A critical re-examination of the economics of blindness prevention under the Onchocerciasis Control Programme.

This article is a critical re-examination of the recent cost-effectiveness analysis of the Onchocerciasis Control Programme (OCP) in West Africa, undertaken by Prost and Prescott in 1984. We use the same approach, namely, measuring effectiveness of the programme by the number of healthy years of life added by the prevention of blindness. This work focuses on certain technical aspects of the data used for estimating parameter values in the cost-effectiveness calculations. Through examination of available data and the relationships between certain key variables, we estimate values that differ su

The Efficacy of Combined Medication With Methylprednisolone and Erythropoietin in the Treatment of Ischemia-Reperfusion Injury to the Spinal Cord in Patients With Cervical Spondylotic Myelopathy.

Introduction Cervical myelopathy (CM) is caused by degenerative or congenital changes in the discs and soft tissues of the cervical spine, leading to chronic compression of the spinal cord. The current treatment for moderate-to-severe CM is surgical decompression, which is effective in most cases; however, it can cause inflammation of the nervous system and spinal cord reperfusion injury, resulting in perioperative neurological complications and suboptimal neurological recovery. The aim of this study was to investigate the therapeutic effects of the combination of erythropoietin and methylpred

[Effect of diazepam and barbiturates on the in vivo metabolism of halothane and enflurane in rats].

The effect of pretreatment with barbiturates or diazepam on the in vivo metabolism of halothane or enflurane (92 ppm) was investigated upon application by inhalation. In male rats exposed to halothane the elimination half-life from the atmosphere of the closed exposure system amounted to 0.64 h; pretreatment with thiopental or methohexital (40 mg/kg i.p.) caused a small, but significant prolongation of the half-life (1.3 or 1.4-fold), whereas hexobarbital (75 or 100 mg/kg i.p.) had no effect in this respect. Much stronger and dose-dependent effects were seen in rats pretreated with pentobarbit

Pain management in sickle cell disease: palliative care begins at birth?

People with sickle cell disease (SCD) are living longer, but their lives are impacted even more by the unpredictable intermittent or constant pain that is often poorly managed over a lifetime. To address this problem, an interdisciplinary team approach is needed that brings the medical professionals together for optimal compassionate care that is coordinated from the beginning of life and throughout the patient's lifespan. The hematologist, whenever possible, should take the lead. Effective models that have been developed to address SCD pain, such as the Day Hospital, The Recidivism Project and

Patient-reported outcome measures applied in chronic diseases relevant to asthma remission: a scoping review.

BACKGROUND: Asthma remission is a meaningful treatment goal that requires patient and healthcare provider agreement. However, there is currently no validated way to incorporate patient perspectives of remission in this process. Patient-reported outcome measures (PROMs) can allow comprehensive assessment of remission by capturing the impact on individual patient experiences. The aim of this study was to identify PROMs used to define remission in other chronic diseases, thereby informing development of a PROM for asthma remission. METHODS: A scoping review of primary research studies was conduc

Myocardial Perfusion PET for the Detection and Reporting of Coronary Microvascular Dysfunction: A JACC: Cardiovascular Imaging Expert Panel Statement.

Angina pectoris and dyspnea in patients with normal or nonobstructive coronary vessels remains a diagnostic challenge. Invasive coronary angiography may identify up to 60% of patients with nonobstructive coronary artery disease (CAD), of whom nearly two-thirds may, in fact, have coronary microvascular dysfunction (CMD) that may account for their symptoms. Positron emission tomography (PET) determined absolute quantitative myocardial blood flow (MBF) at rest and during hyperemic vasodilation with subsequent derivation of myocardial flow reserve (MFR) affords the noninvasive detection and deline

Peak nasal inspiratory flow: a useful and handy tool for the diagnosis of nasal obstruction in the elderly.

The global population is aging, with the over-65 age group expected to double in the USA by 2030. Three subcategories of elderly people have been identified: "young old" (from 65 to 74 years), "older old" (from 75 to 84) and "oldest old" (85 or more). Rhinosinusitis is a common disease that affects more than 31 million people in the USA every year. Nasal obstruction is one of the most common symptoms in patients affected by rhinosinusitis. An accurate nasal obstruction evaluation in the elderly is becoming of increasing interest for medical doctors, especially for geriatricians. Peak nasal ins
