Orienting in a defensive world: mammalian modifications of our evolutionary heritage. A Polyvagal Theory.

The vagus, the 10th cranial nerve, contains pathways that contribute to the regulation of the internal viscera, including the heart. Vagal efferent fibers do not originate in a common brainstem structure. The Polyvagal Theory is introduced to explain the different functions of the two primary medullary source nuclei of the vagus: the nucleus ambiguus (NA) and the dorsal motor nucleus (DMNX). Although vagal pathways from both nuclei terminate on the sinoatrial node, it is argued that the fibers originating in NA are uniquely responsible for respiratory sinus arrhythmia (RSA). Divergent shifts i…

Tension gastrothorax complicating acute traumatic diaphragmatic rupture.

Herniation of abdominal viscera into the thorax following traumatic diaphragmatic hernia can simulate acute tension pneumothorax. A case is presented of a blunt trauma victim with apparent acute diaphragmatic rupture, tension hemothorax, or tension hemopneumothorax. Nasogastric tube insertion demonstrated tension gastrothorax, but was followed by acute clinical decompensation. Percutaneous needle thoracostomy decompressed the stomach without causing spillage of gastric contents. Autopsy experimentation was performed to demonstrate that needle decompression of the distended stomach is well tole…

Reye's syndrome. A review from the forensic viewpoint.

Reye's syndrome, encephalopathy and fatty change in the liver and other viscera, typically occurs suddenly in infants and children recovering from a viral illness, particularly influenza or varicella. Its rapid clinical course may suggest a drug-related insult and the differential diagnosis includes a variety of toxins. There are grounds for suspicion that exogenous substances--including aspirin--may be cofactors with recent viral illness in the syndrome's pathogenesis. For these reasons, medical examiners may be called upon to rule the diagnosis in or out, to assess the possibility of direct …

Muscular and other abnormalities in a case of Edwards' syndrome (18-trisomy).

This paper describes the anatomical variations observed in a specimen exhibiting Edwards' Syndrome (18-trisomy). The clinical and autopsy data are compared with those reported in earlier literature. Aside from having a tracheoesophageal fistula, the viscera were characterized by abnormalities of the heart, lungs, liver and kidneys. The facial musculature was relatively undifferentiated. Only a few abnormalities were recorded in the otomandibular and suprahyoid structures. The infrahyoid region had three pairs of supernumerary muscles, including the "sternohyoideus azygos." The bluk of abnormal…

Abnormal embryogenesis induced by thiopental.

The effects of thiopental on chick embryos were analyzed in the present study. Thiopental was dissolved in saline and injected into embryona … ations observed were exencephaly, anencephaly, twisted limbs, twisted neck, microphthalmia, everted viscera, and hemorrhage above the left eye and in both cerebral hemispheres. The results of the present study indicate that thiopental has a tendency to cause malformations in the chick embryos tested.

Central nervous system defects in early human abortuses.

Thirteen embryos with various central nervous system (cns) defects were examined macroscopically and by serial sectioning, and were compared with seven control embryos. All had been aborted spontaneously. Examples of anencephaly, anencephaly with spina bifida, spina bifida alone, encephalocele, iniencephaly and cyclops were found. Histological assessment was directed mainly to the cns, the skeletal axis, the neural-crest derivatives and remaining viscera. The cns was affected by postmortem change in some cases. Brain tissue was absent in most anencephalic embryos and spinal-cord tissue was abs…

Urologic complications of pelvic irradiation.

We evaluated 964 patients who had undergone radiation therapy for malignant disease of the pelvic viscera during a 5-year period and who ahd been followed for 5 to 10 years after completion of treatment. Therapy had been given for gynecological tumors in 493 cases, genitourinary tumors in 240 cases, bowel tumors in 172 cases and various other malignant tumors in the pelvis in 59 cases. The incidence of cystitis, hematuria, ureteral obstruction and fistula formation was determined for each group. There were 203 urological complications, an incidence of 21 per cent, but only 25 of these complica…

Traumatic Abdominal Wall Hernia With Sigmoid Colon Transection Following Bicycle Handlebar Injury: A Rare Case Report.

Traumatic abdominal wall hernia may occur when a focal blunt force from a bicycle handlebar transmits substantial energy to the abdominal wall and viscera despite minimal external injury. This injury creates a diagnostic challenge, as early clinical findings can be subtle while serious intra‑abdominal injury evolves. We report a case of a 45‑year‑old man who presented with delayed, progressive abdominal pain after low‑velocity handlebar trauma. An early contrast‑enhanced CT was decisive in identifying a large abdominal wall defect with bowel herniation, free air, and suspected colonic perforat…

Lead placement and associated nerve distribution of an implantable periurethral electrostimulator.

