Repetitive ocular vestibular evoked myogenic potential stimulation for the diagnosis of myasthenia gravis: Optimization of stimulation parameters.

OBJECTIVE: To determine the most effective stimulation parameters for the diagnosis of ocular myasthenia gravis (MG) using repetitive ocular vestibular evoked myogenic potentials (oVEMP) for quantification of the extraocular muscle response decrement. METHODS: Repetitive bone-conducted oVEMPs were elicited in 18 MG patients and 20 healthy subjects. We compared four different stimulus repetition rates (20 Hz, 30 Hz, 40 Hz, 50 Hz) and 100 Hz continuous stimulation, as well as recordings from the inferior oblique muscles and the lateral rectus muscles to determine the most sensitive and specific

Vestibular-Balance Rehabilitation in Patients with Whiplash-Associated Disorders.

BACKGROUND AND OBJECTIVE: Whiplash associated syndrome is one of the neck disorders that is accompanied by several chronic symptoms. Balancing problems arising from common disorders and dizziness are considered as manifestations that are time-consuming to evaluate and treat. The present study aims to investigate the effect of vestibular-balance rehabilitation using a test that can be used for differential diagnosis of these lesions by comparing dizziness handicap inventory (DHI) and Smooth Pursuit Neck Torsion (SPNT) scores. METHOD: This was an analytical cross-sectional study with a two-stag

Vestibular implant: does it really work? A systematic review.

INTRODUCTION: People with vestibular loss present a deficit in the vestibular system, which is primarily responsible for promoting postural control, gaze stabilization, and spatial orientation while the head moves. There is no effective treatment for a bilateral loss of vestibular function. Recently, a vestibular implant was developed for people with bilateral loss of vestibular function to improve this function and, consequently, the quality of life of these patients. OBJECTIVE: To identify in the scientific literature evidence that vestibular implants in people with vestibular deficit impro

[Vestibular tests in infants with TORCH and after CNS infections].

Thorough evaluation of hearing and balance in infants with congenital and perinatal TORCH (ang. Toxoplasmosis, Other, Rubella, Cytomegalovirus, Herpes) infections and after infections of the central nervous system (CNS) is essential due to serious danger of inner ear damage. Caloric stimulation is one of the most reliable diagnostic methods of vestibular testing in infants. Recording of vestibular-evoked myogenic potentials (VEMPs) makes evaluation of the otolith organ and its pathways possible. Potentials resulting from sacculo-collic reflexes are recorded on the surface of the neck muscles.

Observation of vestibular asymmetry in a majority of patients over 50 years with fall-related wrist fractures.

Previously [The abstract has been shortened somewhat, and the headings removed, in order to conform to journal style], we have observed vestibular asymmetry in about one-third of healthy senior citizens and in about two-thirds of subjects with previous hip fractures and no other significant ailments. Wrist fractures are considered a harbinger of hip fractures. If vestibular asymmetry is correlated with falls and fractures among the elderly then it should also be reflected among subjects with wrist fractures. Sixty-six consecutive patients (mean age 67.8 years) who had sustained a fall-related

[Bechterew phenomenon in the human].

BACKGROUND: Vestibuloocular and vestibulospinal reactions following the bilateral loss of vestibular function are designated as Bechterew's phenomenon. In vestibular physiology, under experimental conditions, this phenomenon has played an important role for understanding the mechanisms underlying vestibular compensation. However, clinical reports about this phenomenon in humans are extremely rare. PATIENTS AND METHODS: Bechterew's phenomen was observed throughout every stage of its development in a patient with consecutive bilateral loss of vestibular function. The disorder was documented by

Infralabyrinthine approach to vestibular neurectomy in Menière's disease.

The infralabyrinthine approach to vestibular neurectomy was performed in 9 patients with unilateral Menière's disease. According to the AAOO (1972) criteria, 7 of 9 cases were graded as class B and the remaining 2 cases as class C. Otherwise, according to the AAO-HNS (1985) criteria, 6 patients who could be followed over 2 years were all graded as 'complete' at the vertigo control. The compensation of the spontaneous vestibular signs was rapid in the first 2 postoperative weeks, though an occasional imbalance on movement persisted even 3 years after the operation. No specific caloric reaction

Characteristics of postural stability in patients with aminoglycoside toxicity.

