (HCM) currently relies on arrhythmic burden quantification by 24 or 48-hour Holter monitoring. Whether this approach adequately captures arrhythmic burden, compared with longer-term continuous monitoring, is unclear. We sought to assess the long-term incidence of nonsustained ventricular tachycardia (NSVT) in HCM patients at low or moderate SCD risk, using implantable cardiac monitors (ICMs) paired with a novel Bluetooth-enabled 2-way communication platform. METHODS: This prospective, single-arm, observa

Exploring factors affecting quality implementation of lymphatic filariasis mass drug administration in Bole and Central Gonja Districts in Northern Ghana.

Ghana has been implementing Mass Drug Administration (MDA) since the year 2001, and Lymphatic Filariasis transmission has been interrupted in 76 out of the 98 targeted districts. The remaining districts have a microfilaria prevalence above the 1% threshold needed for the interruption of transmission. This study assesses the level of lymphatic filariasis MDA coverage and explored factors affecting the quality of implementation of the MDA in the Bole and Central Gonja Districts of Northern Ghana. A concurrent mixed methods study design approach was used to provide both a quantitative and qualita

Management of chlamydia and gonorrhoea infections diagnosed in primary care using a centralised nurse-led telephone-based service: mixed methods evaluation.

BACKGROUND: Up to 18% of genital Chlamydia infections and 9% of Gonorrhoea infections in England are diagnosed in Primary Care. Evidence suggests that a substantial proportion of these cases are not managed appropriately in line with national guidelines. With the increase in sexually transmitted infections and the emergence of antimicrobial resistance, their timely and appropriate treatment is a priority. We investigated feasibility and acceptability of extending the National Chlamydia Screening Programme's centralised, nurse-led, telephone management (NLTM) as an option for management of all

Conservative care with or without manipulative therapy in the management of back and/or neck pain in Danish children aged 9-15: a randomised controlled trial nested in a school-based cohort.

BACKGROUND: A substantial number of children experience spinal pain, that is, back and/or neck pain. Today, no 'gold-standard' treatment for spinal pain in children exists, but manipulative therapy is increasingly being used in spite of a lack of evidence of its effectiveness. This study investigates the effectiveness of adding manipulative therapy to other conservative care for spinal pain in a school-based cohort of Danish children aged 9-15 years. METHODS AND FINDINGS: The design was a two-arm pragmatic randomised controlled trial, nested in a longitudinal open cohort study in Danish publi

Applying a mobile survey tool for assessing lymphatic filariasis morbidity in Mtwara Municipal Council of Tanzania.

BACKGROUND: A number of methods have been used to estimate lymphatic filariasis (LF) morbidity, including: routine programmatic data, cluster random surveys and the "town crier" method. Currently, few accurate data exist on the global LF morbidity burden in Tanzania. We aimed to estimate prevalence of lymphedema and hydrocele in Mtwara Municipal Council using mobile phone based survey. METHODS: A cross-sectional survey was conducted among adults of Mtwara Municipal council with access to mobile phones. A sample size of at least 384 completed surveys was required to estimate prevalence of lymp

Parental expectations for specific follow-up after pediatric intensive care stay: a prospective observational cohort study.

UNLABELLED: To assess parental expectations regarding specific follow-up after a pediatric intensive care unit (PICU) stay, and whether these expectations differ according to pediatric post-intensive care syndrome (PICS-p) risk factors. We also explored differences across four patient profiles based on age, pre-existing conditions, and progression toward chronic illness. This study was a questionnaire-based, prospective observational cohort study within the PICU of a French teaching hospital. The study population consisted of all children discharged alive from the PICU between July 2023 and Ju

Waterpipe use among people who smoke in rural Vietnam: Secondary analysis from the M2Q2 randomized clinical trial.

INTRODUCTION: Waterpipe (WTP) tobacco smoking is highly prevalent in Vietnam. WTP use is associated with higher nicotine exposure and increased risk of pulmonary and cardiovascular diseases. The aim of this study is to understand WTP use and how it may influence smoking behaviors and cessation. METHODS: We conducted a secondary analysis of data from the mHealth Messaging to Motivate Quitline Use and Quitting (M2Q2) randomized trial, which evaluated a mobile health peer-texting intervention in Northern Vietnam. Logistic and mixed-effects logistic regression was used to assess the association b

Alcohol Attentional Bias Predicts Same-Day Desire to Get Drunk and Alcohol Consumption in Non-Treatment Seeking Young Adults Identified With Hazardous Alcohol Use in the Emergency Department.

BACKGROUND: Attentional bias (AB) toward alcohol cues is theorized to reflect automatic motivational processes that contribute to heavy drinking, but ecologically valid, real-time assessments of how these biases manifest in daily life remain limited. This study used ecological momentary assessment (EMA) and weekly mobile Stroop tasks to test whether daily AB predicts same-day desire to get drunk and alcohol use among non-treatment-seeking young adults with hazardous drinking. METHODS: This secondary analysis used data from young adults (N = 579; aged 18-25) with hazardous drinking recruited f

Social Network Determinants of Cigarette Forgoing and Quit-Related Conversations in a 15-Day Ecological Momentary Assessment Study in Mexico and the US.

INTRODUCTION: It is unclear why people of Mexican ancestry who smoke report relatively high rates of discussing quitting-related topics, which predicts cessation attempts. METHODS: Using Ecological Momentary Assessment (EMA), adults from Mexico (n=40) and with Mexican heritage living in the US (n=52) who smoked daily were sent texts/emails each morning with cessation tips and cessation benefits over 15 days. At baseline, participants reported their personal network characteristics across up to eight different alters. Each evening of the 15 days, participants reported forgoing any cigarettes t