The aim of this study was to determine gross and neuroanatomic features of a novel periurethral neuromuscular electrostimulator. Periurethral leads were placed in eight female cadavers. In two cases, leads were imaged after placement to enhance anatomic understanding. Pelvic viscera were removed en bloc for analysis of lead placement in the six remaining cadavers. Excised tissue was sectioned and immunostained to identify general, afferent, sympathetic, and nitric oxide synthase efferent nerve fibers. The electrodes were found within/lateral (n = 4), within/posterolateral (n = 9), and anterola…

Long-term opioid treatment in Behçet's syndrome with intractable abdominal pain--a case report.

Opioids are the most potent and effective analgesics available for treating acute and chronic cancer pain, but its usefulness in treating non-cancer chronic pain is still controversial. We report a 23-year-old male suffering from Behçet's disease with persistent excruciating abdominal pain. Oral non-steroid anti-inflammation analgesics and milder opioids (codeine and tramadol) failed to relieve the pain. His excruciating abdominal pain resulting from ulcerative viscera aggravated with repeated gastrointestinal tract surgeries. Intravenous morphine given through patient-controlled analgesia (PC…

Percutaneous nephrolithotripsy in ectopic kidneys.

BACKGROUND: Percutaneous nephrolithotomy (PCNL), although an accepted treatment modality in anatomically normal kidneys, is still not universally performed for calculi in pelvic ectopic kidneys. Fear of injury to abdominal viscera makes it a technically challenging procedure. PATIENTS AND METHODS: We have performed PCNL in nine patients with calculi in pelvic ectopic kidneys. Technical factors which made this procedure safe include ultrasound-guided puncture, use of a mature tract or an Amplatz sheath, routine postoperative double-J stenting, and nephrostogram prior to nephrostomy tube remova…

Amniotic fluid alpha fetoprotein (AFAFP) and maternal serum alpha fetoprotein (MSAFP) in abdominal pregnancies: correlation with extent and site of placental implantation and clinical implications.

The hypotheses are evaluated that in abdominal pregnancies 1) elevated MSAFP is due to an altered maternal-placental interface, and 2) diffe … karyotype were normal in all three cases. Elevated MSAFP values were associated with more extensive visceral implantation, longer surgical operative time, greater blood loss and transfusion requirements. Abdominal pregnancies with elevated MSAFP appear to have more extensive placental involvement of the abdominal viscera; this would, in fact, account for …

Missed Wooden Perineum Foreign Body Leading to Extensive Necrotizing Soft Tissue Infection: A Case Report.

Penetrating perineal trauma is rare but carries a high risk due to contamination, hidden tracts, and proximity to major vessels and viscera. Retained organic foreign bodies, particularly wood, are often radiolucent and can perpetuate infection or erode into vascular structures. Necrotizing soft tissue infection (NSTI) is a devastating complication with significant mortality. A 53-year-old woman from rural Nepal presented 8 days after falling onto a tree branch, sustaining a perineal impalement. Her wound had been sutured primarily at a local center. She developed severe pain, swelling, foul di…

Cardiac and mediastinum involvement in Burkholderia thailandensis infection: A case report and literature review.

Melioidosis, an infectious disease caused by Burkholderia pseudomallei, is prevalent in Southeast Asia and Northern Australia, presenting various clinical manifestations from asymptomatic to life-threatening infections. Although primarily affecting the lungs, intra-abdominal viscera, and musculoskeletal system, melioidosis can rarely involve the heart and mediastinum, which pose significant diagnostic and therapeutic challenges. Herein, we present the case of a 53-year-old male farmer who presented with persistent fever and chest pain, progressing to pericarditis and cardiac tamponade. Imaging…

Recurrent Nausea and Vomiting with Weight Loss Associated with Hypothyroidism: Fact or Myth.

BACKGROUND: Hypothyroidism is a commonly encountered endocrine disorder presenting in various clinical settings. It usually presents with classic manifestations, which are readily recognized and, therefore, easy to diagnose. However, occasionally, patients present with unusual symptoms, which becomes a challenge to diagnose. Thyroid dysfunction affects many body organs, including the gut and viscera. Studies show that intestinal motility might be affected by multiple factors, such as neuromuscular dysfunction, myopathy, or alterations in hormone receptors. CASE PRESENTATION: Here, we present …
