Sensitivity and specificity in methods for examination of the eye astigmatism.

PURPOSE: We usually use objective and subjective methods for examination of the eye astigmatism in optometry, respectively ophthalmology. Objective methods enable to measure sphere-cylindrical refraction of the eye. If we want to prescribe new glasses or contact lenses we usually use subjective methods. The aim of this study was to measure sensitivity and specificity of some subjective and objective methods for examination of the eye astigmatism. We supposed that automatic objective refraction will be the most exact method so we choose this method as the reference method. For comparison we cho

Is King-Devick Testing, Compared With Other Sideline Screening Tests, Superior for the Assessment of Sports-related Concussion?: A Critically Appraised Topic.

BACKGROUND: Concussion affects almost 4 million individuals annually. There are many sideline screening tools available to assist in the detection of sports-related concussion. The King-Devick (K-D) test in association with Mayo Clinic utilizes rapid number naming to test saccadic eye movements in order to screen for concussion. An ideal screening tool for concussion would correctly identify all athletes with active concussion. The accuracy of K-D testing compared with other sideline screening tools is undetermined. OBJECTIVE: To critically assess current evidence regarding the utility of K-D

[New visual field testing possibilities (a preliminary report)].

UNLABELLED: There are currently no portable mobile perimeters that allow visual field testing outside ophthalmologist's examination rooms. PURPOSE: To develop a mobile perimetry technique based on use of a virtual reality headset (VR). MATERIAL AND METHODS: The study involved 26 patients (30 eyes) with II-III stage primary open-angle glaucoma (POAG) with compensated IOP. Perimetry was performed for each patient twice - on Humphrey analyzer (test 30-2, 76 points) and employing similar strategy on a perimeter integrated into VR headset (Total Vision, Russia). Visual field testing was performed

Unilateral Amblyopia Affects Two Eyes: Fellow Eye Deficits in Amblyopia.

Unilateral amblyopia is a visual disorder that arises after selective disruption of visual input to one eye during critical periods of development. In the clinic, amblyopia is understood as poor visual acuity in an eye that was deprived of pattern vision early in life. By its nature, however, amblyopia has an adverse effect on the development of a binocular visual system and the interactions between signals from two eyes. Visual functions aside from visual acuity are impacted, and many studies have indicated compromised sensitivity in the fellow eye even though it demonstrates normal visual ac

Introduction to microperimetry and its use in analysis of geographic atrophy in age-related macular degeneration.

PURPOSE OF REVIEW: This article discusses recent advances in the fundus-guided perimetry (microperimetry) and its utilization in evaluation and monitoring of patients with geographic atrophy. RECENT FINDINGS: Although best-corrected visual acuity has been gold standard in clinical practice for decades, it does not provide an entire assessment of visual function that determines daily activity and quality of life of a patient. Furthermore, psychophysical tests, including low-luminance visual acuity, reading speed, and contrast sensitivity, cannot be used to quantify retinal sensitivity or detec

Visual form-processing deficits: a global clinical classification.

INTRODUCTION: Patients who have difficulties recognising visual form stimuli are usually labelled as having visual agnosia. However, recent studies let us identify different clinical manifestations corresponding to discrete diagnostic entities which reflect a variety of deficits along the continuum of cortical visual processing. DEVELOPMENT: We reviewed different clinical cases published in medical literature as well as proposals for classifying deficits in order to provide a global perspective of the subject. Here, we present the main findings on the neuroanatomical basis of visual form proc

Visual impact of Zernike and Seidel forms of monochromatic aberrations.

PURPOSE: The aim of this study was to examine the impact of different aberrations modes (e.g., coma, astigmatism, spherical aberration [SA]) and different aberration basis functions (Zernike or Seidel) on visual acuity (VA). METHODS: Computational optics was used to generate retinal images degraded by either the Zernike or Seidel forms of second through fourth-order aberrations for an eye with a 5-mm pupil diameter. High contrast, photopic VA was measured using method of constant stimuli for letters displayed on a computer-controlled, linearized, quasimonochromatic (lambda = 556 nm) display.

Optic neuritis--more than a loss of vision.

BACKGROUND: Optic neuritis is an acute inflammation of the optic nerve that results in painful loss of vision. Patients often present to a general practitioner, and early recognition is important as treatment may improve the speed of vision recovery. OBJECTIVE: This article provides information on the signs and symptoms of optic neuritis and discusses appropriate referral, investigations and management. DISCUSSION: Optic neuritis is the presenting symptom in up to one-fifth of people with multiple sclerosis. Diagnosis of optic neuritis is based on history and examination, therefore obtaining

Objective accommodation measurement with the Grand Seiko and Hartinger coincidence refractometer.

