Assessment and management of snoring: a surgical perspective.

This article reviews the assessment and treatment options for both 'simple snorers' and those with obstructive sleep apnoea. Accurate assessment of all snorers is essential for optimal outcome and new surgical techniques now provide good symptom relief while maintaining normal palatal anatomy. The need for involvement of both respiratory physicians and otolaryngologists in the patient's care is emphasised.

[The uvulopalatopharyngoplasty operation for obstructive sleep apnea syndrome].

We performed uvulopalatopharyngoplasty operations on 216 patients and conducted 6-month to 2-year follow-ups. 40 patients have had polysomnograms before and after operations. The clinical results showed 50 percent markedly improved, 28 percent somewhat improved, and 22 percent unimproved. The total success rate was 78%. The surgical procedures and polysomnogram methods have been introduced, and the pathogenesis, diagnostic criteria and results were discussed.

Manometric and endoscopic localization of airway obstruction after uvulopalatopharyngoplasty.

The most widely reported surgical procedure for obstructive sleep apnea syndrome is uvulopalatopharyngoplasty. The success rate for this procedure is variable, and the reason for failure is incompletely understood. Failure in some patients is postulated to result from tongue-base obstruction. To investigate this, we identified the level of collapse and obstruction in 11 cases of uvulopalatopharyngoplasty failure, using upper airway manometry and videoendoscopy, while patients slept. Airway manometry measured the initial level of complete obstruction. Videoendoscopy identified significant resti

Changes in voice characteristics after uvulopalatopharyngoplasty.

Previous reports have warned that tonsillectomy or uvulopalatopharyngoplasty (UPPP) may alter patients' speech by increasing the amount of nasal resonance as well as by changing voice timbre due to enlargement of the vocal tract. However, very few objective investigations, excluding nasality problems, have been carried out. We studied eight patients who underwent surgery for UPPP and recorded pre- and postoperative speech. The speech samples were then rated by seven experienced listeners, and acoustic spectra of two long vowels,/a/ and /e/, were analyzed using a computer program (MacSpeech Lab

Palatal inclusion cyst: an unusual complication of uvulopalatopharyngoplasty.

Inclusion cyst formation after uvulopalatopharyngoplasty has not been previously reported. We describe a case of postoperative inclusion cyst in a 62-year-old man who presented with recurrent obstructive sleep apnea symptoms. Simple transoral excision of the cyst was curative. Careful eversion of the mucosal edges at the time of the original palatal surgery may have prevented this problem from occurring.

Acoustic characteristics of speech following uvulopalatopharyngoplasty.

Selective uvulopalatopharyngoplasty has resulted in the improved management of sleep apnea; however, the effect of the surgery on speech has not been systematically evaluated. In this study, the speaking fundamental frequency, reading rate, and first and second formants of the vowels of four patients were measured acoustically presurgically and postsurgically. The results indicate that the length of time needed to read a passage was reduced in all four patients 2 weeks following surgery compared to the presurgical duration. In addition, the second formant of nine vowels studied was lower posts

[Effects after 3 months of uvulopalatopharyngoplasty in the treatment of obstructive sleep apnea syndromes in adults].

The results at three months of uvulopalatopharyngoplasty (UPPP) have been evaluated in thirty adult patients with an obstructive sleep apnoea syndrome (SAOS). For the group overall the mean apnoea index (IA) decreased from 57 apnoeas per hour and the mean maximal desaturation decreased to 60% post operatively. However in the overall results different individual facts overlap. 20 of the 30 patients, or 67%, have an IA post operatively of less than 50% of the pre-operative value (responders). 14 of these or 47% have a post operative IA of less than 10 apnoeas per hour, a value considered as non

Pneumothorax as a complication of tracheoesophageal voice prosthesis use.

A previously unreported association of pneumothorax and the use of a tracheoesophageal voice prosthesis is presented. A review of 44 cases revealed that two patients developed a pneumothorax requiring chest tube placement. Pulmonary barotrauma is believed to be the cause of the injury. In those patients in whom the tracheoesophageal voice prosthesis has been effective, it has produced excellent results. However, certain pulmonary conditions may be considered a relative contraindication to the use of the tracheoesophageal voice prosthesis in patients who must generate significant force to produ

Snoring: not funny--not hopeless.

Snoring means obstructive breathing during sleep, and its most exaggerated form is obstructive sleep apnea. Mild snoring may respond to simple self-help remedies. Heavy snoring responds well to the surgical removal or reconstruction of obstructive elements in the hypopharynx, nasopharynx or nasal passages.

Evaluation of the effectiveness of uvulopalatopharyngoplasty.

A new surgical procedure to treat obstructive sleep apnea by uvulopalatopharyngoplasty (UPPP) was evaluated in 66 patients, 63 men and 3 women, with objectively documented sleep apnea syndrome. Removal of redundant tissue in the oropharynx (UPPP) significantly improved excessive daytime sleepiness, reduced by half the frequency of apneas and hypoxia occurring during sleep, and improved the quality of sleep. Closer analysis indicated that all 66 patients did not benefit to the same degree. Among patients classified as responders, the frequency of apnea was reduced to a level seen in healthy adu

[Polysomnographic follow-up of sleep apneas after uvulopalatopharyngoplasty].

Uvulopalatopharyngoplasty (UPPP) was performed in 12 patients with obstructive sleep apnea. Pre-surgical clinical and polysomnographic data were compared with those one to three months after surgery. Excessive daytime sleepiness was partially improved in 4 cases and completely abolished in 8. Total sleep time, sleep efficiency, number of arousals and sleep latency were similar in both evaluations. REM latency to stage 2 markedly increased on the follow-up. Stage distributions were similar in both nights except for stage 4 which was more frequently absent on the first recording. SaO2 levels bel

Complications of the 'treatment' of tonsillar infection by traditional healers in Nigeria.

Traditional healers in Nigeria attempt to treat tonsillar infections either by performing uvulectomy with a sickle knife or by digital rupture of the tonsil. These procedures result in various complications, including haemorrhage, septicaemia, cellulitis of the neck, peritonsillar and parapharyngeal abscess, upper airway obstruction, and pharyngo-laryngocele with pneumothorax. These complications are illustrated with case reports. The danger of encouraging traditional healers to practise unsupervised and unscientific medicine is stressed.