Postural stability in patients with bilateral vestibular deficits from aminoglycoside toxicity was characterized by examining their ability to use different sensory cues to maintain balance and by recording their automatic postural responses to sudden translational and rotational (pitch) perturbations of the support surface. We found our patients had increased sway on sensory tests in which either visual or somatosensory cues were altered and were unable to maintain their balance when both visual and somatosensory cues were altered compared to age-matched normal subjects. The amount of vestibu

A new autosomal dominant syndrome of idiopathic progressive vestibulo-cochlear dysfunction with middle-age onset.

A kindred is described with progressive autosomal dominant vestibulo-cochlear dysfunction resulting in instability in the dark, head movement dependent oscillopsia and hearing loss. The first symptoms appeared in the 4th decade and progressed to vestibular areflexia, presumably in the 5th decade and to almost total deafness in the 6th-7th decade of life. The history was negative for other neurological, otological or infectious diseases, or the use of neuro-ototoxic drugs. The affected subjects showed remarkable compensation for the loss of vestibular function.

Endolymphatic hydrops severity in magnetic resonance imaging evidences disparate vestibular test results.

OBJECTIVES: It has been suggested that in Ménière's disease (MD) a dissociated result in the caloric test (abnormal result) and video head-impulse test (normal result) probably indicates that hydrops affects the membranous labyrinth in the horizontal semicircular canal (HSC). The hypothesis in this study is that based on endolymphatic hydrops' cochleocentric progression, hydrops should also be more severe in the vestibule of these patients than in those for whom both tests are normal. METHODS: 22 consecutive patients with unilateral definite MD were included and classified as NN if both tests

Dizziness.

BACKGROUND: The article gives an overview of the most frequent forms of vertigo, that are of peripheral-labyrinthine, central-vestibular, psychogenic, or physiologic origin. Dizziness or vertigo is a result of a mismatch between 3 sensory systems: the vestibular, the visual, and the somatosensory systems. These systems are mutually interactive and redundant in that orientation and balance are guided by simultaneous reafferent cues. The functional ranges of the systems overlap, thus permitting them to compensate in part for each other's deficiencies. SUMMARY: Vertigo is not a well-defined dise

Human vestibular cortex as identified with caloric stimulation in functional magnetic resonance imaging.

Anatomic and electrophysiological studies in monkeys have yielded a detailed map of cortex areas receiving vestibular afferents. In contrast, comparatively little is known about the cortical representation of the human vestibular system. In this study we applied caloric stimulation and fMRI to further characterize human cortical vestibular areas and to test for hemispheric dominance of vestibular information processing. For caloric vestibular stimulation we used cold nitrogen to avoid susceptibility artifacts induced by water calorics. Right and left side vestibular stimulation was repetitivel

Vestibular schwannoma surgery with cochlear implantation under continuous monitoring using the auditory nerve testing system: Three cases with distinct clinical scenarios.

During the simultaneous resection of vestibular schwannomas and cochlear implantation, acoustic nerve preservation is essential. A recent study highlighted the effectiveness of the Auditory Nerve Test System (ANTS) for evaluating cochlear nerve function following vestibular schwannoma resection and for determining its suitability for cochlear implantation. However, the use of this system for the continuous intraoperative monitoring of acoustic nerve preservation has only been documented in one case in the literature. We recently encountered three distinct clinical cases: a case of neurofibroma

Critical evaluation of the benefits and limitations of foam posturography in vestibular disorders: a narrative review.

This review synthesises current evidence on the clinical utility of foam-based posturography in the assessment of vestibular pathologies. It critically evaluates: (1) the physiological mechanisms underlying foam posturography and its capacity to determine vestibular contributions; (2) methodological variability across testing protocols, including differences in foam properties, stance conditions and measurement parameters; (3) the diagnostic sensitivity, specificity and reliability of foam posturography in various vestibular disorders; (4) its performance relative to other balance and vestibul