PURPOSE: Subjective push-up tests and dynamic retinoscopy are standard clinical accommodation tests. These are inadequate for assessing if accommodation can be restored in presbyopes. Commercially available clinical autorefractors offer potentially reliable methods for objective accommodation measurement. This study evaluated accuracy and reliability of the Grand Seiko WR-5100K autorefractor for objective accommodation measurement in young adults. METHODS: Twenty-two subjects, aged 21 to 30 years (mean 25.6 +/- 2.26) participated. Three methods were used to stimulate and measure accommodation

The psychophysics of glaucoma: improving the structure/function relationship.

Perimetry of some kind remains an important tool in the detection, diagnosis and monitoring of glaucomatous damage to the visual pathway. However, recent studies have served to reinforce the suspicion that conventional perimetry does not possess the sensitivity to detect the earliest signs of functional loss resulting from glaucoma. The relationship between differential light threshold and ganglion cell loss is extremely weak and, in the early stages of glaucoma, non-existent. Alternative, more novel perimetric techniques seem to offer promise of better detectability for early loss by claiming

[A probable case of Creutzfeldt-Jakob disease with electroretinogram alterations].

Although visual disturbance is recognized as a clinical feature of Creutzfeldt-Jakob disease (CJD), lesions within the retina have to date, recorded little attention. In this single case study, we report a case of retinopathy observed in a 72-year-old female patient with CJD. The patient admitted to the hospital complaining of a 2-month history of visual disturbance, dysarthria, and gait disorder. Electroencephalogram showed periodic synchronous discharges synchronizing with myoclonic jerks. Electroretinogram (ERG) using single bright flash in a dark-adapted state demonstrated the absence of b

[Epidemiology of refraction abnormalities in Northern Navy young soldiers].

Screening ophthalmological studies were carried out in 2851 soldiers arriving to Northern Navy, in order to detect ocular diseases, refraction abnormalities, and functional disorders. Visual disorders were detected in 529 subjects (19%), refraction abnormalities in 87% of these. Myopia predominated (344 subjects, 66%) in the structure of ophthalmological disorders. Hypermetropia was detected in 114 patients (21%), anisometropia in 85 (16%). Twelve percent of all examinees were in need of optic correction. Epidemiological analysis showed an obvious increase in the prevalence of myopia from Sout

Contrast sensitivity in multiple sclerosis. A study in 35 patients with and without optic neuritis.

Binocular and monocular contrast sensitivity (CS) functions were determined for 35 patients with multiple sclerosis (MS) of whom only 21 had a history of optic neuritis. CS was abnormal in all 21 of these patients; alterations of CS affected all spatial frequencies, not only for the eye with optic neuritis, but also for the clinically unaffected eye (p less than 0.01). Approximately one third of the 14 patients with MS but without any history of past visual disturbances showed an optic nerve involvement. Our results show that (1) clinically visual impairment is bilateral, even if optic neuriti

A comparative study of visual field defects seen in patients with low-tension glaucoma and chronic simple glaucoma.

In a study comparing the visual field defects of patients with chronic simple and low-tension glaucoma differences in the nature of the defect were found between the 2 groups. Patients with low-tension glaucoma had far 'steeper sided' defects than those seen in chronic simple glaucoma (p less than 0.01).

Cataract update.

Cataracts can be congenital or can result from traumatic injuries, local or systemic disease, or the aging process (senile cataracts). Senile cataracts are classified as nuclear, cortical or posterior subcapsular. Of the several surgical procedures and lenses used in treatment, the most exciting advance is extracapsular cataract extraction followed by insertion of a posterior chamber intraocular lens. Physicians should be aware of several common misconceptions concerning cataracts.

Monocular spatial distortion in strabismic amblyopia.

We examined monocular spatial vision of strabismic amblyopes by measuring errors of relative directionalization (specifying whether or not two targets are in vertical alignment) and partitioning (equating left- and right-field spaces). Abnormally large errors were made when fixation occurred with the amblyopic eye; these errors did not attributable to reduced acuity, unsteady fixation, or eccentric fixation. From the results we infer that monocular space perception of strabismic amblyopic eyes is severely distorted and is characterized by "bending" of vertical lines of direction and by local "
